An overview of prevention research: issues, answers, and new agendas.

Efforts to curtail alcohol abuse and alcoholism can be divided into primary, secondary, and tertiary prevention. Primary prevention attempts to stop a problem or illness from occurring in the first place. Secondary prevention identifies persons in the early stages of problematic or illness behavior and refers them for counseling or treatment, which is considered tertiary prevention. Five research areas concerned with primary and secondary prevention are selected for discussion: youth, the mass media, the worksite, blacks and Hispanics, and alcohol-related behavior that increases the risk of AI

Living arrangements and women's health.

The recent changes in the traditional patterns of living arrangements in the U.S.A. raise interest in their consequences for the well being of the individuals involved. In this paper the physical health of women in different living arrangements was studied. It was suggested that living arrangements differ in the degree of adult support and the nurturant responsibilities that often accompany women's social support. Since previous research seems to indicate that social support promotes health and nurturant obligations constrains it, it was predicted that women whose living arrangements offer ste

Comparison of rehabilitation outcome between patients with psychophysiologic disorders and organic diseases.

Two groups of patients with either psychophysiologic disorders or organic diseases were examined four weeks prior to an in-patient rehabilitation treatment, at the beginning and at the end of the rehabilitation, and six months thereafter. Data were collected through questionnaires assessing personality dimensions, physical complaints, hypochondriac behaviour, mood, disease concepts, living habits, and socio-economic as well as medical data. The groups were comparable according to a wealth of socio-economic and medical data but differed in extraversion, emotional lability, physical complaints,

Does depressive illness in the elderly have a poor prognosis?

The few available longitudinal studies of depressive illness in the elderly indicate a poor prognosis. Various risk factors are discussed. Both severe and chronic physical illness, and severity of the depressive disorder are the two most important prognostic factors. The possibility of interactional effects between these risk factors should be studied. Most studies to date have been of severely depressed patients and this may give an unduly pessimistic picture. A case is made for the desirability of well-designed prognostic longitudinal studies with subjects being drawn from a wide spectrum of

[Catamnestic results of 64 patients--5 years following inpatient psychosomatic treatment].

The present study is an attempt at evaluating with clinocosomatic, psychodiagnostic, and sociologic instruments the alterations achieved by (psychodynamically oriented) psychosomatic in-patient therapy. Out of a total of 64 patients who in 1975 had undergone treatment for an average period of two months 46 (i. e. 76.7 per cent) were available for re-examination after a five-year interval. In two thirds of the patients the various complaints and the general autonomic instability had receded. The psychical and social situation had improved for between two thirds and three fourths of all re-exami

The fecal bacterial microbiota of healthy and sick newborn foals.

BACKGROUND: The fecal bacterial microbiota of normal foals and foals with enterocolitis has been characterized using next-generation sequencing technology; however, there are no reports investigating the gut microbiota in foals hospitalized for other perinatal diseases. OBJECTIVE: To describe and compare the fecal bacterial microbiota in healthy and sick foals using next-generation sequencing techniques. ANIMALS: Hospitalized (17) and healthy foals (21). METHODS: Case-control study. Fecal samples were collected from healthy and sick foals on admission. Sick foals were further divided into s

Health-related quality of life of children and adolescents with cerebral palsy: hearing the voices of the children.

The aim of the study was to determine whether pediatric patients with cerebral palsy (CP; 24.3% hemiplegic, 30.4% diplegic, 38.5% quadriplegic) can self-report their health-related quality of life (HRQOL) with the Pediatric Quality of Life Inventory Version 4.0 (PedsQL 4.0) Generic Core Scales. The PedsQL was administered to 148 families (84 males, 64 females; mean 10y, SD 3y 9mo, range 5 to 18y) recruited from a children's hospital clinic and state medical therapy clinics. Sixty-nine children (47%) were able to self-report. Children with CP self-reported a significantly lower HRQOL (mean 65.9

Malingering in the medical setting.

Malingering of mental illness has been studied extensively; however, malingered medical illness has been examined much less avidly. While in theory any ailment can be fabricated or self-induced, pain--including lower back pain, cervical pain, and fibromyalgia--and cognitive deficits associated with mild head trauma or toxic exposure are feigned most frequently, especially in situations where there are financial incentives to malinger. Structured assessments have been developed to help detect both types of malingering; however, in daily practice, the physician should generally suspect malingeri

[Chronic fatigue syndrome. Definition, diagnostic measures and therapeutic possibilities].

