Notes on Interpretation and Conversation in Clinical Work.

The paper delves into the dynamic interplay between interpretation and conversation within the frame of post-Bionian field theory. It highlights the crucial task of discerning whether the emotional atmosphere, the quality of the therapeutic bond, or the "basic assumption" that is active in the analytic dyad foster or hinder psychic growth. It underscores the significance of "becoming" and emotional resonance, as elucidated by Bion's concept of "at-one-ment," over mere intellectual accord and factual knowledge. The pivotal moment of interpretation hinges more on the analyst's "silent" receptivi

Intrastellar: An Exploration of Nonlocal Unconscious Communication in the Quantum Field of Analysis.

By presenting an apparently telepathic experience as a possible manifestation of nonlocal field dynamics rather than as a supernatural event, this article broadens psychoanalytic conceptions of nonlocal unconscious communication, particularly in the way it may emerge during moments of intense emotional resonance between patient and analyst. By placing psychoanalysis in dialogue with contemporary scientific perspectives, this study seeks to foster interdisciplinary reflection while maintaining clinical sensitivity, theoretical rigor, and openness toward phenomena that challenge the assumptions

Unconscious affective communication in the formation and transformation of emerging subjectivity.

The author addresses "primitive states of mind", a significant point of current development in contemporary psychoanalysis, with the aim of recognising and elaborating such configurations in the analytic relationship. The article revisits the views of Loewald (1980), who saw minds as organising experiences gradually over the course of development according to different principles and changeable organisational structures which continue to operate simultaneously in adult experience on a continuum that runs from consciousness to unconsciousness. The author uses a clinical example to describe vari

Women's experiences of discussing health behaviours within their maternity care: a systematic review and meta-synthesis.

INTRODUCTION: Health behaviours are important because they have been linked to pregnancy outcomes. During maternity care women are provided with health information, but little is known about how women experience these discussions. Therefore, a systematic review was conducted to investigate women's experiences of discussing health behaviours within their maternity care interactions. METHODS: Bibliographic databases were searched to retrieve studies reporting qualitative data regarding women's experiences. Included records were assessed for quality using the Critical Appraisal Skills Programme

Exploring therapist effectiveness across multiple domains over time.

Despite the common assumption that increased therapist experience correlates with improved client outcomes, studies reveal inconsistent relationships between therapist experience and therapy effectiveness. Some studies have demonstrated a slight worsening of client outcomes as therapists gain more experience. Yet, there are few studies that have examined how therapists change in their client outcomes over time, especially across different outcomes (e.g., depression, life functioning, anxiety). The present study used the Behavioral Health Measure-17 to investigate therapist effectiveness over t

Complexities of interprofessional identity formation in dental hygienists: an exploratory case study.

BACKGROUND: In a super-aging society, medical-dental collaboration is increasingly vital for comprehensive patient care. Particularly in dysphagia rehabilitation and perioperative oral functional management, dental hygienists' active involvement is pivotal to interprofessional collaborative practice. Despite this societal expectation, dental hygienists' experiences and perceptions of interprofessional collaboration have not been explored. This study aims to investigate dental hygienists' interprofessional identity formation and perceptions of interprofessional collaboration. Specifically, it w

The myth of the safe space: Sexual orientation disparities in therapist effectiveness.

Psychotherapy has been shown to be effective on a broad level (e.g., Wampold & Imel, 2015); however, a growing body of literature has revealed that some therapists have outcome inequities within their caseloads. These inequities have been observed on the basis of social identities including race (see Imel et al., 2011, for example) and gender measured on the binary (Owen et al., 2009). However, despite the great need for further research on sexual minority populations in psychotherapy, this phenomenon has yet to be explored on the basis of sexual orientation (i.e., if a disparity exists within

Hand movements that change during psychotherapy and their relation to therapeutic outcome: An analysis of individual and simultaneous movements.

Hands are, compared to other body parts, the body parts that display the most gestural movements during an interaction and their movement is sensitive to reveal anxiety states. However, psychotherapy research focuses on movement synchrony of different body parts to be an indicator of improved symptoms. The present study investigates symptoms in social anxiety disorder and considers both, individual and simultaneous hand movements. 56 video recordings of 28 patient-therapist dyads with patients with social anxiety at the beginning and in the end of psychotherapy were analyzed. Two independent b

Enlightened Self-interest in Altruism (ESIA).

Altruism and the medical profession have been linked throughout the history of medicine. Students are drawn to the calling of medicine because of altruistic values, dedication to service, and the desire to alleviate suffering and promote healing. Despite a dedication to these values, altruism in medicine is threatened by empathy erosion that develops in the clinical years of medical school and an increasing rate of medical student burnout. Currently, there are two widespread movements in medicine aimed at addressing the dual loss of altruism and physician and student burnout-professionalism an

Thoughts on the limits of a mutual technique.

