Pylorus preserving pancreatoduodenectomy for gastrinoma. A case report.

A case of a young female patient affected by Zollinger-Ellison syndrome with occult gastrinoma during exploratory laparotomy, is reported. On the basis of transhepatic portal catheterization the patient underwent pylorus preserving pancreatoduodenectomy according to Longmire-Traverso. A malignant gastrinoma of the duodenal wall, 2 mm in diameter, was observed on the operative specimen in the suspected gastrinogenic area. The procedure led to complete clinical recovery of the patient who did not respond to the medical treatment of the ulcer. The prognosis was good with oncologic radicality of s…

Pylorus-preserving pancreatoduodenectomy. A clinical and physiologic appraisal.

Since 1978, 252 patients from different centers in the world have undergone pylorus-preserving pancreatoduodenectomy. Fifty-five per cent of the patients had malignant tumors in the region of the head of the pancreas. The overall operative mortality rate was 2.8%. Anastomotic leakage and fistulae occurred in 19% of the patients. Pancreatic, biliary, and enteric fistulae represented 11%, 4%, and 4%, respectively. Peptic ulcers were subsequently diagnosed in seven patients (3%), two of whom required vagotomy and antrectomy. Delayed recovery of gastric function was the most common complication of…

The clinical significance and natural history of the double pylorus.

The double pylorus is a fistulous communication between the gastric antrum and duodenal bulb. In most instances, it is believed to be a complication of peptic ulcer disease. In prior reports, the longest follow-up has been only 5 years. We report four cases of double pylorus, three of which have been followed medically (one for 7 years), and a fourth which required surgery for recurrent pain after 10 years of medical treatment. In previously reported cases, only 20% have required surgery, usually for refractory symptoms, whereas 80% of patients have been managed medically and have been free of…

[Pathogenesis of atresia of the pylorus in patients with epidermolysis bullosa].

A new case of pyloric atresia associated with epidermolysis bullosa was observed. To our knowledge seventeen other cases of this condition have been previously reported, five of these also presented aplasia cutis congenita. Although there is no clear agreement as to which type of epidermolysis is associated, junctional bullous epidermatosis is the most commonly reported form. This is unlikely to be a chance association, since both are extremely rare and autosomal recessive hereditary conditions. Some authors have proposed a pleiotropic effect of a single gene or autosomal recessive, with close

Postvagotomy diarrhoea and dumping treated with reconstruction of the pylorus.

The results of reconstruction of the pylorus in 12 patients with disabling diarrhoea and/or dumping after vagotomy and pyloroplasty are reported. Eight patients, primarily operated on with a truncal vagotomy and pyloroplasty, all indicated frequent diarrhoea as their principal symptom. After the reconstruction operation the stools were normalized in five, and the frequency of diarrhoea was reduced considerably in two patients. Three of four patients who had had a selective vagotomy and pyloroplasty complained of severe dumping after all kinds of food; after the reconstruction these symptoms we…

Preservation of the pylorus in pancreaticoduodenectomy a follow-up evaluation.

We have previously reported our efforts to minimize postgastrectomy symptoms in two patients with benign disease who underwent resection of the head of the pancreas and the duodenum. In these cases the pylorus and first portion of the duodenum were preserved during pancreaticoduodenectomy. Our experience has now been extended to encompass 18 patients, eight of whom were available for comprehensive evaluation an average of six months postoperation. These studies have attempted to differentiate malabsorption of pancreatic insufficiency from possible gastrointestinal dysfunction of the new alimen…

[Gastric resection with preservation of the pylorus with a variant of pyloroplasty].

The authors present the case of a female patient with gastric ulcer of the small curvature of the stomach who showed a particular neuropsychical lability, in whom emergency surgery was performed before it was possible to obtain data concerning the behaviour of the gastric acidity. In an attempt to avoid the postoperative dumping syndrome the authors performed a medio-fastric resection, removing the area with the ulcer and maintaining the antro-pylorus. For the obtention of a good drainage of the remaining stomach a variant of the pyloroplastia technique was associated.

[Nutritional Status after Laparoscopic-Assisted Pylorus-Preserving Gastrectomy].

PATIENTS AND METHODS: Patients with gastric cancer who underwent laparoscopic-assisted pylorus-preserving gastrectomy (LAPPG group)or laparoscopic-assisted distal gastrectomy(LADG group)between January 2010 and December 2019 were reviewed and their postoperative nutritional status and long-term outcomes retrospectively evaluated. RESULTS: In total, 83 patients(LAPPG group, n=23; LADG group, n=60)were included. Weight loss rates 1, 6, 12, and 24 months postoperatively in the LAPPG and LADG groups were 5.7% and 7.1%, 6.6% and 9.6%, 5.8% and 10.1%, and 5.2% and 8.7%, respectively. The LADG group…

Stomach Intestinal Pylorus-Sparing Surgery for Morbid Obesity.

BACKGROUND AND OBJECTIVE: The Roux-en-Y duodenal switch (RYDS) is one of the most efficient forms of bariatric surgery. However, diarrhea, malnutrition, ulcers, and internal hernias have hampered its widespread adoption. The stomach intestinal pylorus-sparing (SIPS) procedure was developed to alleviate these sequelae while retaining the same weight loss as the RYDS. In this study, we report our midterm experience with this novel technique. METHODS: Retrospective analysis was performed on data from 225 patients who underwent a primary SIPS procedure by 2 surgeons at a single center from Octobe…

Pylorus-preserving pancreaticoduodenectomy in patients undergoing chronic hemodialysis.

This report describes three patients who underwent a pylorus-preserving pancreaticoduodenectomy (PpPD) and received maintenance hemodialysis due to chronic renal failure. The three cases were diagnosed to have bile duct cancer, intraductal papillary mucinous neoplasm, and carcinoma of the ampulla of Vater, respectively. They underwent chronic hemodialysis for 7.7 years. They all underwent a PpPD with lymph node dissection. The mean operation time was 373 min and mean blood loss was 647 ml. During the postoperative courses, hemodialysis was restarted on postoperative day 1 in all three cases. I…
