PURPOSE: The aim of the current study is to reveal the morphological features and nutrient foramina distribution of scaphoid bone in a Turkish population, which may be helpful to diagnosis and surgical treatment of scaphoid fractures.
METHODS: One hundred and two (51 right, 51 left) dry scaphoid bone were measured. The following measurements were performed: length, width, area of articular surfaces, mid-peripheral width, number of posterior grooves, width and length of the grooves, length of the scaphoid tubercle and number of nutrient foramina, proximal or distal location of these foramina, …

Scaphoid fractures are the most frequent fractures of carpal bones, accounting for 70 to 80% of all carpal fractures. Appropriate and early diagnosis of scaphoid fractures is imperative. To relieve symptoms and prevent degenerative changes, scaphoid non-union surgery should aim at healing and restoration of the normal anatomy, and correction of carpal deformities. Since the scaphoid non-unions are a heterogeneous group, and categorization is not easy, the management remains controversial with a reported failure rate as high as 40% and the absence of a "gold standard" treatment. Based on (a) Qu

PURPOSE: Rotational instability of scaphoid fracture nonunions can lead to persistent nonunion. We hypothesized that a hybrid Russe technique would provide improved rotational stability compared with an instrumented corticocancellous wedge graft in a cadaver model of scaphoid nonunion.
METHODS: A volar wedge osteotomy was created at the scaphoid waist in 16 scaphoids from matched-pair specimens. A wedge was inset at the osteotomy site or a 4 × 16-mm strut was inserted in the scaphoid and a screw was placed along the central axis (model 1). The construct was cyclically loaded in torsion until

PURPOSE: Conventional treatment of scaphoid nonunion with collapse deformity and bone loss involves the use of a structural corticocancellous bone graft. With internal screw fixation, we propose that length and alignment can be restored and maintained using only cancellous interposition graft.
METHODS: We describe 12 patients with established scaphoid waist nonunions. Average patient age was 22 years, all were male, and 8 cases involved the dominant wrist. Time from injury to treatment averaged 11 months. We performed open reduction through an anterior approach with correction of the deformit

PURPOSE: This study examined whether multidetector computed tomography (CT) is superior to bone scintigraphy for diagnosis of an occult scaphoid fracture.
METHODS AND MATERIALS: In a study period of 22 months, 100 consecutive patients with a clinically suspected scaphoid fracture and no fracture on scaphoid radiographs, were evaluated with CT within 24 hours after injury and bone scintigraphy between 3 and 5 days after injury. The reference standard for a true (radiographic occult) scaphoid fracture was either (1) diagnosis of fracture on both CT and bone scintigraphy or (2) in case of discre

The wrist surgeon must be conversant with the many methods of treating scaphoid nonunions. Cancellous bone grafting combined with K-wire fixation is a relatively straightforward technique that has been shown in several studies to predictably produce healing of scaphoid nonunions. Although there may be a trend toward early use of vascularized bone grafts for the treatment of scaphoid nonunions, it would seem reasonable to reserve that procedure for cases in which the vascular supply to the scaphoid fragments is compromised, and for which there is little hope that a nonvascularized bone graft wi

Stable fixation of small proximal pole nonunions of the scaphoid by conventional techniques of bone grafting remains difficult. Because of this, long-lasting immobilisation is necessary. Treatment by retrograde Herbert-screw fixation offers the possibility of stable osteosynthesis resulting in shorter immobilisation. Even in case of avascular proximal fragments, it allows healing of the nonunion. We report on the 29 months follow-up of 23 patients treated in this manner showing bony consolidation in 17 cases (74%). Four patients demonstrated fibrous nonunion with minor symptoms, and in two pat

From 1984 to 1993, 30 patients with persistent scaphoid non-union despite bone grafting procedures underwent additional reconstructive surgery. Twenty-five patients were followed-up between one and eleven years after revision with a mean follow-up time of 4.8 years. Fifteen patients had bone graft procedures alone, ten patients had additional internal fixation with the Herbert screw or Ender plate. At follow-up, scaphoid union was achieved in 16 patients (64%), 19 patients had arthritic changes of the radiocarpal joint. Only patients with scaphoid union showed satisfactory functional results.

UNLABELLED: We present the surgical outcomes of non-vascularized bone grafting taken from the iliac crest in 24 patients with scaphoid nonunion and avascular necrosis. The Fisk-Fernandez technique was used in 11 patients, and cancellous bone grafting was used in 13 patients. Bony union was achieved in 22 of the 24 patients. Non-vascularized iliac bone grafting can be used for the surgical management of scaphoid nonunion with avascular necrosis. Although revascularization of the proximal fragment after surgery was not evaluated, bony union was confirmed in nearly all patients.

BACKGROUND: Operative treatment of the scaphoid nonunion includes avascular corticocancellous and cancellous bone grafts and increasingly the use of vascular pedicled and free vascular corticocancellous grafts. Especially the latter require a fair amount of operative expertise and experience in microsurgical techniques.
OBJECTIVE: Which criteria lead to the decision for or against an avascular graft used for reconstruction of scaphoid nonunion? Is our current classification system of a scaphoid pseudarthrosis able to illustrate our diagnostic potential in a way that makes us refer to operativ

Scaphoid nonunion is a complex problem that may continue to exist despite conventional methods of bone grafting and internal fixation. Vascularized bone grafting has been proposed as a treatment option for scaphoid nonunions with or without avascular necrosis of the proximal pole. Zaidemberg and colleagues first introduced the dorsal distal radius graft that has gained considerable popularity in the past 2 decades. Less commonly, the 2,3 intercompartmental supraretinacular artery pedicled vascularized bone graft has been used successfully with equally promising results. It has become the graft

BACKGROUND: The most controversial problem in treating scaphoid fractures is whether bone grafts are necessary for cystic-type fractures.
METHODS: We treated 105 scaphoid fractures using Herbert screws (1988-1997), AO 3.0 mm cannulated screws (1998-2002), and Acutrak screws (2003-2006). The patients ranged in age from 14 years to 67 years (average, 26 years). Our classifications were based on the radiographic findings: linear type (51 cases); cystic type (24 cases); and sclerotic or displaced type (30 cases). Linear and cystic types did not have any displacement more than 2 mm. If the fractur

A new vascularized bone graft from the base of the second metacarpal was used to treat scaphoid non-union and Kienböck's disease. In two patients with scaphoid non-union, the procedure promoted healing, even when the proximal segment was poorly vascularized. In one patient with Kienböck's disease, the graft held stable and wrist pain was markedly reduced. Further clinical experience may establish this procedure as an option for carpal bone fractures or disease.

Thirty-eight scaphoid nonunions were treated by iliac crest bone grafting and staple fixation. A distractor was inserted through an anterior incision and the scaphoid was distracted to restore its original length. The nonunion was then grafted and held with a staple. Consolidation was achieved in 36 cases despite 6 cases of partial avascular necrosis. Pain at the graft site, which was usually transient, was the only complication. Wrist extension was significantly improved. The main indication for this procedure is pseudarthrosis of the middle third of the scaphoid.
