Obstructive sleep apnea syndrome (OSAS) was diagnosed in157 subjects based on clinical symptoms, physical evaluation, cephalometric x-ray films, and polysomnography. These index cases identified 844 living first-degree relatives. Mailings were sent to 792 (94%). The mailing consisted of two identical questionnaires, one for the family member of the index case and one to be given to a friend (not a relative) of approximately the same age. In response, we received 531 (63%) questionnaires from relatives and 198 (25%) questionnaires from age-matched nonrelated friends, which were used as a contro

For treatment of patients with complex craniosynostosis, increased intracranial pressure, severe exophthalmos, and apnea attacks are important indications for operative intervention. Little has been reported on obstructive sleep apnea in complex craniosynostosis. Obstructive sleep apnea attacks in complex craniosynostosis are evaluated in relation to the upper and middle pharyngeal space and to the dimensions of the maxilla and mandible measured on cephalograms. The value of the pharyngeal air space in craniosynostosis had no relationship with either the frequency of sleep apnea attack or with

We examined the effect of 375 mg theophylline at night in 95 patients (14 women, 81 men) with obstructive sleep apnoea. The apnoea-hypopnoea index decreased significantly from 20.9 +/- 17.2 to 13.5 +/- 15.8/hour, the nadir of oxygen saturation increased from 80.6 +/- 8.5 to 82.4 +/- 8%. A survey of the longterm effects of theophylline in 16 initially responding patients revealed after 31-922 (190 +/- 217) days therapy no statistically significant change of the apnoea-hypopnoea index; however, the nadir of oxygen saturation was lower compared to the beginning of the therapy (83.8 +/- > 79.3 +/-

Uvulopalatopharyngoplasty (UPPP) is a commonly performed procedure for obstructive sleep apnea (OSA). However, results are inconsistent. Patients in whom the UPPP procedure has failed have a smaller change in airway size as compared to responders, and also many demonstrate continued obstruction at the palate. We present a modification, transpalatal advancement pharyngoplasty, that increases upper oropharyngeal and retropalatal airway size by advancing the soft palate. Eleven patients with severe OSA and multiple sites of airway narrowing were corrected by this method. Three patients had prior

To ascertain whether there are structural and functional changes in the upper airway of obstructive sleep apnea syndrome (OSAS) pts. We studied 33 pts of OSAS with positive polysomnographic tests, 14 pts of non-apnea snore with negative polysomnographic tests, and 18 normal subject by using CT scanner (Somatom Dr 3). The results are as followings 1. There were narrowing region in the upper airway in OSAS pts and the non-apnea snore pts. Most of the narrowings were located at the level of oropharynx, which was correspondent to the level of the soft palate and the uvula. The length of the soft p

In order to assess the complications of sleep apnea, we have reviewed a data base of 619 consecutive admissions to a university sleep disorders center. Although patients with obstructive sleep apnea (OSA) described more subjective sleepiness than patients with central sleep apnea (CSA) or primary snoring (PS), the multiple sleep latency test (MSLT) indicated similar levels of physiologic sleepiness in the two apneic groups, which was greater than among those with PS. There was no significant relationship between individual subjective estimates of habitual sleepiness and the MSLT values. Among

Currently, uvulopalatopharyngoplasty (UPPP) is the most common surgical procedure used for the treatment of obstructive sleep apnea. Patients with clinically significant obstructive sleep apnea in whom medical treatment has failed or who are unwilling to comply with medical therapy are considered candidates for UPPP. The initial surgical results obtained in nonselected patients with obstructive sleep apnea were highly variable, approximately half of the patients experiencing more than a 50% reduction in the frequency of disordered breathing events postoperatively. Although differences in surgi

