Hospital policies for opioid use disorder treatment: A policy content analysis and environmental scan checklist.

OBJECTIVE: Hospital-based delivery of opioid agonist therapy ([OAT]; buprenorphine, methadone) is an often-overlooked component of the opioid use disorder (OUD) care continuum. Hospitals are complex clinical environments and organizational policies may inform access to care. This study aimed to identify and describe OUD-related hospital policies. METHODS: We obtained policies through a purposive sampling of addiction physicians affiliated with 10 U.S. hospitals. Experts provided 25 documents that we analyzed using a framework analysis. We then assessed policy concordance with national recomme

[Anti-cytokine therapy: present status and future perspectives in nephrology].

The increasing understanding of the role of cytokines in chronic inflammatory disease, autoimmunity and neoplastic disease has led to a new generation of therapeutic agents, the anti-cytokine blocking agents. In this article, we review current knowledge of two different available approaches: the use of Thalidomide and the anti-cytokine antibody immune therapy. Thalidomide is an immunodulatory and antiangiogenic drug; the most pronounced effect of this drug is the inhibition of tumor necrosis factor-alpha (TNF-alpha ) production. A few years after its withdrawal from the European and Canadian m

Can Microneedle-Nanocarrier Platforms Deliver True Tolerogenic Immunotherapy for Psoriasis?

Psoriasis is a chronic, relapsing skin disease driven by failed immune tolerance toward self-antigens and sustained IL-23/IL-17 signaling. Systemic drugs and biologics control disease in many patients, yet they do not restore lasting tolerance, and most patients relapse after treatment withdrawal. Tolerogenic vaccines aim to re-educate the cutaneous immune system: when administered under non-inflammatory conditions, they push antigen-presenting cells toward tolerogenic dendritic cells (tDCs) and regulatory T cells (Tregs) instead of pathogenic effectors. Because the hyperkeratotic stratum corn

Sequential antibody induction for immune tolerance in clinical organ transplantation: a feasibility review of immunosuppressant withdrawal protocols.

Long-term immunosuppressant use after solid organ transplantation causes serious complications, including chronic rejection, infection, malignancy, and metabolic disorders, which has motivated the clinical pursuit of immune tolerance induction. Sequential antibody induction protocols - using peri-transplant antibodies (e.g., alemtuzumab, anti-thymocyte globulin, belatacept, anti-CD40 monoclonal antibodies) combined with phased reduction or withdrawal of maintenance drugs - aim to establish "operational tolerance" or complete tolerance. This review systematically examines the immunological rati

Development of a complex 3D in vitro alternative model to evaluate the safety of advanced materials.

Drug-induced liver injury (DILI) i.e., liver damage caused by pharmaceutical and other chemical substances, represents a significant clinical challenge in both its diagnosis and treatment, with DILI remaining a major factor in drug attrition and market withdrawal, accounting for approximately 50 % of reported cases of acute liver failure. For decades, conventional in vitro two-dimensional (2D) monolayer cultures and in vivo animal models have been the gold standard in preclinical studies of DILI. However, these models exhibit critical limitations in the accurate replication of metabolic functi

Sequential immunotherapy for alopecia areata: phase-driven immune rebalancing.

INTRODUCTION: Alopecia areata (AA) is an immune-mediated non-scarring alopecia characterized by T lymphocytes attacking hair follicles (HFs), which has a seriously harmful impact on physical and mental health. The challenges in the treatment of AA lie in the recurrence after drug withdrawal and the treatment-resistant cases. Sequential immunotherapy may be able to address some of these issues. AREAS COVERED: As a new treatment model, sequential immunotherapy employs temporally coordinated immunomodulatory interventions with distinct therapeutic focuses. For patients with recurrence, the purpo

[Safety of atazanavir in patients with HIV and hepatitis B and/or C virus coinfection].

Atazanavir is a protease inhibitor indicated, in combination with other antiretrovirals, as an initial treatment of HIV infection or in previously treated patients. Antiretroviral treatment based on atazanavir has been associated with a low incidence of hepatotoxicity, both in Clinical Trials as well as in cohort studies. However, the finding of hyperbilirubinaemia has been common in these studies, although it usually does not involve withdrawing the treatment. In patients co-infected with hepatitis B or C, the level of virological response to does not appear to be affected and the incidence o

ChemBioHepatox: Multimodal Integrating Chemical Structure and Biological Fingerprint for Robust and Interpretable Hepatotoxicity Prediction.

Drug-induced liver injury (DILI) is a leading cause of clinical trial attrition and postmarketing withdrawal and a major contributor to acute liver failure. As regulators increasingly encourage human-relevant, nonanimal approaches, accurate and interpretable computational tools for liver safety are needed. Existing hepatotoxicity models are limited by activity cliffs, restricted applicability domains, accuracy, and poor interpretability. We developed ChemBioHepatox, a multimodal framework that couples chemical structure with biological assay responses to improve predictive performance and mech

Metformin-associated lactic acidosis: Bridging pharmacokinetic determinants, metabolic pathways, and clinical outcomes.

Metformin is the first-line oral anti-hyperglycemic agent for the treatment of type 2 diabetes mellitus due to its effectiveness, cost-efficiency, and acceptable safety profile. Although metformin possesses a favorable safety profile, a rare but serious adverse event known as metformin-associated lactic acidosis (MALA) may occur, particularly in patients with impaired renal clearance or predisposing comorbidities that favor drug retention. High anion-gap metabolic acidosis, increased concentrations of lactate (>5 mmol/L), and arterial pH of less than 7.35 are the hallmarks of MALA. MALA has a
