The ideal treatment approach to massive, irreparable rotator cuff tears remains a hot topic of debate. In addition to joint-preserving methods including partial repair and subacromial spacer, techniques such as superior capsular reconstruction and tendon transfers are viable alternatives. These techniques are effective in providing pain relief and-to an extent-functionality during short-term follow-up; however, superior capsular reconstruction (SCR) appears to be the treatment of choice to address the pseudoparalysis. The relatively lower success rates of pseudoparalysis treatment in tendon tra

Rotator cuff tear arthropathy (RCTA) is characterized by massive rotator cuff tearing combined with humeral head migration (HHM). The aim of this study is to investigate the quantitative characteristics of this migration and its association with glenoid erosions and prearthropathy scapular anatomy. We quantified HHM and prearthropathy scapular anatomy of 64 RCTA patients with statistical shape modeling-based techniques. Glenoid erosion was classified according to Sirveaux et al. A cutoff value for confirming HHM was 5 mm based on a control group of 49 patients. Group 1 (RCTA without HHM) consi

Common sports, involving raising the arms above the head, i.e., throwing, racquet games and swimming, often result in rotator cuff tendinitis. During the throwing motion, the humeral head and its overlying biceps tendon and rotator cuff must pass rapidly under the coraco-acromial arch. Damage to these structures can occur by several mechanism. First, an increase in the size of the structures passing underneath the arch may lead to impingement. This can occur either by way of hypertrophy of the musculotendinous cuff or by way of inflammation of the cuff. Second, a decreased space available unde

Throwers, or athletes who engage in repetitive overhead motions, are a unique subset of athletes that experience distinct shoulder injuries. Athletes engaged in baseball comprise the majority of patients seeking orthopedic care for throwing related injuries. Injuries specific to throwers most commonly involve the labrum and the undersurface of the rotator cuff. In addition, tissue changes in both the anterior and posterior glenohumeral capsule are common with repetitive overhead motions. These capsular changes alter. This article will examine the pathomechanics of injuries to throwers, elabora

Subtle degenerative tears that so commonly involve the upper third of the subscapularis are oftentimes missed. However, there are physical examination, imaging, and arthroscopic findings that are highly suggestive of subscapularis tearing. Positive belly-press and bear hug tests, a thinner and longer subscapularis tendon, subscapularis muscle, belly loss of bulk or fatty infiltration, narrow coracohumeral distance, and biceps pathology are indicative. Artificial intelligence-based predictive algorithms can estimate the likelihood of subscapularis tearing based on a combined analysis of these a

BACKGROUND: Commercial truck drivers (CTDs) are significantly affected by shoulder injuries; however, little is known about the unique mechanisms of injury (MOIs), specific injuries, or possible preventive measures among this group of workers. This study characterized the MOIs, musculoskeletal disorders (MSDs), and factors associated with MSDs of the shoulder among a group of CTDs.
METHODS: A retrospective medical record review was conducted of CTDs between 21 and 65 years of age who were seen for MSDs of the shoulder between 2007 and 2015.
RESULTS: A total of 130 CTDs were included, who wer

Rotator cuff (RC) tears are common injuries leading to significant dysfunction of the shoulder. Rotator cuff tears alter tension and strain in muscles and tendons. Anatomical studies demonstrated that rotator cuff muscles are comprised of anatomical subregions. However, the strain distribution within the rotator cuff tendons generated from the tension from each anatomical subregion is unknown. We hypothesized that subregions would present distinct 3-dimensional (3D) strain distributions within the rotator cuff tendons, and that the anatomical insertion configuration of the supraspinatus (SSP)

Rotator cuff tears remain one of the most common injuries encountered by shoulder surgeons. Although arthroscopic suture-bridge repair has demonstrated superior biomechanical properties and achieved promising clinical results, tendon retears remain a persistent challenge. To address this issue, we have introduced several refinements to optimize the existing repair techniques. One such refinement is the incorporation of a medial knot in the suture-bridge repair, which can decrease gap formation and enable a watertight repair, thereby improving tendon-bone healing. Another critical modification

