Priapism is a rare but serious adverse effect of psychotropic drugs resulting from their peripheral alpha-adrenoreceptor blocking action. Two patients developed this during treatment with oral phenothiazines: one was treated surgically with a venous shunt, and the second case resolved after intracavernosal injection of an alpha-adrenoreceptor stimulating drug.

Priapism is known from egyptian antiquity, but greek doctors give the name of god Priape, to this pathology. The physiopathology is controversial, between Paré and the authors of Middle age tradition medical. The treatment is at first the use of quack medicines, and the Velpeau used surgery.

The authors analyze 35 cases of priapism treated in the University Hospital of Caracas and conclude that this condition is a surgical emergency because the long-term results in regard to the preservation of the erectile function seem to be directly related to early drainage of the corpora cavernosa. We prefer shunting operations, particularly the glans-cavernous techniques, because of their technical simplicity.

We herein review the last 29 patients with priapism treated at our hospitals with respect to the physiology and pathophysiology of erection and priapism, respectively, and present a rational approach to the management of priapism. Emphasis is given to the corpus cavernosum to corpus spongiosum shunt with a vascular anastomosis technique.

A case of priapism in a child with chronic granulocytic leukemia is presented, and the English literature is reviewed. While common in adult leukemia patients, priapism is rare in children with these neoplasms. Sludging of blood in the corpora cavernosa is almost universally accepted as the mechanism behind the development of painful and persistent erection in these patients. Permanent fibrosis of the cavernosal tissue may not occur in leukemic priapism as it does in other than chronic granulocytic leukemia. Therapy directed toward the priapism is almost uniformly unsuccessful. However, the pa…

Priapism is a surgical emergency, and a glandular cavernous shunt is good emergency treatment. The technique is simple, easy and quick, can be performed under local anaesthesia if necessary, and has a good chance of success; should it fail or the priapism recur, a more formidable shunt may be performed.

Priapism can occur when pathologic stimuli (e.g. lesions of the cervical spinal cord) cause prolonged erection, and when normal stimuli under pathologic circumstances (e.g. sickle cell disease, leukemia) produce prolonged erection. As any priapism eventually leads to permanent impotentia coeundi, immediate therapy is absolutely indicated. All unspecific procedures are useless and merely waste time. The priapism in connection with hematologic diseases must be countered by conservative specific therapy (sickle cell disease by blood transfusions; leukemias by cytostatic drugs). All other forms of…
