Expediting venous drainage in large anterolateral thigh flaps for scalp electrical burns in India: two case reports on the use of primary vein grafts for second vein anastomosis.

Large scalp defects resulting from high-voltage electrical burns require free flaps, preferably skin, to permit optimal coverage and enable future or simultaneous cranial vault reconstruction. The anterolateral thigh permits the harvest of a large area of skin supplied by a reliable perforator. The superficial temporal vessels offer the proximate choice of recipient vessels to enable adequate reach and coverage. The lack of a second vein at this site implies the inability to perform a second venous anastomosis; however, this obstacle can be overcome by using an interposition vein graft, to the…

Analysis of arterial pressure regulating systems in renal post-transplantation hypertension.

OBJECTIVE: To investigate if blood volume expansion, increased sodium retention, changes in neurohumoral arterial pressure control, or altered extrarenal resistance vessel function contribute to the development of renal post-transplantation hypertension. METHODS: F1-hybrids (F1H) obtained from crossing spontaneously hypertensive rats (SHR) and Wistar-Kyoto rats received either an SHR or an F1H kidney graft. Groups consisted of 8-12 animals and were investigated between days 1 and 14 after renal transplantation in three sets of experiments including arterial pressure recordings, plasma volume

[Various Bypass Surgery:Preoperative Simulation for STA-MCA Bypass].

In STA-MCA bypass surgery, it is important to select the optimal recipient using preoperative simulation to avoid complications. We report a preoperative simulation for STA-MCA bypass using the Brain LAB iPLAN platform®BRAIN LAB)and the 3DCG simulation software GRID®Kompath). Here, we introduce the basics and applications of preoperative simulation for occlusive atherosclerotic lesions and present a target bypass for periventricular anastomosis and peripheral vessels of aneurysms in Moyamoya disease. By creating and visualizing 3D fusion images, the optimal donor and recipient can be selected.

Successful outcome after Nd : YAG laser separation of chorioangiopagus-twins under sonoendoscopic control.

Endoscopic laser coagulation of the chorioangiopagus was successfully performed at 25 weeks' gestation in a pregnancy presenting with acute polyhydramnios due to twin transfusion syndrome. Under local anesthesia, a rigid fetoscope housed in a 2.7 mm diameter cannula was introduced transabdominally into the amniotic cavity and the communicating vessels were coagulated by Nd : YAG laser through a fiber that was passed down the side-arm of the cannula. There was rapid normalization of urine output in both fetuses and resolution of the oligo- and polyhydramnios in the donor and recipient twins, re

Techniques for overcoming atretic changes of the portal vein in living donor liver transplantation.

BACKGROUND: Spontaneous diversion of the portal flow through collateral vessels into the systemic circulation is frequently observed in liver transplant recipients with severe portal hypertension. This induces main portal vein atretic change and modifies flow into the collateral even after donor graft implantation. These atretic changes make liver transplantation challenging. In this article we described several methods for overcoming this challenge by appropriate surgical techniques. METHODS: Three anastomotic techniques for living donor liver transplantation were performed in patients with

Simplifying Supermicrosurgical End-to-Side Lymphaticovenous Anastomosis Using a Triangular Silicone Background Sheet Spacer.

Supermicrosurgical lymphaticovenous anastomosis (LVA) is an established but technically demanding treatment for lymphedema. A key challenge is maintaining simultaneous visualization of both lymphatic and venous lumens. We present our method of using a triangular silicone background sheet as an inter-vessel spacer to simplify LVA. After placing the initial posterior wall suture, the spacer is inserted between the lymphatic vessel and recipient vein to provide controlled, tremor-free exposure of both lumens. The degree of tension is adjusted by the depth of spacer insertion. Strategic spacer pos

Feto-feto-fetal transfusion syndrome in monozygotic monochorionic triamniotic triplets: vascular evaluation by a cast model.

A unique cast model of the placenta in a rare case of feto-feto-fetal triplet transfusion syndrome (FFFTTS) allowed the demonstration of why the transfusion syndrome developed in one fetus and not in the other two in that single placenta. The vasculature anatomy of a monochorionic triamniotic triplet placenta with FFFTTS of three healthy infants (one donor, two recipients) born in the 35th week of gestation was cast by means of dental casting materials. After the cast hardened, the tissue was corroded, revealing the cast blood vessels. The diameters and lengths of the chorionic blood and intra

The use of serratus anterior artery as the pedicle for complex humerus reconstruction with a vascularized fibular graft: Anatomical study and clinical application in a case series.

