[Evaluation of the incidence of radiodermatitis lesions in interventional cardiology].

PURPOSE: The percutaneous coronary interventions use large doses of ionizing radiation, particularly when treating complex lesions. The incidence of radio-induced skin lesions is poorly known. Our goal was to evaluate the frequency of occurrence of such lesions, as well as the factors that may contribute to a high radiation dose. The recommended DAP (dose-area product) cutoff for skin monitoring after percutaneous coronary interventions is 500Gy cm2. PATIENTS AND METHOD: We prospectively studied the incidence of acute (after 5-7 days) and subacute (after 7 days to 6 months) skin lesions follo

Bacterial Contamination of Angiographic Materials in Diagnostic and Interventional Neuroangiography.

PURPOSE: Bacterial contamination has been reported to occur during angiographies, although data on its frequency and relevance are limited. The purpose of our study was to determine whether angiographic materials such as catheters and guide wires remain sterile during angiographies. We sought to differentiate between different materials, and to detect the frequency, the extent and the spectrum of bacterial contamination. METHODS: We prospectively collected 698 fluid or material samples from 100 neuroangiographies. Per angiography we analyzed proximal ends and distal tips of catheters and guid

Evaluation and Initial Stabilization of Undifferentiated Shock.

Vascular Interventional Radiologists (VIRs) are increasingly involved in consultation and management of patients with circulatory shock. The ability to recognize the underlying pathophysiology of a patient's shock and treat appropriately is therefore critical to VIR practice. Here, we discuss the initial evaluation and stabilization of undifferentiated shock. Shock is defined as tissue hypoperfusion from inadequate oxygen delivery and/or impaired tissue oxygen utilization. The etiologies of shock can be grouped into 3 main categories: distributive, hypovolemic, cardiogenic, and obstructive; ea

ESR Essentials: image guided drainage of fluid collections-practice recommendations by the Cardiovascular and Interventional Radiological Society of Europe.

This ESR Essentials article intends to provide detailed, step-by-step, information on the role of imaging in the diagnosis, procedural management, and follow-up of patients with fluid collections. Evidence-based medicine recommendations for the positioning of percutaneous drainages and/or for diagnostic/therapeutic aspiration of fluid collections are provided. Although medical history, clinical symptoms, physical examination, and laboratory tests can raise suspicions regarding a collection, an imaging assessment is usually necessary for the diagnosis. Radiologists can easily identify fluid col

The Reinforced Cementoplasty with Spindles Technique.

Reinforced cementoplasty with spindles is a recently introduced technique that is mainly used for pathological fractures or for bone metastases at risk of fracture in locations with shear stresses. The technique is less challenging to perform than percutaneous screw insertion and does not require equipment sterilization. No general anesthetic is required. A small trocar is all that is needed, and sutures are often unnecessary. Reinforced cementoplasty can therefore be considered as a technical evolution of cementoplasty with the simple addition of material within the trocar. This technique des

[Principles of interventional treatment in epistaxis].

Epistaxis is a common ear, nose, and throat emergency, but rarely a direct cause for hospital admission. Patients who receive inpatient treatment usually suffer from recurrent posterior epistaxis. Despite otolaryngologic measures such as posterior packing or surgery, bleeding may recur or continue or the source may not be surgically accessible. For these patients, endovascular therapy is a real, sometimes the only, treatment option. In addition to idiopathic causes of posterior epistaxis, the cause may also be symptomatic. In this case, the cause must be identified and treatment adapted accord

Lung Cancer Biopsies.

Image-guided percutaneous transthoracic needle biopsy (PTNB) is a well-established and minimally invasive technique for evaluating pulmonary nodules. Implementation of a national lung screening program and increased use of chest computed tomography have contributed to the frequent identification of indeterminate pulmonary nodules that may require tissue sampling. The advent of biomarker-driven lung cancer therapy has led to increased use of repeat PTNB after diagnosis. Percutaneous insertion of markers for preoperative localization of small nodules can aid in minimally invasive surgery and rad

[Creation of a crystalline lens radiation exposure defense cover and the effect of radiation exposure decrease on neuro-interventions].

A variety of radiation hazards resulting from interventional radiology (IVR) have been reported in recent years. Particularly affected are the skin and the crystalline lens, with their high radiation sensitivity. During neurological interventions, the radiological technologist should consider decreasing radiation exposure. We found exposure projections where the exposure dose became a radiation hazard for the crystalline lens, and examined an efficient method of cover for the exposure projections used for neurological interventions. The exposure projection for maximum crystalline lens radiatio

[Temporary vena cava filters for prevention of pulmonary embolism].

