Tuberculous Sacroiliitis Presenting as a Large Gluteal Abscess.

Tuberculous sacroiliitis is a rare form of extrapulmonary tuberculosis. Its nonspecific clinical presentation often leads to delayed diagnosis and treatment, increasing the risk of severe complications such as deformities and functional limitations. We report the case of a 67-year-old male patient who presented with a progressively enlarging gluteal mass and weight loss. Clinical examination revealed a 20 × 20 cm soft, non-tender mass in the right gluteal region, associated with limited joint mobility. Computed tomography (CT) identified osteolytic lesions in the right sacroiliac joint and a c…

[Clinical and imaging features of infective sacroiliitis in children].

Objective: To summarize the clinical, radiological characteristics, and prognosis of infectious sacroiliitis in children. Methods: A case-control study was conducted, including 12 cases of infectious sacroiliitis diagnosed in the Rheumatology and Immunology Department of the Children's Hospital affiliated with the Capital Institute of Pediatrics from June 2018 to June 2023. These cases comprised the case group. Concurrently, 28 cases of pediatric idiopathic arthritis involving the sacroiliac joint in the same department served as the control group. Basic patient information, clinical features,…

Positive HLA-B27 and sacroiliitis is not always spondyloarthritis.

A 36-year-old man was treated for several years with multiple agents for ankylosing spondylitis based on positive human leukocyte antigen-B27 and sacroiliitis. He was also diagnosed with osteoporosis and hypophosphatemia. Over these years, from being an avid runner, he became dependent on a walker for ambulation. The lack of treatment response and the low phosphorus were clues that eventually led to a diagnosis of tumor-induced osteomalacia. This case discusses the importance of not solely relying on genetic markers and sacroiliitis for diagnosing ankylosing spondylitis as other conditions can…

Unilateral infective sacroiliitis in a boy presenting with a limp.

A 9-year-old boy admitted to a district general hospital with a 1-week history of fever and a 2-day history of right hip pain. Initial workup revealed raised inflammatory markers and unremarkable imaging studies. After clinical review, there was minimal improvement of the patient's condition 5 days after presentation; therefore, an MRI of the pelvis/hips was carried out, which supported a clinical diagnosis of right-sided infective sacroiliitis. Infective sacroiliitis is rare and only represents 1%-2% of septic arthritis in children. The condition still remains a diagnostic challenge first due…

Spondyloarthropathy: diagnostic imaging criteria for the detection of sacroiliitis.

Diagnostic imaging is crucial to the diagnosis and monitoring of spondyloarthropathies. Magnetic resonance imaging is the most relevant tool for the early detection of sacroiliitis, allowing the institution of therapeutic strategies to impede the progression of the disease. This study illustrates the major criteria for a magnetic resonance imaging-based diagnosis of spondyloarthropathy. The cases selected here present images obtained from the medical records of patients diagnosed with sacroiliitis over a two-year period at our facility, depicting the active and chronic, irreversible forms of t…

Sacroiliitis mimics: a case report and review of the literature.

BACKGROUND: Radiographic sacroiliitis is the hallmark of ankylosing spondylitis (AS), and detection of acute sacroiliitis is pivotal for early diagnosis of AS. Although radiographic sacroiliitis is a distinguishing feature of AS, sacroiliitis can be seen in a variety of other disease entities. CASE PRESENTATION: We present an interesting case of sacroiliitis in a patient with Paget disease; the patient presented with inflammatory back pain which was treated with bisphosphonate. This case demonstrates comorbidity with Paget disease and possible ankylosing spondylitis. We also present a review …

Acupuncture and postpartum pyogenic sacroiliitis: a case report.

INTRODUCTION: Pyogenic sacroiliitis, a rare form of septic arthritis, occurs in patients following trauma, intravenous drug use, genitourinary infections and pregnancy. Here we report a rare case where both acupuncture and pregnancy served as predisposing risk factors to the development of this infection. CASE PRESENTATION: A 33-year-old white woman received several sessions of acupuncture treatment during her gestation at the site of her sacroiliac joint for sciatica; she developed biopsy-confirmed sacroiliitis with methicillin-sensitive Staphylococcus aureus during the immediate postpartum …

Sacroiliitis as an initial presentation of acute lymphoblastic leukaemia.

A 17-year-old male was admitted to the hospital because of fever, low back pain and knee pain which had started a couple of months ago. Pelvic CT-scan confirmed left sacroiliitis. Complete blood count revealed pancytopenia.A bone marrow biopsy and aspiration was performed under local anaesthesia. The pathologist reported Acute Lymphoblastic Leukaemia. The reported patient is the first case of acute lymphoblastic leukaemia, accompanied by sacroiliitis.

[Miliary tuberculosis presented as a sacroiliitis: a case report].

Miliary tuberculosis is a clinical picture due to hematogenous dissemination of a very large amount of bacilli. We present a case with miliary tuberculosis in which the diagnosis and treatment was delayed due to focusing on complaints about the sacroiliac joint. The 28-year-old male patient was admitted with a five-month history of pain in his left sacroiliac joint which was not traumatic. As a definitive diagnosis could not be reached by means of laboratory tests performed, an open biopsy was planned. In the chest X-ray obtained for routine preoperative evaluation, diffuse, bilateral opacitie

Pyomyositis of the iliacus muscle complicated with septic sacroiliitis.

We report a rare case of pyomyositis of the iliacus muscle in a 29-year-old woman. After 2 weeks of adequate treatment, secondary septic sacroiliitis occurred, a complication that had not been described previously. Pyomyositis of the iliacus muscle must be considered in the differential diagnosis of acute pain in the hip region.

An assessment of back pain and the prevalence of sacroiliitis in sarcoidosis.

OBJECTIVES: Sarcoidosis is a chronic granulomatous multisystem disease in which arthritis is relatively common. Arthritis of the sacroiliac joints (sacroiliitis) has been described in sarcoidosis but is thought to be rare. The objective of this study was to determine the prevalence of sacroiliitis in a secondary-care population of patients with sarcoidosis. METHODS: Patients attending a specialist secondary-care sarcoidosis clinic underwent evaluation of spinal symptoms using a standard back pain questionnaire, examination of spinal mobility, and laboratory measurements of erythrocyte sedimen…

Tuberculous sacroiliitis. A case report and review of the literature.

BACKGROUND: Infections of the musculoskeletal system are tuberculous in nature in 1-5% of cases. The sacroiliac joint is involved in 3-9.7%. We describe the case of a 32-year-old man with tuberculous sacroiliitis presented as a growing mass on the lateral aspect of his right proximal and mid-thigh. Open biopsy, histology, cultures and PCR established the diagnosis of tuberculosis. RESULTS: After surgical drainage of the abscess, the patient was administrated a triple antibiotic regimen for 12 months. Seven years postoperatively, the patient is disease-free with no functional limitation. This …
