Long-term outcomes of percutaneous retrogasserian glycerol rhizotomy in 3370 patients with trigeminal neuralgia.

AIM: To assess the long-term outcomes of percutaneous retrogasserian glycerol rhizotomy (PRGR) in patients with medically unresponsive idiopathic trigeminal neuralgia. MATERIAL AND METHODS: A total of 3370 patients undergoing PRGR between 1983 and 2003 in our hospital were followed up for 13.1 years (range 3-23 years), and the outcomes were investigated. RESULTS: Of the 3370 patients, 99.58% had complete initial pain relief. A total of 2750 patients were followed up when 112 died of unrelated causes and 508 were lost to follow up. Of the 2750 patients, 962 (35%) had pain recurrence at variou…

Percutaneous trigeminal nerve radiofrequency rhizotomy guided by computerized tomography with three-dimensional image reconstruction.

BACKGROUND: Percutaneous radiofrequency trigeminal rhizotomy offers high rates of complete pain relief for classic idiopathic trigeminal neuralgia. When performed under fluoroscopy, it may cause anxiety for an inexperienced neurosurgeon owing to lack of familiarity with the regional anatomy, lack of reliable landmarks on fluoroscopy, and perceived risks associated with inadvertent puncture of neurovascular structures near the foramen ovale. The purpose of this study is to describe a new procedure to maximize patient security and shorten operative time. METHODS: From January 2006 to May 2009, …

Does sacral posterior rhizotomy suppress autonomic hyper-reflexia in patients with spinal cord injury?

OBJECTIVE: To study the occurrence of autonomic hyper-reflexia (AHR) after intradural sacral posterior rhizotomy combined with intradural sacral anterior root stimulation, performed to manage the neurogenic hyper-reflexic bladder and to determine the pathophysiological basis of the uncontrolled hypertensive crisis after sacral de-afferentation. PATIENTS AND METHODS: Ten patients with spinal cord injury operated using Brindley's method between September 1990 and February 1994 were reviewed. Systematic continuous non-invasive recordings of cardiovascular variables (using a photoplethysmograph) …

The frequency of intradural conjoined lumbosacral dorsal nerve roots found during selective dorsal rhizotomy.

The frequency of occurrence of conjoined nerve roots (CR) in the cauda equina has been reported to range from 0.3% to 2%. We found intradural conjoined nerve roots in 13 of 123 (10.6%) patients undergoing selective dorsal rhizotomy for spasticity due to cerebral palsy. The anomaly occurred most commonly at L5-S1 and S1-S2. There was no consistent relationship to other anomalies of segmental anatomy. Our finding of a significantly higher number of conjoined nerve roots in these patients is primarily due to the surgical exposure of the entire cauda equina. This anomaly is more common than previo…

Incidence of spinal deformity in children after multiple level laminectomy for selective posterior rhizotomy.

Fifty-five children with cerebral palsy had multiple-level laminectomies for selective posterior rhizotomies for the relief of spasticity. They were followed up clinically and radiologically to assess their spinal stability and the possible development of post-laminectomy deformity of the spine. The majority of the deformities found were related to cerebral palsy and did not appear to be due to the laminectomy: 16% had scoliosis, 5% kyphosis, 7% lordosis, and 9% spondylolysis/spondylolisthesis. Spondylolysis is the only abnormality that appeared to be more common in this group than in children

T1 rhizotomy for post-stroke hand flexion spasticity: a preliminary report of 15 cases.

BACKGROUND AND OBJECTIVE: Stroke is the leading cause of central upper limb spastic paralysis, with hand flexion spasticity being the most common and functionally debilitating manifestation. Various surgical approaches have been explored, yet optimal management remains challenging. This study proposes T1 rhizotomy as a novel surgical intervention for post-stroke hand flexion spasticity. METHODS: Fifteen consecutive patients with unilateral post-stroke hand flexion spasticity underwent T1 rhizotomy between November 2023 and December 2024. Under general anesthesia, the T1 nerve root was exposed…

Partial sensory rhizotomy in therapy-refractory and recurrent trigeminal neuralgia - a single center experience.

BACKGROUND: Partial sensory rhizotomy (PSR) is an "ultima ratio" procedure for patients with therapy-refractory trigeminal neuralgia (TN). The treatment can be offered to patients without a neurovascular conflict or to patients who did not benefit either from previous microvascular decompression (MVD) or from other interventional procedures. This study presents our experience with PSR. METHODS: Our prospectively maintained database was searched for patients who underwent PSR. We conducted a retrospective analysis of all patients with PSR. Clinical data, MR imaging, surgical videos, and OR not…

Radiofrequency rhizotomy for multiple sclerosis-related trigeminal neuralgia: a systematic review and meta-analysis.

BACKGROUND: Radiofrequency rhizotomy (RFR) is one of the primary therapeutic options for managing patients suffering from multiple sclerosis-related trigeminal neuralgia (MS-TN). Given the increasing use of RFR for MS-TN and the absence of consensus on its efficacy, we aimed to assess the clinical application of RFR in this patient population. METHODS: On April 7, 2025, we conducted a systematic literature search. Studies that evaluated the pain-related outcomes of RFR in MS-TN patients were included. RESULTS: Fifteen studies involving 278 MS-TN patients who received RFR were included. The m…

Middle meningeal artery pseudoaneurysm and pterygoid plexus fistula following percutaneous radiofrequency rhizotomy: illustrative case.

