Blood haemoglobin concentrations and reticulocyte counts were determined in 430 school children in grades 1, 3 and 5 in a small township in rural Zambia at the end of the rainy season. Running capacity was estimated by the 12-min running test. Age, sex, height, weight, social and nutritional indicators were recorded. Mean haemoglobin was 125.3 g/1 +/- 12.3 SD. Peripheral blood samples from dogs infected with Babesia gibsoni were analyzed by a flow cytometer for the percentage of reticulocytes after staining with a membrane-permeable fluorochrome, thiazole orange. A 41-year-old woman treated with isoniazid (INH) for latent tuberculosis infection and an oral corticosteroid for sarcoidosis developed severe anemia two months after initiating INH. A 51-year-old white man of Irish extraction was found to have apparent 'sporadic' hereditary spherocytosis with a reticulocyte count of 6%. Twelve of his 13 siblings were examined and found to be haematologically normal. The patient's erythrocytes were found to have a diminished amount of spectrin as compared to his siblings and to unrelated controls. It is suggested that the proband may represent either a new mutant or possibly double heterozygosity for two inherited biochemical variants of the red cell membrane skeleton which individually give no haematological abnormalities.
