"When they see us, it's like they have seen the benefits!": experiences of study benefits negotiations in community-based studies on the Kenyan Coast.

BACKGROUND: Benefit sharing in health research has been the focus of international debates for many years, particularly in developing countries. Whilst increasing attention is being given to frameworks that can guide researchers to determine levels of benefits to participants, there is little empirical research from developing countries on the practical application of these frameworks, including in situations of extreme poverty and vulnerability. In addition, the voices of those who often negotiate and face issues related to benefits in practice - frontline researchers and fieldworkers (FWs) -

Recruitment of patients with de novo Parkinson disease: successful strategies in a randomized exercise clinical trial.

INTRODUCTION: Recruitment of sufficient patients with Parkinson disease into clinical trials is a barrier to successful, timely study completion. Non-pharmacologic studies have shown to be even more challenging for recruitment, despite some studies focusing on de novo Parkinson disease populations. This paper describes successful recruitment techniques from a randomized exercise clinical trial in Parkinson disease. METHODS: Several recruitment strategies were used to enroll de novo patients with Parkinson disease into a year-long clinical trial. Strategies focused on infrastructure included f

Enhancing vaccine clinical trials participation among elderly: challenges and strategies.

As the fastest growing demographic worldwide, the elderly population is projected to reach nearly 1 billion by 2030 and double the 2022 number by 2050. Despite being the largest consumers of medications, they have been historically underrepresented in clinical trials. However, we are witnessing a promising shift as their participation in trials is on the rise. In fact, recent megatrials enrolling more than 30,000 elderly participants demonstrated the feasibility of large-scale clinical trials involving this very specific population. However, unique challenges, including multi-morbidities, frai

Blood-based Aβ42 increases in the earliest pre-pathological stage before decreasing with progressive amyloid pathology in preclinical models and human subjects: opening new avenues for prevention.

Blood-based (BB) biomarkers for Aβ and tau can indicate pathological processes in the brain, in the early pathological, even pre-symptomatic stages in Alzheimer's disease. However, the relation between BB biomarkers and AD-related processes in the brain in the earliest pre-pathology stage before amyloid pathology develops, and their relation with total brain concentrations of Aβ and tau, is poorly understood. This stage presents a critical window for the earliest prevention of AD. Preclinical models with well-defined temporal progression to robust amyloid and tau pathology provide a unique opp

How well do hoarding research samples represent cases that rise to community attention?

This study used archival data from three different research groups and case file data from three independent community organizations to explore how well research samples reflect cases of hoarding that come to community attention. Using data from 824 individuals with hoarding, we found that research volunteers differ from community clients in several ways: community clients are older, more likely to be male and less likely to be partnered; they have lower socio-economic status and are less likely to demonstrate good or fair insight regarding hoarding severity and consequences. The homes of comm

Participant Outcomes from Methods of Recruitment for Videogame Research.

OBJECTIVE: The most productive methods of recruitment for a videogame for health (G4H) trial are not known. Success or failure of recruitment methods has been reported for a variety of clinical trials, but few specifically for G4H trials. This study's goal was to recruit 444 overweight or obese (body mass index percentile between the 84.5th-99.4th percentiles) children between the ages of 10-12 years. The article reports the results of different methods of participant recruitment. MATERIALS AND METHODS: Participants had to agree to three fasting blood samples (baseline, immediately after, and

An economic analysis of income and expenditures by heroin-using research volunteers.

At a Detroit research program from 2004 to 2005, out-of-treatment chronic daily heroin users (N = 100) were interviewed to evaluate relationships between past 30-day income and factors influencing heroin price, expenditures, and consumption. Weekly heroin purchasing frequency was positively related to income and number of suppliers, and negatively related to time cost (min) from primary supplier. Daily heroin consumption was positively related to income and injection heroin use, and negatively related to unit cost of heroin. Implications and limitations are noted. Simulations are underway to a

Pitfalls in anorexia nervosa research: The risk of artifacts linked to denial of illness and methods of preventing them.