BACKGROUND: Percutaneous treatment for trigeminal neuralgia is a safe and effective therapeutic methodology and can be accomplished in the form of balloon compression, glycerol rhizotomy, and radiofrequency thermocoagulation. These procedures are generally well tolerated and demonstrate minimal associated morbidity. Moreover, vascular complications of these procedures are exceedingly rare. OBSERVATIONS: We present the case of a 64-year-old female with prior microvascular decompression and balloon rhizotomy who presented after symptom recurrence and underwent a second balloon rhizotomy at our …

Facial Sensory Restoration After Trigeminal Sensory Rhizotomy by Collateral Sprouting From the Occipital Nerves.

BACKGROUND AND IMPORTANCE: Lesioning procedures are effective for trigeminal neuralgia (TN), but late pain recurrence associated with sensory recovery is common. We report a case of recurrence of type 1A TN and recovery of facial sensory function after trigeminal rhizotomy associated with collateral sprouting from upper cervical spinal nerves. CLINICAL PRESENTATION: A 41-yr-old woman presented 2 yr after open left trigeminal sensory rhizotomy for TN with pain-free anesthesia in the entire left trigeminal nerve distribution. Over 18 mo, she developed gradual recovery of facial sensation migrat…

Risk factors for progressive neuromuscular scoliosis requiring posterior spinal fusion after selective dorsal rhizotomy.

OBJECTIVE Selective dorsal rhizotomy (SDR) via limited laminectomy is an effective treatment of lower-extremity spasticity in the pediatric population. Children with spasticity are also at risk for neuromuscular scoliosis; however, specific risk factors for progressive spinal deformity requiring posterior spinal fusion (PSF) after SDR are unknown. The authors' goal was to identify potential risk factors. METHODS The authors performed a retrospective cohort study of patients who underwent SDR via limited laminectomy between 2003 and 2014 and who had at least 1 year of follow-up. They analyzed d…

Endosurgical repair of an iatrogenic facial arteriovenous fistula due to percutaneous trigeminal balloon rhizotomy.

A 56-year-old woman with right-sided trigeminal neuralgia (TN), who underwent technically uneventful percutaneous balloon rhizotomy, developed significant bilateral pulsatile tinnitus on the first post-operative day. Although the patient reported significantly improved neuralgia, auscultation revealed a right facial bruit. Magnetic resonance angiography (MRA) of the face and brain demonstrated prominent right facial and jugular venous vascularity. Catheter angiography confirmed the suspected facial arteriovenous fistula (AVF). A transarterial approach was used to explore the AVF which arose fr…

Patient reports of satisfaction after microvascular decompression and partial sensory rhizotomy for trigeminal neuralgia.

OBJECTIVE: There are no reports of patient satisfaction surveys after either a microvascular decompression (MVD) or a partial sensory rhizotomy (PSR) for trigeminal neuralgia. This study compares patient satisfaction after these two types of posterior fossa surgery for trigeminal neuralgia, because it is postulated that recurrences, complications, and previous surgical experience reduce satisfaction. METHODS: All patients who had undergone their first posterior fossa surgery at one center were sent a self-complete questionnaire by an independent physician. Among the 44 questions on four stand…

Hand somatosensory cortex activity following selective dorsal rhizotomy: report of three cases with fMRI.

INTRODUCTION: Selective dorsal rhizotomy (SDR) is an effective treatment for lower extremity spasticity in cerebral palsy. Cortical organization in sensory cortex may be abnormal in cerebral palsy, and deafferentation is known to lead to cortical reorganization in many situations. METHODS: We used functional magnetic resonance imaging (fMRI) of hand sensory stimulation to determine if the partial deafferentation of the lower extremity sensory system, associated with SDR, led to any alterations in the cortical somatosensory representation for the upper limbs. Three patients with spastic dipleg…

Trigeminocardiac reflex: a unique case of recurrent asystole during bilateral trigeminal sensory root rhizotomy.

BACKGROUND: The trigeminocardiac reflex is the sudden-onset of dysrhythmia and hypotension during manipulation of any of the branches of the trigeminal nerve. The trigeminal nerve and cardioinhibitory vagus nerve constitute the afferent and efferent pathways in the reflex arc. The trigeminocardiac reflex has been reported to occur during craniofacial surgery, balloon-compression rhizolysis of the trigeminal ganglion, and tumour resection in the cerebellopontine angle. PATIENT & METHOD: A 2-year-old male patient with haemangioma near the sella turcica underwent rhizotomies of both sides of the

Changes in hip migration after selective dorsal rhizotomy for spastic quadriplegia in cerebral palsy.

Selective dorsal rhizotomy is increasingly used for management of spastic quadriplegic cerebral palsy but rates of hip stability following the operation have not been reported. Determining hip stability by radiographic measurement of lateral migration of the femoral head beyond a lateral edge of the acetabulum after dorsal rhizotomy allows an objective assessment of the outcome of the operation. This prospective study examined the effect of selective dorsal rhizotomy on lateral migration of the femoral head in 45 children with spastic quadriplegic cerebral palsy. The children ranged in age fro…

Variability in selective dorsal rhizotomy surgery: an analysis of the Cerebral Palsy Research Network registry.

OBJECTIVE: Selective dorsal rhizotomy (SDR) is a surgical procedure to reduce spasticity and improve function in children with cerebral palsy (CP). Randomized trials have shown that SDR is superior to physical therapy alone for reduction of spasticity and improvement in gait, but there is wide variation in surgical technique. The purpose of this study was to describe the scope of patient factors and surgical technique of SDR performed for the management of spasticity in children with CP. METHODS: This study is a cross-sectional analysis of data included in the Cerebral Palsy Research Network …
