Hyposplenic patients are at risk of overwhelming post-splenectomy infection (OPSI), which carries mortality of up to 70%. Therefore, preventive measures are warranted. PURPOSE OF THE STUDY: To evaluate the effectiveness of testicular scintigraphy in differentiating testicular torsion from acute epididymitis.

A review of hospital admissions during 80 months revealed only 8 patients with episodes of priapism of approximately 400 pediatric male patients with sickle cell disease. The patients, who ranged in age from 5 to 19 years, underwent a 99mtechnetium penile scan, and 4 had a low and 4 had a high flow scan. Three cases resolved with hydration alone. Five patients received exchange transfusion of whom 3 subsequently underwent shunt procedures. One patient with a 5-year history of recurrent stuttering episodes was placed on transfusion therapy for 6 months and stuttering episodes have not recurred.

From 1980 to 1992, we found false negative results from Technetium (Tc)-99m abdominal scans in seven children with massive gastrointestinal bleeding from ectopic gastric mucosae. Other examinations--including endoscopy, gastrointestinal series studies, angiography, and Tc-99m-labeled red blood cell scans to search for the source of the bleeding--were all in vain. A Meckel's scan was performed on a 7-yr-old boy because of rectal bleeding. He was premedicated with pentagastrin (6 micrograms/kg subcutaneously) followed by 259 MBq (7 mCi) [99mTc]pertechnetate intravenously. The study was essentially negative. Because of persistent rectal bleeding and a strong clinical suspicion of a Meckel's, a repeat study was performed 6 wk later. He received pentagastrin as before, but this time was also given a histamine H2 receptor blocker (zantac). A Meckel's diverticulum was clearly evident.

Acute lower gastrointestinal hemorrhage in a 17-year-old boy was caused by a Meckel's diverticulum 1 day after ingestion of 500 mg acetylsalicylic acid. After conservative treatment of the bleeding, the diverticulum was diagnosed in the free interval by technetium pertechnetate scintigraphy, which showed an accumulation in the right lower quadrant simultaneously with the accumulation in the gastric mucosa. Measurements were made of the activity in samples of breast milk obtained from a patient with postpartum thyroiditis following administration of [123I]sodium iodide and subsequently [99mTc]pertechnetate 24 hr later. Both 123I and 99mTc were found to be excreted exponentially with an effective half-life of 5.8 hr and 2.8 hr, respectively. Less than 10% of the activity was incorporated into breast-milk protein. After administration of [123I]sodium iodide breast feeding should be discontinued for 24-36 hr to reduce the absorbed dose to the child's thyroid.
