Methods to monitor the integrity of cranial motor nerves during operations on skull base tumors have been developed over the past decade. These methods can help the surgeon to identify cranial motor nerves that are located in the surgical field but which may not be visible directly. Methods have also been developed that allow monitoring of the function of sensory systems such as the auditory system and it has been shown that brainstem auditory evoked potentials can provide important information about the integrity of the auditory nervous system. It has been shown in several studies that such n…

The authors describe two cases of giant intradiploic epidermoid cysts of the cranial vault and a case of a large epidermoid arising from the frontal sinus and extending intracranially. The aetiology of epithelial cysts is reviewed. The characteristic radiological and CT scan appearances of diploic epidermoid cysts are described, their clinical presentation and surgical management briefly discussed.

A series of 30 chondroblastomas was reviewed: 21 had occurred in the lateral part of the temporal bone, 6 in the mandible, 1 in the parietal bone, and 2 in the region that included the temporal bone and mandible. Of the 30 patients in the series, 20 were males and 9 were females; the sex of 1 patient was not stated. The ages of 29 patients ranged from 2 years 11 months to 70 years (mean, 43.5 years). Radiologic findings were not suggestive of a specific diagnosis, although the lesions appeared to be benign. Histologically, most tumors were classic chondroblastomas. However, some showed aneurys

Congenital depression of the neonatal skull has had an incidence of 0.1% (1/10 000) in our newborn population during the past 8 years. These skull depressions have two pathogenetic types: deformation without fracture and fracture accompanied by depression. The cause of skull depression being the pressure exerted by the digits and fist of the newborn on his skull has not been previously reported. The treatment of choice for selected cases is nonsurgical elevation with an obstetric vacuum extractor. A CT scan should be performed prior to this treatment to rule out intracranial complications such…

Fifty-four patients with abnormalities primarily involving the base of the skull were evaluated by magnetic resonance (MR) imaging. The results were compared with information obtained by other radiologic studies, primarily X-ray CT. On MR imaging, better anatomic definition of soft tissues in the deep compartments of the nasopharynx was achieved through the high level of tissue contrast discrimination and lack of bone artifacts. The parapharyngeal fat plane, separating pterygoid from pharyngeal musculatures, was consistently demonstrated. However, the inability of MR to image compact bone prov…

X-ray findings and the result of microradiographic examinations of hyperostotic processes of the skull (Osteomas, Meningioma, Fibrous Dysplasia, Morbus Paget) are described. The films permit a reliable diagnosis statement only in a few cases, because similar findings can be seen in different diseases. Microradiographic investigations demonstrate the microstructure of the tissue of hyperostotic processes. There are no specific findings typical for one disease alone, but there are qualitative and quantitative variations in the different diseases mentioned above. Microradiography gives informatio…

A case of paracoccidioidomycosis of the skull is presented. No evidence of a previous lung, mucous membrane, or nervous system infection was found. Radiological and surgical findings suggested osteomyelitis. The authors stress the importance of including paracoccidioidomycosis in the differential diagnosis of osteolytic skull lesions in countries where this fungus is endemic.

The author presents two cases of compound elevated fractures of the skull and discusses the mechanics of their production. These lesions, seldom recognized in modern clinical practice, are first described in the oldest scientific and surgical treatise known, the Edwin Smith Surgical Papyrus, over 5000 years ago.

A new approach to tumors of the skull base is described. This approach is accomplished by forward extension of the translabyrinthine opening into the cerebellopontine angle. The facial nerve is mobilized in the temporal bone from the stylomastoid foramen to its entrance into the internal auditory canal. Having removed the barrier of the facial nerve, additional bone removal can be carried forward to the internal carotid artery, which now becomes the forward limit for temporal bone resection. The access attained through this exposure allows removal of tumors arising from the petrous tip, as wel…

The closure of scalp defects is still surrounded by many uncertainties. Free skin graft has proved most successful. Thiersch graft, split skin graft and full thickness skin graft have their indication for use. Thiersch skin graft is used as a temporary dressing, in order to achieve aseptic wound conditions, but cannot be looked upon as a lasting solution cosmetically and functionally. Split skin is used only for the closure of smaller defects. Full thickness skin grafts are most suitable, because they offer a lasting coverage, which meets to all demands. If the outer table of the skull is expo…