This article reviews the chronic fatigue syndrome (CFS), a disorder whose etiology is unknown. The diagnostic criteria proposed in 1994 by the CDC and the International Chronic Fatigue Syndrome Study Group are introduced. In contrast to widespread belief, there are no laboratory tests available to underpin the diagnosis of CFS; the diagnosis is made solely on the basis of clinical criteria. In the differential diagnosis, the exclusion of other conditions that can cause chronic fatigue, such as neuropsychiatric or sleep disorders, is of critical importance. In this context, the question as to w

Psychosomatic view of endocrine disorders.

This review is concerned with the clinical and research implications of psychosomatic research in endocrinology. A possible role of life events in the pathogenesis of endocrine disease, the psychological distress associated with acute illness, and the issue of quality of life in clinical endocrinology are surveyed. In particular, the psychosomatic aspects of Cushing's syndrome, Addison's disease, hyperthyroidism, hypothyroidism, hyperprolactinemia, acromegaly, growth hormone deficiency, and hirsutism are considered.

First-hand experience: a transplant recipient's story.

I received that wanted, needed, and feared telephone call. I wanted it for the freedom and hope it might bring. I needed it because of bone disease and fatigue. I feared it because of the surgery, possible complications, and the possibility of the most damning word for any transplant patient--rejection.

Behavioral clusters and coronary heart disease risk.

The purpose of the present study was to empirically identify individuals who differed in their patterns of components derived from the structured interview (SI), and to evaluate whether individuals characterized by the different patterns varied in terms of their risk for coronary heart disease (CHD). The present study represents a reanalysis of data from the Western Collaborative Group Study in which components of Type A were individually related to risk for CHD. Subgroups of individuals who differed in the patterns of their component scores were identified by means of cluster analytic techniq

Neuroscience of HIV infection: basic and clinical frontiers.

Whilst this conference presented no new or exciting breakthroughs in our understanding of the psychosocial or neuropsychological aspects of HIV disease, there were some useful contributions to our knowledge in these areas. Previous work suggesting that HIV seropositive individuals with HIV are no more likely to suffer poor psychosocial adjustment than matched HIV-controls. However, where psychiatric disorders are seen, the most common are adjustment disorders followed by drug and alcohol abuse. Mania, whilst quite rare, may be more commonly seen with HIV than should be expected. People with as

Schizophrenia and Marfan syndrome.

Five index patients and three of their first-degree relatives were affected both by schizophrenia and Marfan syndrome. Since the association appears statistically significant, the possibility of linkage disequilibrium between adjacent genes or a cytogenetic abnormality causing both disorders is suggested. These hypotheses are testable and hold promise in attempting to map the 'schizophrenia susceptibility gene' by the candidate-gene approach.

Münchhausen syndrome: psychopathology and management.

A case report on a patient with Münchhausen syndrome is presented and the psychopathological features are compared to other reports in the literature. Diagnosis and differential diagnosis are discussed with regard to other somatoform disorders such as conversion disorder and somatization disorder as well as to malingering and cenesthesic schizophrenia. The management of Münchhausen syndrome is aggravated by the low compliance in these patients, who expose themselves to serious complications of unnecessary medical and surgical procedures. Early diagnosis could to a considerable extent exclude t

Relation between severity of chronic illness and adjustment in children and adolescents with sickle cell disease.

Tested the relationship between illness severity and adjustment in 70 sickle cell patients, aged 8-16. Illness severity was measured by frequency of hospitalization and emergency room visits, frequency and intensity of pain crises, and duration of illness. Adjustment variables were IQ, self-esteem, social and personal adjustment, behavioral problems, school performance, and peer relations. Medical variables-associated illness patterns and type of hemoglobinopathy-were also considered. Sex and age were covariates. The hypothesis that illness severity would affect adjustment was generally not su

Urethral syndrome: a study in psychosomatic urology.

The authors studied a sampling of 50 female patients diagnosed with urethral syndrome (urinary symptoms and pain without infection or organic lesion) on purpose to explore some psychological aspects. The evaluation procedure began with a complete urodynamic examination followed by a clinical interview. After this all patients were given three questionnaires: Symptom Questionnaire, Illness Behaviour Questionnaire and a Biographic Questionnaire prepared specifically for this study. Statistical analysis of the data showed higher levels of hostility, irritability, anxiety, dysphoria and depression

Physical status and psychosocial adjustment in children with spina bifida.

Investigated the relationship between the physical status and psychosocial adjustment of chronically physically handicapped children. The status of 61 children with spina bifida regarding six specific disease or disability parameters was determined from medical charts. Their mothers completed the Child Behavior Checklist as a measure of the children's psychosocial adjustment. Children with spina bifida were reported to display on the average significantly more behavior and social competence problems than expected for children in general. However, children with differing degrees of physical pro
