Detection, referral and control of diabetes and hypertension in the rural Eastern Cape Province of South Africa by community health outreach workers in the rural primary healthcare project: Health in Every Hut.

BACKGROUND: Non-communicable diseases, mainly cardiovascular diseases, diabetes, cancer and chronic respiratory diseases, are responsible for approximately 63% of all deaths occurring worldwide in any given year. The majority of these deaths have occurred in low- and middle-income countries (LMICs). The latest World Health Organization (WHO) report shows that the increase in diabetes is also most pronounced in the LMICs. The South African Labour and Development Research Unit estimated a 9% prevalence within the adult population in 2016. In the Eastern Cape Province, hypertensive heart disease

Health equity and e-cigarette use among young adults in rural areas: A social determinants of health framework.

Young adults (YAs) aged 18 to 24 in rural areas are increasingly using Electronic Nicotine Delivery Systems (ENDS), commonly known as e-cigarettes, in the United States. Rural communities already face significant health disparities due to higher tobacco use, contributing to worse health outcomes, including increased mortality from heart disease, cancer, and respiratory diseases. Using the adapted World Health Organization's Social Determinants of Health framework, this paper examines how structural and intermediary determinants shape ENDS use among rural YAs. Special attention is given to how

Health systems model for chronic disease secondary prevention in rural and remote areas - Chronic disease: Road to health.

Objectives Cardiac rehabilitation (CR) provides evidence-based secondary prevention for people with heart disease (HD) (clients). Despite HD being the leading cause of mortality and morbidity, CR is under-utilised in Australia. This research investigated healthcare systems required to improve access to CR in rural and remote areas of North Queensland (NQ). Methods A qualitatively dominant case study series to review management systems for CR in rural and remote areas of NQ was undertaken. Data collection was via semi-structured interviews in four tertiary hospitals and four rural or remote com

Understanding the complexities of recruitment and retention of allied health professionals in rural health settings across Aotearoa: a qualitative study.

BACKGROUND: Rural and remote communities in Aotearoa New Zealand face significant challenges in recruiting and retaining Allied Health Professionals (AHPs). While targeted investment exists to increase the numbers of doctors and nurses entering the rural workforce, comparatively little attention has been given to Allied Health Scientific and Technical professions. This study aimed to explore what matters to AHPs’ in rural contexts and how these insights could inform recruitment and retention practices. METHODS: Drawing on Interpretive Descriptive methodology, semi-structured interviews were co

The relationship between physical health and fear of death in rural residents: The mediation effect of meaning in life and mental health.

The current research used questionnaire data to examine the direct and indirect paths between physical health and fear of death. For 386 rural residents, physical health, meaning in life, and mental health were negatively related to fear of death. Physical health affected fear of death through three paths: one was the independent mediation of meaning in life, the other was the independent mediation of mental health, and the third was the serial mediation of meaning in life and mental health. To reduce the fear of death and improve the quality of life among rural residents, educational interven

The Role of Health Extension Workers in Linking Pregnant Women With Health Facilities for Delivery in Rural and Pastoralist Areas of Ethiopia.

BACKGROUND: Women's preference to give birth at home is deeply embedded in Ethiopian culture. Many women only go to health facilities if they have complications during birth. Health Extension Workers (HEWs) have been deployed to improve the utilization of maternal health services by bridging the gap between communities and health facilities. This study examined the barriers and facilitators for HEWs as they refer women to mid-level health facilities for birth. METHODS: A qualitative study was conducted in three regions: Afar Region, Southern Nations Nationalities and People's Region and Tigra

Rural Federally Qualified Health Center Interventions Targeting Persons with Diabetes: A Systematic Review.

Diabetes and its complications are serious public health problems, disproportionately affecting rural Americans. Contributing to the literature, this systematic review examines intervention studies targeting people with diabetes (PWDs) receiving care in rural federally qualified health centers (FQHCs). The study (N=16) is informed by the Study Design, Data, Methods, Outcomes (SDMO) framework. Overall, findings revealed that rural FQHC interventions targeting PWDs were of serious or high risk of bias. Most studies (69%) used pre-experimental, pre-post intervention designs, often embedded within

Low-Latency Digital Health Framework for Rural Areas Leveraging Fog Computing and 5G.

