Multimodal Imaging of Acute Central Retinal Artery Occlusion.

The aim of this study was to describe fluorescein angiography (FA), ocular coherence tomography (OCT) and ocular coherence tomography angiography (OCTA) in the diagnosis of acute central retinal artery occlusion (CRAO). This is an observational case series study performed at Sohag Ophthalmic Investigation Center. Fifteen patients presented by a sudden marked unilateral diminution of vision were included. Corrected Distance Visual acuity (CDVA), color fundus photos, FA, OCT and OCTA, imaging obtained in the first week of presentation and imaging of the other normal eye as a control were assesse…

Treatment of Nonarteritic Acute Central Retinal Artery Occlusion.

Central retinal artery occlusion (CRAO) is an event most often caused by an embolus originating in the ipsilateral carotid artery, aortic arch, or heart. CRAO may result from partial or complete occlusion of the central retinal artery (CRA), which acts as the primary blood supply to the inner neurosensory retina, and typically results in profound vision loss and permanent visual disability. No consensus has emerged regarding the optimal treatment of CRAO. All proposed treatments are of questionable efficacy and many have uncertain risk profiles. In certain circumstances, thrombolysis may be at…

Practice Patterns After Acute Embolic Retinal Artery Occlusion.

PURPOSE: To compare stroke evaluations recommended by retina special-ists and neurologists for retinal artery occlusion (RAO). DESIGN: A cross-sectional survey. METHODS: An anonymous survey was emailed to members of the American Academy of Neurology Stroke Section listserv and vitreoretinal specialists registered with the American Academy of Ophthalmology. The survey was divided based on duration of symptoms before encounter: less than 12 hours, 24-48 hours, and more than 1 week. Institutional review board approval was obtained before data collection. RESULTS: Four hundred forty-eight surve…

Multimodal Images of Acute Central Retinal Artery Occlusion.

Two illustrative cases of acute central retinal artery occlusion (CRAO) are presented with multimodal imaging, including fluorescein angiography (FA) and commercially available optical coherence tomography angiography (OCT-A). In both patients, retinal ischemia was imaged well using both FA and OCT-A, and the two imaging studies provided comparable pictures. OCT-A provides useful information for the diagnosis and management of patients with acute CRAO, without the need for dye injection.

Optical Coherence Tomography Angiography of Retinal Artery Occlusion.

The optical coherence tomography angiography (OCTA) microvascular features of branch and central retinal artery occlusion are described and correlated with fluorescein angiographic and clinical findings. There may be differences in the distribution of reduced vascular perfusion/flow between the superficial and deep retinal capillary plexuses in retinal artery occlusive disease. The extent of macular ischemia and the status of flow in the radial peripapillary capillaries can be evaluated with OCTA. Overall, OCTA can be used to monitor changes in vascular flow during the clinical course of retin…

Bilateral Central Retinal Artery Occlusion in Behçet Disease.

PURPOSE: To report a case of bilateral central retinal artery occlusion as the primary ocular manifestation of Behçet disease. METHODS: A 38-year-old patient, with no history of previous ocular complaints, admitted with bilateral sudden vision loss. His ophthalmologic examination revealed a visual acuity of no light perception in both eyes and the diagnosis of bilateral central retinal artery occlusion. A detailed systemic investigation was conducted. RESULTS: With the history of recurrent oral and genital ulcers, and the presence of ocular and skin findings, the diagnosis of Behçet disease …

Transient retinal artery occlusion during phacoemulsification cataract surgery.

PURPOSE: Transient retinal artery occlusion (TRAO) is a potentially underdiagnosed cause of immediate 'pad off' visual loss following phacoemulsification cataract surgery under sub-Tenon's anaesthesia. METHODS: We describe a series of three patients presenting with enigmatic 'pad off' visual loss following phacoemulsification surgery, each diagnosed with TRAO. We describe the variable clinical presentation, illustrate the value of optical coherence tomography (OCT) imaging in establishing the diagnosis, and present the final visual outcomes. RESULTS: Clinical findings alone may be subtle and…

Central retinal artery occlusion due to infective endocarditis.

We present a case of central retinal artery occlusion (CRAO) in a 59-year-old male suffering from infective endocarditis. The patient was receiving antibiotic treatment for the endocarditis for 5 days prior to the event and received conservative treatment for the CRAO without any improvement. A few days after the event, the patient underwent heart surgery with aortic and mitral valve replacement and eventually recovered without any other side-effects. CRAO is a rare but devastating embolic complication of infective endocarditis and can happen regardless of whether a patient is receiving system…

[Color Doppler imaging and central retinal artery occlusion].

PURPOSE: To assess the role of CDI of retrobulbar vessels in pathogenic diagnosis of CRAO. METHODS: We have used a sonographer with 9 MHz l … ue, as being the source of cholesterol emboli. The second patient had no detectable flow in central retinal artery due to multiple reverberations determined by calcic arterial emboli, placed 5 millimeters behind the optic disc. The chest X-ray showed an aortic plaque, considered the source of the emboli. The third patient had characteristic CDI findings for Hort …

Branch retinal artery occlusion associated with Behçet disease.

PURPOSE: To report 2 cases of branch retinal artery occlusion (BRAO) associated with Behçet disease (BD). DESIGN: Small case series. METHODS: Review of two patients' charts, fluorescein angiography, and optical coherence tomography (OCT). RESULTS: The authors report two young patients, diagnosed with BD, who developed decreased vision secondary to BRAO for the first patient and cilioretinal artery occlusion associated with branch retinal vein occlusion for the second patient. In the two cases, artery occlusion was associated with active intraocular inflammation. CONCLUSIONS: BRAO, with sub…

Circadian incidence of non-inflammatory retinal artery occlusions.

BACKGROUND AND PURPOSE: Myocardial infarction and stroke tend to occur in the morning. We were interested in discovering when the onset of visual loss occurred in patients with retinal artery occlusion (RAO). Assuming that the therapeutic time slot is about 2 to 6 hours, a cumulated incidence during the night might have therapeutic implications. METHODS: We retrospectively evaluated the data of 266 patients (175 male and 91 female patients) with non-inflammatory RAO: 211 patients with central retinal artery occlusion (CRAO), 36 patients with branch retinal artery occlusion (BRAO), and 19 pati…

[Optical coherence tomography in central retinal artery occlusion].

CLINICAL CASES: Three eyes with central retinal artery occlusion (CRAO) have been studied. Optical coherence tomography (OCT) was performed in each of them. Ophthalmoscopic signs of CRAO were equivocal in the three eyes. However, the presence of a hyporeflective signal in the OCT scan could be seen clearly in each of them. DISCUSSION: The presence of a hyporeflective band between the neurosensory retina and the retinal pigment epithelium in OCT images, that persist for several months after a CRAO episode, is useful in establishing the diagnosis in these patients.

[Hemicentral retinal artery occlusion associated with moyamoya syndrome].

Moyamoya syndrome is a rare cerebrovascular disorder characterized by bilateral progressive supraclinoidal stenosis or occlusion of the internal carotid artery and development of collateral vessels in the lenticulostriate region. The syndrome manifests itself in association with another disease or other clinical symptoms. We present the case of a 12-year-old girl with hemicentral retinal artery occlusion due to moyamoya syndrome associated with clinical and laboratory findings of presumed systemic lupus erythematosus.
