A subglottic primary extramedullary plasmacytoma (typ IgA-Lambda) of the larynx is reported. These tumours are very rare. The diagnosis is made more difficult by unspecific symptoms and can only be confirmed by histopathology. Early diagnosis and differentiation between primary extramedullary plasmacytoma and metastasis from a multiple myeloma are very important for the prognosis of the disease. Fifty patients bilaterally adrenalectomized for Cushing's disease were followed for 1 to 22 (mean, 12) years. In 14 of them (28%) Nelson's syndrome appeared within 1.5 to 12 (mean, 4.8) years after adrenalectomy. All the patients were deeply pigmented, 12 had a radiologically demonstrable tumour and six had visual defects. Plasma ACTH levels ranged from 450 to 8000 ng/l. However in every case at least one estimation during circadian studies equalled 2000 ng/l. One patient with an anaplastic pituitary tumour died 3 years after the discovery of the tumour. In a study to assess the natural history of giant-cell arteritis, 90 patients with proved disease were followed up from the time of diagnosis. Early mortality was low and most commonly due to vertebral arteritis, but cerebral infarction did not appear to be a late complication. High maintenance dose steroids and visual loss were associated significantly with a shortened life span (p=0.0003 and p=0.0024). Surgical results in 30 cases of traumatic cataracts using mainly aspiration and extraction techniques are reported. A corrected visual acuity of 6/12 or better was achieved in 64.3% of cases. Three eyes had light perception only. Early surgery using aspiration technique followed by contact lens correction is recommended for traumatic cataracts.

The true prognosis of JRA is unknown. The best interpretation of reports to this date may be that at any given time of examination between 5 and 15 years after onset, 30-50% of children will have grossly active disease and that 70-90% of patients will be in class I-II functional status. Prognosis depends on establishing a relationship between the patient's state in the early stages and the ultimate outcome.
