[A case of lumbar ganglion cyst causing radiculopathy].

Ganglion cysts represent a rare pathology mostly encountered in the lumbar region of the spinal column. Magnetic resonance imaging revealed a ganglion cyst at the L4-5 level in a 46-year-old woman who had a complaint of long-standing pain in her right leg. The cyst was completely excised following total laminectomy at L4. After surgery, her symptoms and neurological signs completely disappeared.

S1 radiculopathy due to adenocarcinoma: a case study.

What may initially appear to be a classic presentation of a common condition--in this case sciatic radiculopathy from presumed disc herniation--can sometimes reflect a more ominous process. This article discusses the presentation, diagnosis, and management of a patient initially referred for neurosurgical consultation for S1 radiculopathy suspected to be due to a work-related injury. Final diagnosis was metastatic adenocarcinoma of the rectum.

Cervicothoracic radiculopathy treated using posterior cervical foraminotomy/discectomy.

OBJECT: The authors conducted a study to identify the effectiveness and morbidity rate associated with treating cervicothoracic disc disease (radiculopathy) via a posterior approach. METHODS: Nineteen patients underwent posterior cervicothoracic laminoforaminotomy during a 5.6-year period. Medical records, imaging studies, office charts, hospital records, and phone interview data were reviewed. Specific information analyzed included patient demographics, side of lesion, and conservative treatment, symptoms, and pre- and postoperative pain levels. Pain was rated using a visual analog scale and

Anatomic issues related to cervical and lumbosacral radiculopathy.

The anatomy of cervical and lumbar spinal nerves is well established. Knowledge of the normal and pathoanatomy of impingement of these roots is helpful in the diagnosis of radiculopathy in many cases. Frequently, symptoms and signs do not correlate with expected anatomical patterns. Variations of nerve roots, spinal nerves, and plexuses occur commonly. Neuroanatomical anomalies and musculoskeletal abnormalities account for many of the variations seen with unexpected radicular patterns. Unfortunately, the delineation of segmental innervation has been achieved by several different methods and de

Electrophysiologic correlates of weakness in L5/S1 radiculopathy.

INTRODUCTION: Management of patients with radiculopathy involves estimating the degree of physiologic and anatomic injury, and weighing that to predict the likely clinical course. OBJECTIVE: To determine whether low distal peroneal and tibial CMAP amplitudes correlate with weakness and fibrillations of functionally relevant muscles in L5/S1 radiculopathy (LSR). METHODS: We reviewed clinical and electrophysiologic data in 66 consecutive patients with LSR. RESULTS: A significantly greater number of patients with low peroneal CMAP amplitudes had weakness of L5 (p = 0.025) and S1 innervated leg

Transpedicular approaches to cervical uncovertebral osteophytes causing radiculopathy.

OBJECT: The anterolateral and posterior transpedicular foraminotomies were developed to enhance the treatment of cervical spondylotic radiculopathy secondary to uncovertebral osteophytes. These novel surgical approaches may be advantageous because they allow direct decompression of the nerve root while preserving normal movement and stability of the motion segment. The use of the pedicle to obtain access to compressive uncovertebral osteophytes, without causing damage to the motion segment, has not been previously reported. METHODS: The authors present the early outcome data obtained in 27 pa

Magnetic stimulation in cervical radiculopathy and health controls.

Magnetic stimulation was performed in 15 cases of unilateral cervical radiculopathy and 16 age-matched healthy controls. Among the muscle potentials which have acute slope and high amplitude, the difference between the minimal and the maximal latency in the same muscle was defined as variability. In the healthy control group, the variability of the muscle potentials evoked by head magnetic stimulation was approximately 2 ms. In cervical radiculopathy, the muscle potential latencies in head magnetic stimulation were normal except for one case because of the wide normal range. However, the inter

Lumbar radiculopathy associated with anterior intraspinal bony lesions.

Thirty-one patients underwent posterior decompression for anterior intraspinal bony lesions which were causing symptoms. The diagnoses were a displaced apophyseal endplate, vertebral corner bulge, and an ossified posterior longitudinal ligament or disc. Symptoms were caused by associated lumbar disc herniations or by gradual impingement by the bony lesion on neural tissue. The success rate after operation was 84%. Anterior spinal canal compromise by bony lesions can be treated by decompression through a posterior approach with minimal destruction of the laminae and facets.

Subacute AIDS-related lumbosacral radiculopathy: a bacterial infection?

A case of lumbosacral polyradiculitis in an HIV-1-positive man (CDC IIB) leading to a flaccid paraplegia below the level of L4 is reported. A detailed analysis of several cerebrospinal fluid samples led to the suspicion of a bacterial infection. After treatment with antibiotics and tuberculostatic agents the neuropathy improved and the patient has survived for 2 years. In contrast to similar cases that were probably caused by cytomegalovirus in terminal stages of AIDS, it is argued that an unidentified bacterial infection was the cause of the polyradiculitis in the present case.

Airport induced "cervical traction" radiculopathy: the OJ syndrome.

The escalation of airline travel compounded by federal deregulation, fostering crowded airports, delayed flights, and inconvenient gate scheduling, has forced the frequent flyer to run from one gate to another, carrying heavy under-the-seat bags. Eleven busy executive, nine men and two women, developed cervical radicular pain complaints associated with weakness and painful dysesthesias in the involved extremity. Four had right C6 root symptoms and seven had left C7 complaints. All had evidence, by x-ray, of cervical degenerative disc disease and three had abnormal electromyograms. Treatment, c

S1 radiculopathy presenting as progressive unilateral calf hypertrophy.

Progressive unilateral calf enlargement is more commonly attributed to vascular, inflammatory, neoplastic, or lymphatic disease than to chronic denervation. We report a 55-year-old female recreational runner with progressive right calf enlargement and long-standing mild low back pain. Lumbar MRI demonstrated multilevel disc herniations, including a right-sided L5-S1 extrusion with compression of the right S1 nerve root. During the initial phase of calf symptoms, calf MRI showed mild asymmetric enlargement of the right posterior compartment with edema-like signal on fluid-sensitive sequences. A

Scapular pain in cervical radiculopathy: A scoping review.

BACKGROUND: Controversy persists regarding the relationship between scapular pain and cervical radiculopathy, with no consensus on cervical radiculopathy's definition, radicular pain distribution, or the diagnostic value of scapular pain. This review aims to map the literature describing scapular pain distribution in cervical radiculopathy in clinical practice and research. METHODS: This scoping review followed JBI methodology, guided by the PRISMA-ScR extension. Studies reporting on cervical radiculopathies with described radicular pain distribution were included. Exclusions applied to radic
