Rostral overnight fluid shift in end-stage renal disease: relationship with obstructive sleep apnea.

BACKGROUND: In both healthy male subjects and men with heart failure, the severity of obstructive sleep apnea (OSA) is related to the amount of fluid displaced from their legs into the neck overnight. Whether overnight rostral fluid shift contributes to the pathogenesis of OSA in patients with end-stage renal disease (ESRD) is unknown. We hypothesized that the change in neck circumference (NC) and severity of OSA are related to the extent of overnight change in leg fluid volume (LFV) in patients with ESRD. METHODS: We studied 26 patients with ESRD (14 men) on conventional hemodialysis. All su

Nocturnal haemodialysis increases pharyngeal size in patients with sleep apnoea and end-stage renal disease.

BACKGROUND: Sleep apnoea is common in patients with end-stage renal disease (ESRD) and is improved by nocturnal haemodialysis (NHD). Recent findings from our laboratory indicate the development of ESRD is associated with pharyngeal narrowing. We hypothesized that NHD increases pharyngeal cross-sectional area and that this is associated with an improvement in sleep apnoea. METHODS: Twenty-four patients (aged 32-68 years), receiving conventional haemodialysis (CHD) (4 h/day, 3 days/week), were recruited for overnight polysomnography and estimation of pharyngeal cross-sectional area at functiona

Does etiology of end-stage renal disease affect sleep quality in kidney transplant recipients?

BACKGROUND: That hypertension (HTN) as a leading cause of end-stage renal disease (ESRD) is linked to sleep disorders in the general population can be the basis of a hypothesis that HTN may be a contributing factor to the poor quality of sleep in some kidney transplant recipients. This study was designed to investigate the correlation between ESRD secondary to HTN and sleep quality among kidney transplant recipients. METHODS: In this case control study, 201 kidney transplant recipients were divided into group I (ESRD) secondary to HTN, (n=82) and group II (ESRD secondary to other causes, n=11

Application of drug-induced sleep endoscopy in predicting the outcomes of velopharyngeal surgery in adult patients with Friedman stage II and III obstructive sleep apnea syndrome.

OBJECTIVE: This study aimed to evaluate the predictive value of drug-induced sleep endoscopy (DISE) for the outcomes of velopharyngeal surgery in adult patients with Friedman stage II and III obstructive sleep apnea syndrome (OSAS). METHODS: A total of 39 male OSAS patients with Friedman stage II and III were retrospectively analyzed. Subjects with an apnea-hypopnea index (AHI) > 5 events/h indicated by polysomnography (PSG) and typical symptoms, such as snoring, sleep apnea, and daytime sleepiness, were included in this study. All these patients underwent pre-operative DISE examinations and

Exploratory heart rate variability patterns across sleep stages following a 7-day multicomponent mantra-based cognitive intervention: A pilot randomized pre-post controlled study.

BACKGROUND: Cognitive-contemplative practices could potentially affect autonomic function, but existing research has focused on waking measures of heart rate variability (HRV). It remains unclear whether changes associated with brief interventions differ across sleep stages. OBJECTIVE: To determine whether differences in HRV according to sleep stage would differ in a group undergoing a 7-day, multicomponent, Hare Krishna mantra-based cognitive therapy (HMBCT) intervention compared to a group engaging in their normal routine activities. METHODS: Thirty-four healthy males aged 18-30 years were…

Actigraphy-derived circadian rhythms, sleep-wake patterns, and physical activity across clinical stages and pathophysiological subgroups in young people presenting for mental health care.

Staging models for youth mental health aim to locate clinical presentations on a spectrum from at-risk states to persistent disorder and predict future illness trajectories. Our previous publications on trans-diagnostic staging proposed three pathophysiological subgroups of major mood or psychotic disorders in youth ('hyperarousal-anxious depression', 'circadian-bipolar spectrum', 'neurodevelopmental-psychosis'). This study aims to investigate differences in objective measures of 24hr sleep-wake patterns, circadian rhythms, and physical activity across clinical stages and pathophysiological su

REM sleep without atonia and vestibular-evoked myogenic potentials: clinical brainstem dysfunction in early-stage Parkinson's disease and isolated REM sleep behavior disorder.

OBJECTIVE: To determine whether the onset of rapid eye movement (REM) sleep behavior disorder (RBD) is associated with changes in brainstem neuronal pathway dysfunction as reflected by vestibular-evoked myogenic potentials (VEMPs) and to evaluate associations between VEMPs and REM sleep without atonia (RSWA) in patients with early-stage Parkinson's disease (PD) and isolated RBD (iRBD). METHODS: Eighty-two early-stage PD patients, 40 iRBD patients, and 41 healthy control individuals underwent one-night video-polysomnography (vPSG) and VEMPs examination. We compared cervical (cVEMP), ocular (oV

Comprehensive Analysis of Machine Learning Models for Five-Class Sleep Stage Classification Using PPG Signals.

Accurate classification of sleep stages is essential for diagnosing sleep disorders and understanding sleep quality. While polysomnography (PSG) remains the gold standard, its invasiveness and cost hinder large-scale deployment. Photoplethysmography (PPG) offers a non-invasive and wearable alternative, yet most prior work either simplifies staging into fewer than five classes or relies on deep learning approaches with high computational demands. This study systematically evaluates traditional machine learning models for five-class sleep stage classification using PPG data from the MESA Sleep D…

Deep Learning for Pediatric Sleep Staging From Photoplethysmography: A Transfer Learning Approach From Adults to Children.

BACKGROUND: Sleep staging is critical for diagnosing sleep disorders. Traditional methods in clinical settings involve time-intensive scoring procedures. Recent advancements in data-driven algorithms using photoplethysmogram (PPG) time series have shown promise in automating sleep staging in adults. However, for children, algorithm development is hindered by the limited availability of datasets, with the Childhood Adenotonsillectomy Trial (CHAT) being the only substantial source, comprising recordings from children aged 5-10. This limitation constrains the evaluation of algorithmic generalizat

AnySleep: a channel-agnostic deep learning system for high-resolution sleep staging in multi-center cohorts.

Sleep is essential for good health throughout our lives, yet studying its dynamics requires manual sleep staging, a labor-intensive step in sleep research and clinical care. Across centers, polysomnography (PSG) recordings are traditionally scored in 30-s epochs for pragmatic, not physiological, reasons and can vary considerably in electrode count, montage, and subject characteristics. These constraints present challenges in conducting harmonized multi-center sleep studies and discovering novel, robust biomarkers on shorter timescales. Here, we present AnySleep, a deep neural network model tha

Plasma Neuron-Specific Enolase is not a reliable biomarker for staging Trypanosoma brucei rhodesiense sleeping sickness patients.

OBJECTIVE: Currently, the only available staging criterion for T. b. rhodesiense requires a lumber puncture to collect and later examine cerebrospinal fluid (CSF). This study examined the potential of plasma Neuron-Specific Enolase (NSE) in discriminating between early and late-stage patients. RESULTS: When median NSE levels were compared between early and late-stage patients, results showed a significant (P < 0.02) upregulation among late-stage patients (599.8 ng/mL). No significant differences (P > 0.9) in NSE levels were observed between early-stage patients (300 ng/mL) and controls (454 n