Ferenczi's appreciation of the inherently mutual nature of the analytic encounter led him, and many who followed, to explore the value of mutual openness between patient and analyst. Specifically, Ferenczi saw the analyst's openness as an antidote to his earlier defensive denial of his failings and ambivalence toward the patient, which had undermined his patient's trust. My own view is that, while the analyst's openness with the patient can indeed help reestablish trust and restore a productive analytic process in the short term, it also poses long-term dangers. In certain treatments it may en

The experiences of staff who support people with intellectual disability on issues about death, dying and bereavement: A metasynthesis.

BACKGROUND: Historically, people with intellectual disabilities have tended to be excluded from knowing about death, dying and bereavement. Staff in intellectual disability services can play a valuable role in improving understanding of these issues in those they support. This qualitative metasynthesis aimed to understand the experiences of staff supporting adults with intellectual disabilities with issues of death, dying and bereavement. METHOD: Thirteen papers were identified following a systematic review of six databases. RESULTS: Three themes were developed following a lines-of-argument

Assimilation, reflexivity, and therapist responsiveness in group psychotherapy for social phobia: A case study.

OBJECTIVE: This case study examined reflexivity and the assimilation of problematic experiences, especially its progress within and between the Assimilation of Problematic Experiences Scale (APES) Stages 2-3, in group psychotherapy for social phobia. METHOD: The data consisted of all of one client's turns expressing the two voices of her main problematic experience in 12 sessions, and all replies by the therapist in direct connection to them. The client's utterances were rated on the APES. RESULTS: A detailed analysis of 13 conversational passages revealed that progress in assimilation happe

[PROPOSAL OF AN OBSERVATION GRID TO DECRYPT CAPABILITIES. AN ILLUSTRATION IN AN ORGANIZATION FOR ALZHEIMER'S PATIENTS].

The capability approach (Sen, 2000) is a relevant theoretical choice to be interested in the lives of people with disabilities and/or loss of independence. However, this promising approach to social problems remain shaded by methodological problems. Thus, although its founder emphasizes the need to understand the meaning that the actors give their lives and their choices in the environment that is theirs, there is clearly rooted in a positivist approach, objectivist, quantitatively. This article's objective is to present a methodology for observation grid to overcome this pitfall. Its deployme

Critical transference in a case of severe epilepsy.

In this paper, the author presents a summary of a brief treatment with a young woman, suffering from severe epileptic illness, which describes the mutuality of cross-transferences of deep and intense feelings and the unorthodox way to receive, to take in persons affected with critical phenomena. This treatment was deeply influenced by the traumatic triad, borrowed from Ferenczi: "amnesia, anaesthesia, aboulia," the ways of forgetting (or erasing) the past/the present/the future. The investigation led us to clarify, on the one hand, the incestophilic--or perhaps incestuous--source in the family

The Caring Professionals Program: educational approaches that integrate caring attitudes and empathic behaviors into health professions education.

Caring attitudes and empathic behaviors are considered by most Americans to be an essential and intrinsic element of appropriate health care, yet little attention is given to this in the curricula of most healthcare professional training programs. This paper describes an ongoing educational intervention to develop healthcare professionals with caring attitudes and empathic behaviors that will be sustained in their professional practice environments. The Caring Professionals Program was designed to enhance and redesign existing learning experiences in four academic programs: physical therapy, o

Exposed and embedded corrections in aphasia therapy: issues of voice and identity.

BACKGROUND: Because communication after the onset of aphasia can be fraught with errors, therapist corrections are pervasive in therapy for aphasia. Although corrections are designed to improve the accuracy of communication, some corrections can have social and emotional consequences during interactions. That is, exposure of errors can potentially silence the 'voice' of a speaker by orienting to an utterance as unacceptable. Although corrections can marginalize speakers with aphasia, the practice has not been widely investigated. AIMS: A qualitative study of corrections during aphasia therapy

Therapist alliance-building behavior within a cognitive-behavioral treatment for anxiety in youth.

Explored the specific behavior of therapists contributing to a child client's perception of a therapeutic alliance with youth (n = 56) who received a manualized cognitive-behavioral treatment for anxiety disorders. The first 3 sessions were coded for 11 therapist behaviors hypothesized to predict ratings of alliance. Child, therapist, and observer alliance ratings were gathered after the 3rd and 7th therapy sessions. "Collaboration" positively predicted early child ratings of alliance, and "finding common ground" and "pushing the child to talk" negatively predicted early child ratings of allia