One of the most serious problems faced by researchers studying eating disorders is denial of illness in individuals with anorexia nervosa. Importantly, the term "denial" not only has different meanings, but in the case of anorexia nervosa its very nature still remains obscure. It is not even known whether it is deliberate or unintentional. Denial of illness in anorexic patients has serious consequences for evaluation of the reliability of information obtained from those individuals. Indeed, the most frequently used screening questionnaires, such as the Eating Attitudes Test (EAT) (Garner & Gar

Partially randomized preference trials in carceral health research: a methodology to advance equity and inclusion.

BACKGROUND: Incarcerated people experience disproportionately poor health outcomes yet often remain excluded from clinical research due to ethical concerns of limited autonomy and coercion. As a result, traditional randomized controlled trials face significant challenges in carceral settings, driven by mistrust of the medical system, randomization limiting choice, and logistical barriers with blinding and masking. These challenges necessitate alternative methodologies that balance scientific rigor while empowering participant choice. MAIN TEXT: This commentary proposes partially randomized pr

Researching special populations: retention of Latino gay and bisexual men and transgender persons in longitudinal health research.

Few studies have sought to assess predictors of retention of racial/ethnic or sexual minorities in longitudinal health research. The purpose of this study is to investigate predictors of retention of Latino gay and bisexual men and transgender (GBT) research participants after the baseline interview. Data come from a sample of 643 Latino GBT individuals in two cities (Chicago and San Francisco). We assessed potential predictors of retention (operationalized as successful re-contact) using binary logistic regression of retention on five factors (sociodemographic/health, residential stability, a

The ethics of biosafety considerations in gain-of-function research resulting in the creation of potential pandemic pathogens.

This paper proposes an ethical framework for evaluating biosafety risks of gain-of-function (GOF) experiments that create novel strains of influenza expected to be virulent and transmissible in humans, so-called potential pandemic pathogens (PPPs). Such research raises ethical concerns because of the risk that accidental release from a laboratory could lead to extensive or even global spread of a virulent pathogen. Biomedical research ethics has focused largely on human subjects research, while biosafety concerns about accidental infections, seen largely as a problem of occupational health, ha

Ethical complexities in contraceptive mandates for adolescent clinical trial participation: a qualitative analysis of U.S. adolescents' perspectives.

BACKGROUND: Clinical trials often require women and girls of childbearing potential to adhere to contraceptive mandates, typically requiring the use of two forms of contraceptives for the duration of the study. Understanding adolescent perspectives on contraceptive mandates within HIV prevention and treatment trials is critical for evaluating whether and when such requirements are ethically justified and, if so, how they might be appropriately applied in trials involving this group. METHODS: We conducted semi-structured interviews with adolescents age 15-24 in the U.S. on their perspectives a

I didn't know I had executive dysfunction, now I can do something about it: perspectives about CO-OP from people with Parkinson's disease.

PURPOSES: People with Parkinson's disease (PD) experience increasing disability across their lifespan. Holistic rehabilitation approaches are needed to maximize their daily functioning and quality of life (QoL). The Cognitive Orientation to daily Occupational Performance (CO-OP) approach targets daily functioning, with established efficacy in other neurological populations. This study is part of a larger trial of CO-OP with people with PD, describing the perspectives of people with PD who received CO-OP about its effect on their lives. METHODS: People with PD participating in a randomized con

Understanding adolescent and parent acceptability and feasibility experience in a large Type 1 diabetes mellitus behavioural trial.

AIMS: Using an 18-month, multisite randomized control trial as an exemplar, the aim of this study was to identify themes related to adolescent and parental feasibility and acceptability for participation in large behavioural trials designed to improve adolescents' Type 1 diabetes self-management. METHODS: Thematic analysis methodology was used to develop themes describing factors related to acceptability and feasibility. RESULTS: Based on a sample of interviews (N = 72), factors contributing to intervention acceptability and feasibility were identified. Aspects of acceptability included: a f

Adequacy of Inclusion of Older Adults in NIH-Funded Phase III Clinical Trials.

In the United States, the population aged 65 and older is rapidly growing, and this group uses more healthcare resources and has unique healthcare needs that do not exist in younger populations. However, it was reported that older adults are excluded or underrepresented in clinical trials for several diseases. We examined phase III clinical trials funded by the National Institutes of Health found in www.clinicaltrials.gov from 1965 to 2015 that addressed top causes for hospitalization and/or disability-adjusted life years in older adults: congestive heart failure (n = 45), cardiac dysrhythmias
