Bone vibration-induced nystagmus is useful in diagnosing superior semicircular canal dehiscence.

OBJECTIVE: To explore vestibular integrity by bone vibration-induced nystagmus (VIN) during bedside examination. Disease or dysfunction of vestibular end organs would reduce or eliminate their contribution to total eye movement response to VIN. BACKGROUND: It is assumed that vibration of the mastoid (at a frequency of 100 Hz) stimulates all vestibular end organs (semicircular canals and otolith structures). Previous studies have described oculomotor responses to vestibular activation by vibratory stimulus. Stimulation of individual semicircular canals produces eye movement in the plane of the

Mechanism of posterior semicircular canal stimulation in patients with benign paroxysmal positional vertigo.

A quantitative study of the stimuli and vestibulo-ocular response associated with benign paroxysmal positional vertigo (BPPV) was made to test and further develop the canalithiasis theory of BPPV. The angular velocities of the head in the planes of the semicircular canals during the Dix-Hallpike test were measured in four healthy subjects using electromagnetic sensors to record the position of the head in a six degrees of freedom paradigm. Next, the nystagmus reactions in seven patients diagnosed with idiopathic BPPV were recorded with video-oculography. The characteristics of the vestibulo-oc

Sound- and pressure-induced vertigo associated with dehiscence of the roof of the superior semicircular canal.

In many types of peripheral vertigo, imaging is not part of the initial evaluation. We present a patient with sound- and pressure-induced vertigo associated with bony dehiscence of the roof of the superior semicircular canal. The diagnosis of this new entity can only be made by high-resolution coronal CT imaging of the temporal bones. In patients with this symptom complex, CT should be performed early in the diagnostic workup.

Interaction of semicircular canal stimulation with carotid baroreceptor reflex control of heart rate.

The carotid-cardiac baroreflex contributes to the prediction of orthostatic tolerance; experimental attenuation of the reflex response leads to orthostatic hypotension in humans and animals. Anecdotal observations indicate that rotational head movements about the vertical axis of the body can also induce orthostatic bradycardia and hypotension through increased parasympathetic activity. We therefore measured the chronotropic response to carotid baroreceptor stimulation in 12 men during varying conditions of vestibulo-oculomotor stimulation to test the hypothesis that stimulation of the semicir

Vestibulo-sympathetic Reflex and Pupillary Responses Elicited by Vertical Semicircular Canal Stimulation.

The vestibulo-sympathetic reflex has been studied mainly using horizontal semicircular canal and otolith stimulation, whereas the autonomic role of the vertical canals remains unclear. We quantified cardiovascular and pupillary responses to vertical canal stimulation and compared stimulus-frequency-locked pupil oscillations between vertical and horizontal rotations in 18 healthy adults. Vertical stimulation used a 70°-tilted rotary chair aligning the vertical canal planes with the earth-horizontal axis. Participants underwent sinusoidal harmonic acceleration at 0.08, 0.16, and 0.64 Hz and the

Validation of NB CE-Chirps in the Diagnosis of Superior Semicircular Canal Dehiscence Syndrome.

Background/Objectives: The aim of this study was to assess NB CE-Chirps for diagnosing Superior Semicircular Canal Dehiscence Syndrome (SSCDS) with cervical and ocular vestibular evoked myogenic potentials (cVEMPs and oVEMPs), and to compare them with Tone Bursts (TBs). Methods: Nine subjects diagnosed with SSCDS were included (four men/five women, median = 61 years, range = 31-79 years). Intensity thresholds at 500 Hz were investigated with both stimuli. A response was also sought when NB CE-Chirps and TBs were delivered at 4000 Hz for c and oVEMPs. Results: Both 500 Hz TBs and 500 Hz NB CE-C

Assessment of Horizontal Semicircular Canal and Graviceptive Perception Before and After Cochlear Implantation.

Background/Objectives: Cochlear implantation (CI) effectively treats severe-to-profound sensorineural hearing loss, but its vestibular effects remain under investigation. This retrospective study assessed perioperative changes in horizontal canal function (video head impulse test, vHIT) and graviceptive perception (subjective visual vertical, SVV). Methods: Forty-three adults (mean age: 59 ± 15 years) implanted between May 2017 and July 2018 were assessed pre- and postoperatively. The prespecified primary analysis was the paired comparison of vHIT gain at 60 ms in the implanted ear, performed

Headache resolution following the middle fossa repair of superior semicircular canal dehiscence.

OBJECTIVE: To investigate predictors of headache resolution following surgical intervention for superior semicircular canal dehiscence (SSCD) syndrome. METHOD: We conducted a retrospective chart review on 344 surgical repairs of SSCD via the middle cranial fossa (MCF) approach conducted at an institution. The primary outcome measure was the resolution of headaches after SSCD repair. Analyses explored the predictive potential of several comorbidities and lifestyle factors pre-operatively. RESULTS: Among a total of 344 repairs, 193 (56 %) reported headaches preoperatively. Among which, 114 (59

Treatment of Meniere's disease with simultaneous triple semicircular canal occlusion and cochlear implantation.

OBJECTIVE: Report three cases of simultaneous triple semicircular canal occlusion (TSCO) and cochlear implantation (CI) as the treatment of intractable Meniere's disease (MD). CASE REPORTS: Patients with MD can present occasionally with intractable vertigo and profound sensorineural hearing loss (SNHL). TSCO and CI have been proposed to control vertigo and restore profound deafness in patients with MD separately. However, a few studies have reported simultaneous TSCO and CI in the same surgical procedure for the treatment of MD. In the present study, we described three patients with MD showin

Traumatic superior semicircular canal dehiscence syndrome: case report and literature review.

Superior semicircular canal dehiscence (SSCD) syndrome was first reported in 1998 by Minor et al. and comprises a spectrum of auditory and vestibular symptoms as a result of 'mobile third window' mechanism. The aetiology of SSCD is debated, but persistent infantile microstructure of the temporal bone was suggested. However, some authors related a 'second event', such as closed head trauma, temporal bone fracture and sudden increase in the intracranial pressure to the precipitation of its symptoms. In this article, we report a patient with a closed head trauma who developed unilateral auditory

Bone Metabolic Markers in the Clinical Assessment of Patients with Superior Semicircular Canal Dehiscence.

BACKGROUND: Superior semicircular canal dehiscence (SSCD) is a bony defect in the osseous shell of the petrous temporal bone. The pathophysiological association between osteoporosis and SSCD remains poorly understood. We investigated the relationship between bone metabolic markers and symptoms in patients with SSCD. METHODS: We collected patient demographics and clinical parameters for adult patients diagnosed with SSCD on high-resolution computed tomography scans. We used point-biserial correlation analysis to investigate the relationship between bone metabolic markers and symptoms in patien

Mechanisms of Hearing Loss in a Guinea Pig Model of Superior Semicircular Canal Dehiscence.

Defective acoustic transmission in the cochlea is closely related with various auditory and vestibular symptoms. Among them, semicircular canal dehiscence (SCD) with a defective semicircular bone is typical. Currently, the pathogenesis of SCD is usually explained by the third window hypothesis; however, this hypothesis fails to explain the variability in the symptoms and signs experienced by superior SCD (SSCD) patients. We evaluated the mechanism of hearing loss in a guinea pig model of bony dehiscence with various sizes and locations along the superior semicircular canal. Auditory brainstem
