Radiation-associated endometrial cancer.

OBJECTIVE: To compare prognostic variables and survival of radiation-associated endometrial cancers with sporadic second endometrial cancers. METHODS: Patients with primary cancers of pelvic organs (urinary system, colorectal, cervix, vulva, and vagina) were identified from the Surveillance, Epidemiology, and End Results (SEER) database between 1973 and 2005. Among these patients, those who received pelvic radiation and subsequently developed endometrial cancer formed the radiation-associated endometrial cancers cohort (cases), whereas those who did not receive pelvic radiation but subsequent…

[Post-radiation choanal stenosis].

OBJECTIVE: Description of a rare complication of radiation therapy for nasopharyngeal carcinomas and the therapeutic value of surgical endoscopic desobstruction. CASE REPORT: A choanal stenosis observed in an 8-year-old boy, 3 years after radiation therapy and chemotherapy for a voluminous nasopharyngeal carcinoma. No initial involvement of the nasal fossa was observed. Concomitant relapse was excluded after clinical and histological examination. RESULTS: A good functional result was achieved after the choana was unblocked in videoendoscopic surgery. After 9 months of follow-up, the breathin…

Cushing's disease: radiation therapy.

The usual first treatment for Cushing's disease is surgical removal of the pituitary adenoma. In patients in whom surgery is unsuccessful or who decline an operation, radiation to the pituitary offers the possibility of remission. No form of radiation delivery results in immediate control of cortisol production. Thus, until radiation treatment becomes effective, medical therapy to lower cortisol production is indicated. The time to remission with radiation therapy cannot be predicted, medical therapy should be discontinued every 6 months to assess response to radiation treatment; a normal 24 h…

[Radiation-induced chromosome aberrations].

Chromosomal aberrations in peripheral lymphocytes have been used as biomarkers of radiation exposure. The results obtained for this cytogentic study suggest that 86 persons who examined were with structural chromosome aberrations. The sample was included 100 persons who were exposed to radiation professionally. The most frequent persons were found with one, two, three and four chromosome aberrations, and 19 persons had six and more aberrations, while 341 total chromosome aberrations were registried. The most frequent radiation-induced chromosome aberrations in vivo were chromatid breaks, acent…

Radiation-induced optic neuropathy.

Radiation-induced optic neuropathy (RION) is a devastating late complication of radiotherapy to the anterior visual pathway resulting in acute, profound, irreversible visual loss. It is thought to be a result of radiation necrosis of the anterior visual pathway. Visual loss may be unilateral or bilateral; simultaneous or sequential. RION occurs commonly between 10-20 months, with an average of 18 months after treatment; but the onset may range from three months to 9 years. Cumulative doses of radiation that exceed 50 Gy or single doses to the anterior visual pathway or greater than 10 Gy are u…

[Radiation myelopathy and plexopathy].

Radiation myelopathy (RM) is a relatively rare disorder characterized by white matter lesions of the spinal cord resulting from irradiation. It is divided into two forms by the latent periods: transient RM and delayed RM. The delayed RM develops usually non-transverse myelopathy symptoms such as dissociated sensory disturbance, unilateral leg weakness, and gait disturbance with asymmetric steps. Spinal MRI shows initially cord swelling and long T1/T2 intramedullary lesion with enhancement, then exhibits cord atrophy. Histopathological findings of delayed RM are white matter necrosis, demyelina…

[Occupational exposure to radiation].

Since the discovery of X-rays, over exposures and radiation in juries have been reported in both patients and persons exposed in the course of their professional duties. Each year, more than 2,500 million diagnostic radiological examinations, 32 million nuclear medicine examinations or treatment procedures and 5.5 million radiotherapy sessions are performed world wide. Despite all precautions, avoidable incidents and accidents occur through out the world every year, albeit with low frequency. Where as diagnostic radiology is generally safe for patients and staff, intervention al procedures (e.…

Genetic susceptibility to radiation.

In the context of space radiation, it is important to know whether the human population includes genetically predisposed radiosensitive subsets. One possibility is that haploinsufficiency for ATM confers radiosensitivity, and this defect involves 1-3% of the population. Using knock-out mice we chose to study cataractogenesis in the lens and oncogenic transformation in mouse embryo fibroblasts to assay for effects of ATM deficiency. Radiation induced cataracts appeared earlier in the heterozygous versus wild-type animals following exposure to either gamma rays or 1 GeV/nucleon iron ions. In add…

Consequences of radiation accidents.

Radiation injury attributable to radiotherapy is a topic that has attracted ample attention in the literature, especially in a vast number of publications in plastic surgery. However, the literature is clearly devoid of compilations regarding the effects of ionized radiation accidents. A case of a radiation accident is presented. It is nearly impossible to anticipate the extent of effects of external irradiation of the skin and subcutaneous tissues. The initially healed area should be expected to show late recurrent necrosis. Patients exposed to ionized external irradiation are no longer radio…

Mycosis fungoides: radiation therapy.

Radiation therapy is the most effective single agent for the treatment of mycosis fungoides. There are well-defined dose-response relationships for achieving a complete response as well as the durability of this response. Techniques of electron beam therapy have been developed that permit treatment of the entire skin. Total-skin electron beam therapy is an important form of management, especially for patients who have thick generalized plaque or tumorous disease. Radiation therapy may also be used selectively for treatment of extracutaneous disease.

[Radiation-induced cardiac disease].

Radiation-induced effects may damage various cardiac structures chronically and cause heart valve dysfunction as well as occlusive lesions of coronary and other arteries exposed to radiation. A 72-year-old woman with a history of radiation treatment after breast cancer was admitted 25 years later with symptoms of tachycardia and acute dyspnea. We found valvular thickening, medium to severe valvular dysfunction and high grade occlusive coronary artery disease in proximal portions. The left subclavian artery also was affected. Surgical treatment was required immediately. Long-term follow-up card…

Ultraviolet radiation and cataract.

While solar radiation falling on earth comprises light in the infrared, visible, UVA, UVB, and even UVC ranges, the light incident on, and thus important to the biology of, the eye lens is essentially in the visible and UVA regions. Thus, direct photochemical damage to the lens from UVB radiation is minor, though long-term UVA (and even visible range) irradiation is seen to lead to lens malfunction. Short-term exposure of the lens in vivo to UVA light leads to compromised optical and biochemical properties which are repaired in time, while higher doses affect permanent damage. Such longer wave…

Fluoroscopic-induced radiation dermatitis.

A case of radiation dermatitis following fluoroscopy is described. Radiation dermatitis occurs rarely after fluoroscopic procedures and may pose a diagnostic challenge. Many patients do not consider fluoroscopy to be a source of radiation exposure, and clinicians may be confused by its unusual presentation.

Ultraviolet radiation screening compounds.

Amongst the diversity of methods used by organisms to reduce damage caused by ultraviolet (UV) radiation, the synthesis of UV-screening compounds is almost ubiquitous. UV-screening compounds provide a passive method for the reduction of UV-induced damage and they are widely distributed across the microbial, plant and animal kingdoms. They share some common chemical features. It is likely that on early earth strong selection pressures existed for the evolution of UV-screening compounds. Many of these compounds probably had other physiological roles, later being selected for the efficacy of UV s…
