Refraction problems after refractive surgery.

While refractive surgery such as radial keratotomy, epikeratophakia, and corneal relaxing incisions offer many potential benefits to patients, they can also generate optical problems such as overcorrection or undercorrection of the prior refractive error and variable vision. The authors offer suggestions for avoiding problems through proper patient selection; candid preoperative communication with the patient about possible difficulties and limitations; understanding of the physiological changes that may occur; and optical techniques for alleviating postoperative vision problems.

[Epikeratoplasty using lyophilized lenticles for the correction if aphakia in adults].

Fifty adult aphakic patients were treated surgically by epikeratoplasty. Forty-one eyes had posttraumatic aphakia including 26 eyes with corneal scars. Results are presented with a mean follow-up of 31 months. 64% of eyes achieved an uncorrected visual acuity of 0.2 or better. Best visual acuity was 0.2 or better in 92% of eyes (this percentage is similar to the preoperative figure) and 0.5 or better in 62%. 78% of eyes had a refraction within 3 diopters of emmetropia. 10% of eyes lost 2 or more lines of best visual acuity. Mean keratometric results and spheric equivalent of the refraction sho

Radial keratotomy for myopia. American Academy of Ophthalmology.

The operation of radial keratotomy is undergoing evolution and development. A few hundred thousand radial keratotomy procedures have been performed during the past decade by approximately 10% of the ophthalmologists in the United States. Clinical and laboratory data have now defined a relative level of safety and effectiveness for the procedure. Published data indicate that radial keratotomy usually achieves partial improvement in uncorrected visual acuity in patients with non-progressive low and moderate amounts of myopia. Undercorrection occurs commonly, and the amount of correction cannot b

Corneal Refractive Surgery Considerations in Patients with Cystic Fibrosis and Cystic Fibrosis Transmembrane Conductance Regulator-Related Disorders.

This article discusses common ocular manifestations of cystic fibrosis (CF) and cystic fibrosis transmembrane conductance regulator-related disorders (CFTR-RD). A structured approach for assessing and treating patients with CF/CFTR-RD seeking corneal refractive surgery is proposed, as well as a novel surgical risk scoring system. We also report two patients with various manifestations of CFTR dysfunction who presented for refractive surgery and the outcomes of the procedures. Surgeons seeking to perform refractive surgery on patients with CF/CFTR-RD should be aware of mild to severe clinical m

[Femtosecond lenticule extraction (FLEx)].

Starting in 2006 a new "all femto" method of refractive correction for myopia and myopic astigmatism was introduced. This new method was originally introduced as femtosecond lenticule extraction (FLEx) and further developed with a small incision into SMILE (small incision lenticule extraction). To simplify the terminology the manufacturer brought this onto the market in April 2010 as ReLEx (refractive lenticule extraction). In this procedure a lenticule of intrastromal corneal tissue and a flap-like access cut are subsequently cut utilizing the VisuMax® femtosecond system (Carl Zeiss Meditec,

[Analysis of echographic findings in 745 cases (author's transl)].

A-mode echography was performed on 745 patients of the University of Jordan, Amman, to exclude intraocular tumor in cases of retinal detachment, to localize foreign bodies in the eye and orbit, to evaluate eyes with opaque refracting media, and to discover the causes of exophthalmus. The equipment provided reliable information about the different ocular and orbital disorders. Abnormal echograms were found in about 48% of the patients who had opaque refractive media, hyphema, or occlusion of the pupil. Therefore, when the retina is intact, an improvement in visual acuity can be expected in only

Changes in Foveal Fixation Axis Position Between Seated and Supine Postures and Their Potential Applications in Refractive Surgery.

The correct centration of the ablation in laser refractive surgery is an unresolved classic challenge. This is usually performed at the pupillary center or corneal vertex, but ideally should be done with the point through which the patient's eye truly aligns when looking. Aligning the ablation with this axis is believed to yield the most accurate and effective refractive outcomes. This study proposes the use of a new device designed to center ophthalmic lenses, specifically progressive addition lenses, with the foveal fixation axis (FFA). This device showed an acceptable repeatability and user

Choice of refractive surgery types for myopia assisted by machine learning based on doctors' surgical selection data.

In recent years, corneal refractive surgery has been widely used in clinics as an effective means to restore vision and improve the quality of life. When choosing myopia-refractive surgery, it is necessary to comprehensively consider the differences in equipment and technology as well as the specificity of individual patients, which heavily depend on the experience of ophthalmologists. In our study, we took advantage of machine learning to learn about the experience of ophthalmologists in decision-making and assist them in the choice of corneal refractive surgery in a new case. Our study was b

2022 Glenn A. Fry Award lecture: Enhancing clinical assessment for improved ophthalmic management.

Detailed clinical assessment is critical to allow sensitive evaluation of the eye and its management. As technology advances, these assessment techniques can be adapted and refined to improve the detection of pathological changes of ocular tissue and their impact on visual function. Enhancements in optical medical devices including spectacle, contact, and intraocular lenses have allowed for a better understanding of the mechanism and amelioration of presbyopia and myopia control. Advancements in imaging technology have enabled improved quantification of the tear film and ocular surface, inform

[Refractive Lens Exchange].

In refractive surgery the removal of the natural lens followed by implantation of an artificial intraocular lens has been an established method to treat refractive errors. The term "clear lens exchange" separates this approach from cataract surgery, where a cloudy lens has to be removed. With regard to currently available intraocular lens technologies, there is a wide range to correct myopia, hyperopia as well as astigmatism. Trifocal or extended depth-of-focus lenses (EDOF) even with a toric component offer the patients a potential benefit to further address presbyopia, respectively. Furtherm

[Intrastromal femtosecond laser-based presbyopia correction].

The surgical correction of presbyopia is gaining more and more popularity in the field of refractive surgery. Besides intraocular treatment with multifocal or accommodative intraocular lenses more and more corneal laser approaches are being established. These are performed either with the Excimer laser as laser in situ keratomileusis (LASIK) or the femtosecond laser. The femtosecond laser treatment is done purely intrastromally without dissection of the epithelium and is characterized by an extremely low risk of infection and inflammation. The procedure leads to a central corneal steepening wi

[Epikeratophakia--a new procedure in refractive surgery of the cornea].

The epikeratophakia operation developed by Kaufman represents an additional procedure in refractive surgery of the cornea. The application of individually prepared corneal tissue lenticles on top of the recipient cornea can be used in refractive (Keratolens) and tectonic (Keratopatch) problems. The most important indication for this operation is monocular aphakia in which a contact lens is not tolerated. As is pointed out in the literature, postoperative visual acuity is reduced in some patients by interface densities and incomplete epithelialization. The advantage of this newly developed meth

[Advances in the research of femtosecond laser-assisted lenticule intrastromal keratoplasty for the correction of hyperopia].

Lenticule intrastromal keratoplasty (LIKE) was introduced clinically in the 1980s as a refractive surgery. With the advancements of femtosecond laser, LIKE has been significantly revitalized. Currently, femtosecond laser-assisted LIKE has been demonstrated to be a safe and effective surgical procedure to correct hyperopia, but the predictability is still a major limitation. In this article, we review the history, procedures, influencing factors of predictability and challenge of LIKE to provide reference for clinical practice of femtosecond laser-assisted LIKE.
