Verocytotoxigenic (Shiga toxin-producing) Escherichia coli: virulence factors and pathogenicity in the farm to fork paradigm.

Verocytotoxigenic Escherichia coli (VTEC) are a good example of the evolution and emergence of pathogenic E. coli. Unknown before the late 1970s, these bacteria are a major cause of hemorrhagic colitis and hemolytic uremic syndrome worldwide. The production of verocytotoxins is the main virulence feature of VTEC but cannot be solely responsible for full pathogenicity. VTEC associated with severe human disease are usually capable of colonizing the intestinal mucosa with a characteristic attaching-and-effacing mechanism, genetically governed by the locus of enterocyte effacement, and possess oth

Fabry disease in mice protects against lethal disease caused by Shiga toxin-expressing enterohemorrhagic Escherichia coli.

Fabry disease is an X-linked recessive disorder in which affected persons lack alpha-galactosidase A (alpha -GalA), which leads to excess glycosphingolipids in tissues, mainly globotriaosylceramide (Gb3). Gb3 is the cellular receptor for Shiga toxin (Stx), the primary virulence factor of enterohemorrhagic Escherichia coli. alpha-GalA-knockout mice were significantly protected against lethal intraperitoneal doses of Stx2 or oral doses of Stx2-expressing bacteria, compared with wild-type (wt) control mice. Kidneys of moribund wt mice revealed tubular necrosis, but no histopathologic changes were

Reduced nitric oxide bioavailability in a baboon model of Shiga toxin mediated hemolytic uremic syndrome (HUS).

BACKGROUND: Although there is agreement that post-diarrheal hemolytic uremic syndrome (D+ HUS) is caused by Shiga toxin (Stx)-producing E. coli, little is known about factors that mediate the host response to these toxins and potentially contribute to pathogenesis. Nitric oxide (NO) is a candidate mediator by virtue of its antiplatelet and renal vasodilatory properties. METHODS: We used a baboon model of HUS to measure plasma and urinary NO metabolites and expression of NO synthase (eNOS and iNOS) in renal tissue following the intravenous administration of Stx-1. RESULTS: Plasma concentratio

Bacteriophage cell lysis of Shiga toxin-producing Escherichia coli for top-down proteomic identification of Shiga toxins 1 & 2 using matrix-assisted laser desorption/ionization tandem time-of-flight mass spectrometry.

RATIONAL: Analysis of bacteria by matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOFMS) often relies upon sample preparation methods that result in cell lysis, e.g. bead-beating. However, Shiga toxin-producing Escherichia coli (STEC) can undergo bacteriophage-induced cell lysis triggered by antibiotic exposure that may allow greater selectivity of the proteins extracted. METHODS: We have developed a sample preparation method for selective extraction of bacteriophage-encoded proteins and specifically Shiga toxins 1 and 2 (Stx1 & 2) expressed from STEC strai

Low-Cost Charged-Coupled Device (CCD) Based Detectors for Shiga Toxins Activity Analysis.

To improve food safety there is a need to develop simple, low-cost sensitive devices for detection of food-borne pathogens and their toxins. We describe a simple, low-cost webcam-based detector which can be used for various optical detection modalities, including fluorescence, chemiluminescence, densitometry, and colorimetric assays. The portable battery-operated CCD-based detection system consists of four modules: (1) a webcam to measure and record light emission, (2) a sample plate to perform assays, (3) a light emitting diode (LED) for illumination, and (4) a portable computer to acquire an

Shiga-toxin-induced firm adhesion of human leukocytes to endothelium is in part mediated by heparan sulfate.

BACKGROUND: Shiga toxin (Stx) is the main pathogenic factor in the haemolytic-uraemic syndrome (HUS). Stx damages the renal endothelium, which leads to inflammation and coagulation. Endothelial heparan sulfate proteoglycans (HSPG), and heparan sulfate in particular, play an important role in the inflammatory process by acting as a ligand for l-selectin. Furthermore, leukocytes are able to interact with chemokines bound to HSPG (examples are IL-8, RANTES and MCP-1). This leads to an activation of integrins on leukocytes and results in more stable leukocyte-endothelial wall adhesion. In this stu

Response to single and divided doses of Shiga toxin-1 in a primate model of hemolytic uremic syndrome.

Postdiarrheal hemolytic uremic syndrome is caused by Shiga toxin (Stx)-producing Escherichia coli. It was shown previously that the baboon, like the human, has glycolipid receptors for Stx in the gut and the kidney and that a single 50- to 200-ng/kg intravenous dose of purified Stx-1 results in thrombocytopenia, hemolytic anemia, and renal thrombotic microangiopathy. For further characterization of factors that modulate disease expression, the baboon's response to the intravenous administration of 100 ng/kg Stx-1 given either rapidly as a single bolus or slowly as four 25-ng/kg doses at 12-h i

Urgent Colectomy for Colonic Necrosis Following Shiga Toxin-Producing Escherichia coli O157 Infection in an Adult.

