Propionibacterium acnes induction of inflammatory responses is a major etiological factor contributing to the pathogenesis of acne vulgaris. In particular, the IL-1 family of cytokines has a critical role in both initiation of acne lesions and in the inflammatory response in acne. BACKGROUND: Inflammation is commonly observed in radical prostatectomy specimens, and evidence suggests that inflammation may contribute to prostate carcinogenesis. Multiple microorganisms have been implicated in serving as a stimulus for prostatic inflammation. The pro-inflammatory anaerobe, Propionibacterium acnes, is ubiquitously found on human skin and is associated with the skin disease acne vulgaris. Malignant melanoma (MM) is an aggressive cutaneous malignancy associated with poor prognosis; many putatively therapeutic agents have been administered, but with mostly unsuccessful results. Of 522 patients with Propionibacterium acnes bacteremia (PAB), 18 (3.5%) had clinically significant PAB. Of these 18 patients, 10 (55.6%) had hospital-acquired bacteremia and 6 (33.3%) had undergone invasive procedures before development of PAB. One patient with a ventricular septal defect presented with infective endocarditis. After the exclusion of 1 patient whose outcome was not available, the overall mortality rate was 5.9% (1/17).

In a randomized, double-blind, placebo-controlled study, dogs with chronic recurrent pyoderma were treated with antibiotics plus intravenous injections of either Propionibacterium acnes or placebo. Responses (an increase, decrease, or clearing of lesions) were measured and evaluated statistically. Eighty percent (12 of 15) of the dogs treated with antibiotics and P acnes compared with 38% (five of 13) of the dogs treated with antibiotics and placebo responded with significant improvement or complete remission of lesions at the end of the 12-week treatment schedule (P less than 0.05).

The influence of B12-CN, B12-OH, coenzyme B12, factor III and factor B on the synthesis of vitamin B12 and porphyrins by different strains of P. shermanii was investigated. Neither compound inhibited the development of propionic bacteria or suppressed porphyrin formation. All nucleotide containing analogues of vitamin B12 produced a strong repressive effect on the synthesis of corrinoid compounds regardless of the modifications in the upper and lower cobalt ligands. Insights into the immune mechanisms at play in skin diseases including acne continue to be made. In this issue, Kistowska et al. confirm that Propionibacterium acnes activates inflammasomes leading to the production of IL-1β and they further identify putative mechanisms by which this process occurs. These data raise interesting questions regarding a multipronged approach by which P. acnes elicits inflammation in early versus late acne and putative differences in the effects of IL-1α and IL-1β in this disease process.