BACKGROUND: The rural locations of telemedicine and urgent care are slowed down by communication latency as a result of the poor network infrastructure and overreliance on centralized cloud computing that adds to time lag in responding. Even with the development of networking technology, reliable low-latency systems to support rural areas are yet to be developed. METHODS: The current paper suggests a Fog-5G Latency Optimization (F5GLO) application framework, which integrates 5G connectivity and fog computing to permit local data processing. To reduce the transmission delay and latency, health

Pathways between climate change and HIV health in rural Kenya: a qualitative analysis.

Climate change-related severe weather events are heavily impacting regions with the highest prevalence of HIV, creating additional vulnerabilities for already vulnerable populations. Little is understood about how people perceive or experience the mechanisms by which extreme weather events affect the health of people living with HIV (PLHIV). We conducted a qualitative study using in-depth, semi-structured interviews with 40 PLHIV enrolled in a cluster randomized clinical trial that included 8 pairs of health facilities in rural Western Kenya, who were 18 years or older, receiving ART for > 6 m

COVID-19 in Rural Ontario Communities: Exploring Women's Mental Health During a Pandemic.

PURPOSE: Socio-demographic inequities in mental health were magnified by COVID-19, with women experiencing greater household burden with less support in Canada and globally. While some health patterns during COVID-19 have been observed globally, there is a research gap in rural mental health during COVID-19 in Canada. We hypothesize there is a disparity in mental health decline during COVID-19 between men and women. METHODS: In rural Ontario, mental health was measured through a survey of approximately 18,000 individuals living in seven counties. In 2021, survey respondents were asked to rate

Exploring Health Care Barriers for the Unhoused: Insights from a Rural Midwestern Community.

INTRODUCTION: People experiencing homelessness are more likely than the general population to have chronic health conditions and often encounter significant barriers to health care access. Many of these barriers can be affected by community-based factors, such as availability of reliable transportation, past experiences with health care systems, and community attitudes toward the unhoused population. This project aims to assess the needs and barriers to health care identified by people experiencing homelessness in a rural Midwestern city. METHODS: The survey used was adapted from a survey pre

Associations between Driving Cessation and Mental Health among Rural and Urban Older Adults.

Driving cessation is robustly associated with poor mental health outcomes among older adults; however, the magnitude of this relationship may differ by rurality. This study examined cross-sectional and longitudinal associations between driving cessation and life satisfaction and depressive symptoms and assessed whether these relationships were moderated by rurality. Data is from participants in the 2014 and 2016 Health and Retirement Study (HRS) (mean age = 73; 58% female) with information on either depressive symptoms (n=5,650) or life satisfaction (n=1,931). Multivariate linear regression mo

Air pollution and children's mental health in rural areas: compositional spatio-temporal model.

Air pollution stands as an environmental risk to child mental health, with proven relationships hitherto observed only in urban areas. Understanding the impact of pollution in rural settings is equally crucial. The novelty of this article lies in the study of the relationship between air pollution and behavioural and developmental disorders, attention deficit hyperactivity disorder (ADHD), anxiety, and eating disorders in children below 15 living in a rural area. The methodology combines spatio-temporal models, Bayesian inference and Compositional Data (CoDa), that make it possible to study ar

Plasmodium falciparum community prevalence and health-seeking behaviours in rural Sussundenga District, Mozambique.

BACKGROUND: Impacts of nationally directed malaria control interventions hinge on understanding malaria transmission and prevention at the community level. The decision to seek care or health-seeking behaviours provide valuable insight on knowledge of malaria, access to care, and efficacy of malaria case management. Thus far, few studies have focused on central Mozambique. The aim was to describe community level Plasmodium falciparum prevalence and health-seeking behaviours among residents of Sussundenga, Mozambique, a rural village in Manica Province with high malaria incidence reported at th