INTRODUCTION: Free vascularized fibular graft is an optimal option for the reconstruction of long, complex bone defects in the humerus. Previous surgeries, infection, and trauma, particularly with brachial artery revascularization, may limit the use of conventional recipient vessels. In this study, we review the use of the serratus anterior artery as a recipient vessel, the anatomical validation of its feasibility and its clinical application in a series of cases. METHODS: The anatomical study consisted of 14 dissections on 7 Thiel-embalmed cadavers. The distance from the origin of the serrat…

Fetoscopic laser ablation in twin-to-twin transfusion syndrome: tips for counselling.

Twin-to-twin transfusion syndrome (TTTS) is a serious complication that affects approximately 10-15% of monochorionic twin pregnancies. The most important role for the development of this condition is the presence of an unbalanced flow through the inter-twin vascular anastomoses. Depending on the number, type and direction of the connecting vessels, blood can be transfused disproportionately from one twin (the donor) to the other twin (the recipient). The diagnosis is defined prenatally by ultrasound and involves of two main criteria: the presence of a monochorionic diamniotic (MCDA) pregnancy

Intraoperative transit-time ultrasonography combined with FLOW800 predicts the occurrence of cerebral hyperperfusion syndrome after direct revascularization of Moyamoya disease: a preliminary study.

BACKGROUND: Cerebral hyperperfusion syndrome (CHS) is a common complication after direct bypass surgery in patients with Moyamoya disease (MMD). Since preventive measures may be inadequate, we assessed whether the blood flow difference between the superficial temporal artery (STA) and recipient vessels (△BF) and the direct perfusion range (DPR) are related to CHS. METHODS: We measured blood flow in the STA and recipient blood vessels before bypass surgery by transit-time probe to calculate △BF. Perfusion changes around the anastomosis before and after bypass were analyzed with FLOW800 to obta…

Rapid Resolution of Polyhydramnios Foretells Circulatory Collapse for the Donor Twin in Feto-Fetal Transfusion Syndrome.

Feto-fetal transfusion syndrome is a pathological process unique to diamniotic monochorionic pregnancies. It is the consequence of an unbalanced fetal blood flow through communicating vessels within a shared placenta. When it occurs, a polyuric, hypervolemic recipient twin co-exists with a hypovolemic oliguric donor. The presence of polyhydramnios or oligohydramnios is considered a poor prognostic indicator, whereas normal amniotic fluid volumes indicate a lack of clinically significant twintwin transfusion. In addition, the spontaneous normalization of amniotic fluid volume is usually seen as

Transfer of free skin grafts with a preserved subcutaneous vascular network.

We present our new approach for preserved subcutaneous vascular network (PSVN) skin transplantation. To confirm the revascularization of the grafts, we undertook a microscopic study and found that, because of the rich anascroscopic study and found that, because of the rich anastomotic communication between the recipient vessels and the PSVN on the undersurface of the skin graft, successful adaptation to the recipient bed is achieved--enhancing the transplantability of the skin graft. We also noted that the absence of injury to the dermis of the graft permits the undersurface of the graft to be…

Optical coherence tomography for early detection of cardiac allograft vasculopathy in pediatric heart recipients.

BACKGROUND: Cardiac allograft vasculopathy (CAV) is a leading cause of graft failure following pediatric heart transplantation (HT). Early detection could improve long-term outcomes, but conventional coronary angiography often misses early-stage disease. Optical coherence tomography (OCT), a high-resolution intravascular imaging modality, may enable earlier and more sensitive CAV detection. METHODS: This retrospective study analyzed pediatric HT recipients who underwent OCT between 2012 and 2024. OCT-derived maximal intima thickness (MIT) and cross-sectional area ratios were correlated with c

In vivo perfusion of free skin flaps using extracorporeal membrane oxygenation.

BACKGROUND: The vessel-depleted, irradiated, and frozen neck, as well as severe atherosclerosis of recipient vessels represent challenging problems in free flap transfer. Extracorporeal free flap perfusion theoretically allows free flap reconstructions in the absence of local donor vessels, but is associated with a number of technical issues. In this study, a novel technique is presented using a commercially available system for extracorporeal membrane oxygenation (ECMO), modified for small blood volumes. METHODS: After preclinical testing, an ECMO system certified for lung support was used t…

Factors affecting the patency of the internal jugular vein after neck dissection for malignant hypopharyngeal tumors-Significance of free jejunal flap transfer using the internal jugular vein as the recipient vein.

BACKGROUND: Although preserved internal jugular veins can become occluded after neck dissection, we have not experienced free jejunal flap necrosis when the veins were used as recipient vessels. Thus, we investigated whether using the internal jugular veins as the recipient vessel for free jejunal flap transfer after neck dissection might contribute to postoperative internal jugular vein patency. METHODS: This retrospective cohort study involved patients who underwent internal jugular vein-preserving neck dissection for hypopharyngeal malignant tumors. We identified and assessed 2 treatment g…