Sutures and suture anchors are commonly used in rotator cuff repairs as they offer cost-effective and mechanically robust solutions for tendon-bone interface (TBI) healing. However, in large to massive rotator cuff tears, where substantial tendon loss and limited intrinsic healing potential are present, conventional repair techniques alone often fail to restore the native biomechanics and structural integrity. Consequently, retear rates in these cases remain unacceptably high. This review summarizes recent advances in graft-based augmentation strategies aimed at improving outcomes in these cha

Massive irreparable rotator cuff tears without glenohumeral arthritis are a common cause of shoulder pain and disability. Many surgical treatment options have been proposed, including debridement, partial repair, tendon transfer, superior capsule reconstruction, balloon spacer placement, bursal acromial reconstruction, and reverse shoulder arthroplasty. Interposition graft bridging reconstruction, as evidenced by the mid-term results of the current study, may also be considered, at least for now. However, let's see if this procedure will truly stand the test of time because all orthopaedic sur

BACKGROUND: Rotator cuff related shoulder pain (RCRSP) is a common and disabling shoulder condition and surgical management is becoming more common. The rates and costs of surgical interventions have been on the rise. Understanding decision-making related to surgery and providing adequate information to people with RCRSP may improve patient-centred care and potentially reduce rates of surgery.
OBJECTIVES: To explore the decision-making processors of people who have undertaken surgery for RCRSP.
DESIGN: An in-depth thematic analysis.
METHOD: Interviews were conducted with patients from Melbo

The long head of the biceps tendon can be a significant pain generator in the setting of rotator cuff tearing. Surgeons who encounter long head of biceps tendon pathology at the time of rotator cuff repair can be confident in a high likelihood of statistically significant clinical improvement with concomitant long head of biceps surgery. Patients can be reliably counseled preoperatively that if they have both clinically apparent long head of biceps tendon pathology and rotator cuff tearing, and if they undergo long head of biceps tendon surgery with rotator cuff repair, they can expect compara

Full-thickness rotator cuff tears are common. When symptomatic, they can affect quality of life. Surgical repair might improve patients' overall health. We systematically reviewed postoperative outcomes in 10 studies comparing mini-open repair and all-arthroscopic repair techniques. Data regarding patient demographics, rotator cuff pathology, postoperative rehabilitation protocols, American Shoulder and Elbow Surgeons (ASES) scores, University of California Los Angeles (UCLA) scores, pain scores, and incidence of recurrent defects were extracted. There were no statistically significant differe

After preparation of the bone bed, two doubly loaded suture anchors with suture eyelets are inserted at the articular margin of the greater tuberosity. A retrograde suture-passing instrument penetrates the rotator cuff to retrieve the sutures through the modified Neviaser or subclavian portal. An ipsilateral pair of suture eyelets in the suture anchor is passed through the margins of the rotator cuff tear. The blue suture of the second and third pair is pulled out of the lateral cannula, and the threaded blue suture of the third pair in the needle is passed through the blue suture of the secon

Rotator cuff pathology is present in nearly half the adult population over the age of 50 years and remains a leading cause of shoulder pain and dysfunction. These musculotendinous injuries may be the result of an acute trauma or chronic degeneration. Clinical examination involves inspection, range of motion, and strength assessment with special testing used to isolate the involved rotator cuff muscles. The classification of these injuries involves identification of tear size, thickness, morphology, and the presence of tendon retraction or muscular atrophy to guide clinical management. Magnetic

BACKGROUND: Rotator cuff tears are common musculoskeletal injuries frequently requiring surgical intervention, yet retears occur at a notable rate. Teriparatide (recombinant human parathyroid hormone, rhPTH) has shown promise in animal studies for enhancing bone and tendon healing. However, limited human data exist on its clinical benefits when used adjunctively in arthroscopic rotator cuff repairs.
PURPOSE: To investigate whether the systemic administration of teriparatide improves functional outcomes following arthroscopic rotator cuff repair in a triple-blind, randomized controlled trial (
