Quadricuspid Pulmonary Valve With Fenestration: Cadaveric Findings.

Quadricuspid pulmonary valves (QPV) are rare entities. Such valves can be associated with other cardiac anatomical anomalies. In this report, we present a case of a quadricuspid valve with an additional variant and discuss the morphometrics of this anatomical variation. During the routine dissection of an adult male body, two anatomical variations were found within the pulmonary trunk. This individual had a QPV. In addition, one of the leaflets of this valve contained fenestrations. No additional cardiac anomalies were identified. Clinicians who review imaging of the heart or treat patients wi…

Left axillary approach for pulmonary valve replacement.

A 14-year-old girl was scheduled for pulmonary valve replacement. A computed tomography scan showed an enlarged cardiac silhouette with an aneurysmal pulmonary artery. A less-invasive approach through the left axilla with peripheral cannulation was selected. The patient was draped in the decubitus position, with a roll under the left shoulder and the left arm over the head. The anatomical landmarks were the left nipple and the tip of the scapula. A 5-cm vertical incision in the mid-axillary line was performed, and the thorax was entered through the fourth intercostal space. Peripheral cannulat…

Transcatheter pulmonary valve replacement in pediatric patients.

INTRODUCTION: Right ventricular outflow tract (RVOT) dysfunction is common among individuals with congenital heart disease (CHD). Surgical intervention often carries prohibitive risks due to the need for sequential pulmonary valve (PV) replacements throughout their life in the majority of cases. Transcatheter pulmonary valve replacement (tPVR) is one of the most exciting recent developments in the treatment of CHD and has evolved to become an attractive alternative to surgery in patients with RVOT dysfunction. AREAS COVERED: In this review, we examine the pathophysiology of RVOT dysfunction, …

Unmasking pulmonary hypertension following tricuspid valve replacement.

In this Viewpoint, we highlight a possible hemodynamic problem arising following tricuspid valve replacement (TVR) in patients with severe chronic tricuspid regurgitation, represented by "unmasking" of pulmonary hypertension (PH) following the surgery. We share an observation that should alert cardiologists to the fact that this increasingly utilized surgery is not risk free, and careful assessment of the right ventricular function and pulmonary circulation preoperatively is extremely important, especially in patients with preexisting risk factors for PH, since TVR may lead to a sudden increas

Percutaneous Pulmonary Valve Implantation Alters Electrophysiologic Substrate.

BACKGROUND: Percutaneous pulmonary valve implantation (PPVI) is first-line therapy for some congenital heart disease patients with right ventricular outflow tract dysfunction. The hemodynamics improvements after PPVI are well documented, but little is known about its effects on the electrophysiologic substrate. The objective of this study is to assess the short- and medium-term electrophysiologic substrate changes and elucidate postprocedure arrhythmias. METHODS AND RESULTS: A retrospective chart review of patients undergoing PPVI from May 2010 to April 2015 was performed. A total of 106 pati…

Extended application of percutaneous pulmonary valve implantation.

OBJECTIVES: This study was designed to report a novel indication for percutaneous pulmonary valve implantation in patients with previous right ventricular outflow tract (RVOT) patch. BACKGROUND: Current indications for percutaneous pulmonary valve implantation are limited to patients who had pulmonary valve stenosis and/or regurgitation in a right ventricle-to-pulmonary artery conduit. Percutaneous pulmonary valve implantation has not been previously reported in patients with severe pulmonary valve regurgitation following repair of tetralogy of Fallot (TOF) using RVOT patch. METHODS: After a…

Current experience with percutaneous pulmonary valve implantation.

Transcatheter valve replacement has recently been introduced into clinical practice and has the potential to transform the management of valvular heart disease. To date, the largest human experience exists with percutaneous pulmonary valve implantation in patients with repaired congenital heart disease who require re-intervention to the right ventricular outflow tract. The application of this approach, however, is presently restricted to certain right ventricular outflow tract morphologies, because the device needs to be anchored safely to prevent device dislodgement. Early results of percutan…

Advances in experimental percutaneous pulmonary valve replacement.

BACKGROUND: Percutaneous pulmonary valve implantation is emerging as an alternative and additional option for a successful surgical scheme. To date, these procedures are performed by the balloon-in-balloon technique. The use of self-expanding stents for percutaneous valve replacement is assumed to improve preservation of the valve in its folded condition in the application device and the valve's long-term functioning. Therefore, initial experience with the development of a completely percutaneous transfemoral technique for pulmonary valve implantation using a self-expanding valved stent is des…

Aortic valve replacement with cryopreserved pulmonary allograft.

Experimental as well as clinical data support the finding of using the pulmonary valve as a systemic semilunar valve. From January 1990 to December 1992, 89 patients received pulmonary cryopreserved allografts as aortic valve replacement. Sixteen subjects suffering from native or prosthetic valve endocarditis were included. Age ranged from 10 to 74 years. Sixty-five patients received isolated aortic valve replacement and 24 patients required additional surgical measures, such as coronary artery bypass graft, mitral valve reconstruction, replacement of the ascending aorta, supravalvular patch a…

[Percutaneous transluminal valvuloplasty in pulmonary valve stenosis].

Forty five patients with moderate or severe pulmonary valvular stenosis had been treated by percutaneous balloon valvuloplasty. The age of these patients varied between 55 days and 24 years. Nine patients (group 1) had a right ventricular/pulmonary artery gradient (RV/PA) between 25 and 50 mmHg, thirty six patients (group 2) had a RV/PA gradient superior to 50 mmHg. Fourty nine dilatations had been practised. The right ventricular pressure failed from 106.31 +/- 46.03 mmHg to 61.11 +/- 32.62 mmHg. The RV/PA gradient passed from 86.02 +/- 45.64 mmHg to 39.58 +/- 32.16 mmHg and the ratio of righ

Intraoperative balloon valvuloplasty in pulmonary valve stenosis.

Balloon valvuloplasty was used in the operating room on 7 patients to visually assess the valvular changes induced by inflation of a balloon catheter. All patients had typical pulmonary valve stenosis and an associated cardiac condition which necessitated surgery. Of 23 fused commissures present, 21 were successfully opened with a single balloon inflation. In 2 patients, damage to the leaflets was observed. In 1 patient there was partial detachment and in the other a small tear was noted at the margin of one leaflet. These intraoperative results may be similar to the results obtained with perc…

[Balloon valvuloplasty of congenital pulmonary valve stenosis].

Percutaneous balloon valvuloplasty can be considered a therapeutic alternative to surgery for pulmonary valve stenosis. In this analysis, our results will be reported with respect to the various balloon sizes as related to the diameter of the valvular ring as well as differing balloon catheters and the presence of other anomalies in addition to pulmonary valve stenosis. Balloon valvuloplasty was carried out in 23 infants and children ranging in age from one month to eleven years and in two adults at the age of 22 and 50 years, respectively, with a systolic pressure gradient across the pulmonar…