INTRODUCTION: Shiga toxin-producing Escherichia coli (STEC) O157 infection is usually treated conservatively; surgery is rarely required and mostly reserved for children with hemolytic uremic syndrome (HUS). We report a case of STEC O157 requiring urgent surgery in an adult without HUS. CASE PRESENTATION: A 73-year-old woman presented with abdominal pain and diarrhea. Abdominal CT showed diffuse wall thickening in the ascending and transverse colon. Infectious enteritis and acute exacerbation of chronic renal failure were diagnosed, and antibiotics and continuous hemodiafiltration were initia

Protective effects of bismuth hydroxide against hemolytic uremic syndrome caused by Shiga toxin producing Escherichia coli.

Shiga toxin-producing Escherichia coli (STEC) is a major cause of foodborne disease and hemolytic uremic syndrome (HUS), a severe condition for which no specific therapies are currently available. The major virulence factor of STEC is Shiga toxin 2 (Stx2), which is encoded by the Stx2-converting bacteriophage 933 W used in this study, and phage induction plays a central role in toxin release and disease progression. In this study, we evaluated the protective in vivo effects of bismuth hydroxide gel (BHOG), a low-cost gastrointestinal compound with previously described antimicrobial and anti-tox

Whole-Genome Sequence Analysis of Shiga Toxin-Producing Escherichia coli Isolated from Livestock Animals in Ghana.

Shiga toxin-producing Escherichia coli (STEC) is a zoonotic pathogen of public health concern, requiring a One Health approach to clarify its transmission and distribution. However, its prevalence and genomic characteristics in livestock and companion animals remain underexplored in low-income countries. We investigated prevalence and genomic features of STEC in animals in western Ghana, representing the first genomic report of STEC in Ghana. Fecal samples (97) were collected from goats (n = 33), sheep (n = 33), dogs (n = 30), and a cat (n = 1), with STEC detected in 12.1% of goats and sheep s

Multidrug resistance and lipopolysaccharide remodeling associated with mcr-1 acquisition in Shiga toxin-producing Escherichia coli.

The emergence of plasmid-mediated colistin resistance in Shiga toxin-producing Escherichia coli (STEC) is an important public health concern because these strains can combine antimicrobial resistance with intestinal pathogenic potential. However, although mcr-harboring STEC has been reported from food-related sources, the phenotypic consequences of mcr-1 acquisition in STEC remain poorly understood. In this study, we investigated the prevalence of mcr variants and associated antimicrobial resistance determinants among 144 mcr-harboring STEC isolates and examined the phenotypic impact of mcr-1

Genomic epidemiology reveals statewide dispersal of clinical Shiga toxin-producing Escherichia coli and their antimicrobial resistome.

Shiga toxin-producing Escherichia coli (STEC) is a zoonotic pathogen that can be transmitted to humans through contaminated food or contact with infected animals. Here, we aim to characterize the population diversity and genome characteristics of the clinical STEC population in New York State collected over a span of six years. A total of 1,655 clinical isolates were analyzed using whole-genome sequencing, phylogenetic reconstruction, in silico identification of acquired antimicrobial resistance genes and plasmid types, and delineation of genetic clusters. The STEC population consists of 178 s

Factors affecting dialysis duration in children with Shiga toxin-producing Escherichia coli-associated hemolytic uremic syndrome.

BACKGROUND: Predicting disease severity can be informative for management of HUS. Dialysis requirement, volume depletion, elevated white blood cell counts, very young age, and use of antimotility agents are known factors associated with severe HUS. METHODS: A retrospective cohort analysis was performed to identify factors associated with dialysis duration using electronic medical record and chart review of 76 children ≤ 18 years of age at presentation with STEC-HUS identified through billing data from July 2008 to April 2020 at James Whitcomb Riley Hospital for Children, Indiana University, I

Molecular epidemiology, clinical features and significance of Shiga toxin detection from routine testing of gastroenteritis specimens.

After introduction of faecal multiplex PCR that includes targets for stx1 and stx2 genes, we found stx genes were detected in 120 specimens from 111 patients over a 31-month period from 2018-2020 from a total of 14,179 separate tests performed. The proportion of stx1 only vs stx2 only vs stx1 and stx2 was 35%, 22% and 42%, respectively. There were 54 specimens which were culture positive, with 33 different serotypes identified, the predominant serotype being O157:H7 (19%). Eighty-two patients had clinical data available; we found a high rate of fever (35%), bloody diarrhoea (34%), acute kidney

Pathogenomes and variations in Shiga toxin production among geographically distinct clones of Escherichia coli O113:H21.

Infections with globally disseminated Shiga toxin-producing Escherichia coli (STEC) of the O113:H21 serotype can progress to severe clinical complications, such as hemolytic uremic syndrome (HUS). Two phylogeographically distinct clonal complexes have been established by multi locus sequence typing (MLST). Infections with ST-820 isolates circulating exclusively in Australia have caused severe human disease, such as HUS. Conversely, ST-223 isolates prevalent in the US and outside Australia seem to rarely cause severe human disease but are frequent contaminants. Following a genomic epidemiology