All night polysomnographic studies performed in 149 patients with OSA syndrome revealed marked disturbances of the sleep structure and the disorders of breathing during sleep: obstructive apneas in all patients, hypopneas in 25% and central apneas in 12%. The mean apnea index was 68 +/- 29, the mean percent of sleep time spent in apnea 50 +/- 21%. The typical obstructive apnea duration was 25 +/- 7s in NON REM sleep and 40 +/- 34s in REM sleep. The longest obstructive apnea duration was 48 +/- 24s in NON REM sleep and 71 +/- 42s in REM sleep. During obstructive apneas the typical arterial oxyg

The purpose of this study was to determine whether lateral radiographs of the pharynx and fluoroscopy of the lateral pharynx in the sleeping patient could be used as predictors of surgical success in patients undergoing soft-palate surgery for obstructive sleep apnea. A total of 12 patients had surgery after radiologic evaluation. There were six surgical successes and six failures. No successes occurred in patients with a soft-palate length of less than 46 mm, nor were there any successes among patients whose initial point of obstruction on fluoroscopy was inferior to the level of the soft pal

Auditory brainstem response (ABR) has been used by several investigators to study the role of the brain stem in the pathophysiology of obstructive sleep apnea (OSA). These studies have produced conflicting results. We studied 27 preoperative OSA patients and 17 controls using click stimuli presented at a slow (11.7/second) rate and at a fast (57.7/second) rate. ABR was repeated postoperatively in 18 patients. There were no statistically significant differences in the ABRs of preoperative OSA patients when compared with the control group. However, the preoperative recordings showed statisticall

Although uvulopalatopharyngoplasty has been proposed as a treatment for obstructive sleep apnea, there is limited objective data concerning its efficacy. We have examined 40 patients with moderate to severe obstructive sleep apnea before and 3 months following uvulopalatopharyngoplasty. A standardized questionnaire and detailed overnight sleep study were performed before and after surgery. In addition, 15 patients were retested 1 year postoperatively. Uvulopalatopharyngoplasty resulted in a statistically significant reduction in symptoms, blood pressure, apnea index, and total apneic time. The

Twenty-two infants and children were found to have clinically significant obstructive sleep apnea. A history suggesting complete or partial airway obstruction during sleep was obtained on all patients, and physical examination of the sleeping patient revealed snoring, retractions, or OSA in 21 patients. Nevertheless, the mean delay in referral for 20 patients first seen after the neonatal period was 23 +15 (+ SD) months. Sixteen of 22 patients (73%) developed serious sequelae: cor pulmonale in 12 (55%), failure to thrive in six (27%), permanent neurologic damage in two (9%), and behavioral dis

Sleep apnea occurring in association with neurologic lesions is usually of the central type. We have described a patient in whom obstructive sleep apnea followed a lateral medullary syndrome. We hypothesize that the upper airway obstruction during sleep was due to weakness of the palatal and pharyngeal muscles resulting from the neurologic lesion. The patient initially had reduced respiratory center motor output (mouth occlusion pressure) which increased significantly after tracheostomy. We conclude that obstructive as well as central apnea may follow neurologic lesions. Each patient should be

Studies of the Treacher-Collins syndrome have emphasized hearing and surgical considerations. Although craniofacial anomalies have been associated with respiratory disorders in infancy, the presence of such problems in older children has not been emphasized. An eight-year-old with Treacher-Collins syndrome presented a history of recent behavioral problems at home, poor attention span and performance in school, daytime somnolence, and sleep apnea with relatively long periods of chest movement but no airflow. He also had abnormal sleep behavior consisting of rocking to and fro on his hands and k

The coexistence of chronic obstructive pulmonary disease (COPD) and obstructive sleep apnea (OSA) in the same patients is defined as COPD/OSA overlap syndrome (OVL). OSA and sleep complaints are quite common among COPD patients and contribute to an increase in the risk of COPD exacerbation and mortality. Patients with OVL are more likely to develop cardiometabolic disease than patients with OSA or COPD alone. We must consider OSA as a treatable trait since the use of positive pressure ventilation reduces severe exacerbations, all-cause hospitalizations, and mortality in patients with COPD.