Pulmonary embolism (PE) is a serious complication in deep venous thrombosis (DVT), and the prevention of PE is a major target of treatment. Vena cava filters (VCF) can be inserted, and the temporary type and retrievable type of VCF have appeared in recent years in addition to permanent models. These have introduced new criteria for VCF insertion and changes in the algorithm for the treatment of PE. Among cases traditionally calling for permanent VCF, those requiring prevention of PE, especially in the early stage, are suitable for temporary VCF insertion. These include floating thrombi in the

Enhanced accuracy of needle placement in horizontal stereotactic core biopsy by a plastic slot.

Horizontal stereotactic core biopsy can be used in the investigation of clinically occult mammographic abnormalities especially when breast thickness is less than 3 cm. We designed a 6-mm plastic slot that can be inserted between the bushing and the biopsy gun to enhance the accuracy of needle placement within the lesion. With this device, the centre of the lesion can be targeted at the centre of the biopsy trough. We advocate the use of this piece of small, simple and inexpensive instrument in every case of horizontal stereotactic core biopsy.

The role of ultrasound in breast imaging.

Sonography has become an important tool for evaluating breast abnormalities, including inflammatory processes, hematomas, ductal ectasia, cysts and solid lesions. This article describes protocols for ultrasound imaging of the breast and the sonographic appearance of common breast conditions, with particular attention to differentiating benign from malignant lesions. Ultrasound's role in interventional breast procedures and radiation therapy planning also are discussed.

Vascular intervention in the thorax: bronchial artery embolization for haemoptysis.

Massive haemoptysis is defined as the expectoration of more than 600 mls of blood in 48 h. Many patients are not surgical candidates because of the presence of severe bilateral pulmonary disease and these individuals are best managed by bronchial artery embolization. Occlusion of both the bronchial arteries and hypertrophied non-bronchial systemic arteries is essential if bleeding is to be controlled. A pulmonary arterial source of haemorrhage is uncommon but should always be considered in a patient who has further haemoptyses shortly after a technically successful embolization of bronchial an

Antegrade placement of a ureteric stent by a pull-through technique.

Internal double-J ureteric stents are valuable in the treatment of ureteric obstruction, obviating the need for an external drainage catheter. Retrograde placement of these stents is often performed by the urologist, or, if such placement fails, antegrade placement is performed by the interventional radiology service. In cases of high-grade obstruction it may be possible to pass a guide wire through the stricture but impossible to do so with a catheter. The authors describe a pull-through technique, which was used to place a ureteric stent in a 65-year-old man with bilateral hydronephrosis. Th

Radiation exposure to children in diagnosing and at hydrostatic reduction of intussusception.

The energy imparted to the children in diagnosing and hydrostatic reduction of intussusception was measured in 45 children by means of an area-dose measurement device and the mean absorbed dose was estimated. The device was provided with data on tube kVp, mAs and shutter positions and the results were presented as dGy x cm2. The device had been calibrated against a 30 cm3 ionisation chamber at the relevant kVp range. The median energy imparted and mean absorbed dose were 10.8 mJ and 0.94 mGy, respectively. 70% of the total dose was delivered during fluoroscopy. The complex irradiation situatio

[The implantation of Hickman catheters. A new function of interventional radiology].

Hickman catheters are used mainly in patients with hematologic diseases, especially lymphatic and myelotic leukemias, and malignant lymphomas. They facilitate the administration of chemotherapeutics, hyperosmolar solutions and other substances with local toxicity as well as frequent taking of blood samples. Usually Hickman catheters are placed by surgical cut-down on a jugular vein. In lieu of this surgical placement, we recommend the implantation of Hickman catheters by means of interventional radiology techniques. In a period of 13 months 78 Hickman catheters were placed in 67 patients, 37 c

Nonoperating Room Anesthesia and Procedural Sedation in Interventional Radiology: A Collaborative Framework for Safe Growth.

Interventional radiology (IR) has become one of the fastest expanding areas of procedural care, with increasing demand for image-guided diagnostic, vascular, oncologic, and emergency interventions. This growth has created parallel demands for both procedural sedation (PS) and anesthesiology services, the so-called nonoperating room anesthesia (NORA) within the IR suite. These services should not be viewed as competing models, but as different points on a continuum in which the level of sedation or anesthesia is matched to the patient, procedure, location, level of care, clinicians' qualificati

Cross-sight transjugular intrahepatic portosystemic shunt (CS-TIPS): combined ultrasound-fluoroscopy guidance for simultaneous percutaneous portal and hepatic vein puncture and access closure with plug deployment.

Transjugular Intrahepatic Portosystemic Shunt (TIPS) is a well-established procedure for managing portal hypertension and its complications that are refractory to medical and endoscopic therapy. However, complex cases may render the conventional approach ineffective. We describe the cross-sight (CS) technique, which combines ultrasound and fluoroscopic guidance to allow simultaneous puncture of the hepatic and portal veins through a single transhepatic needle pass. Following portal access, stent graft deployment is performed via the jugular route, while the percutaneous tract is closed with a
