The camptocormia angle has been established as a strong indicator for evaluating the progress of Parkinson's disease and the efficacy of therapeutical approaches. A wearable setup is proposed to measure the camptocormia angle with the perpendicular method using five inertial sensors. This study identifies suitable inertial measurement unit sensors for mobile long-term measurement. Moreover, a machine-learning approach is presented for segmenting the recorded data into periods with different dominant activities. An artificial neural network was the better classifier compared to a support vector

Lateral trunk flexion is often seen in patients with Parkinson disease, sometimes coming on as a subacute phenomenon associated with medication adjustments, and in others with gradual onset that seems related to a neurodegenerative process related to the evolution of the disease.Either acute or subacute presentations seem to be pure abnormalities in the coronal plane and are usually reversible. However, a chronic form occurs often in a combined fashion with anteroposterior flexion (camptocormia), improves only partially, remains stable, or even worsens over time. The acute/subacute phenotype is

Camptocormia or 'bent spine syndrome' is a rare manifestation of Parkinson's disease. The postural deformity can be a great source of disability. Camptocormia is typically not responsive to dopaminergic medication. Results with deep brain stimulation to treat camptocormia have been mixed but generally poor. The authors report two cases of camptocormia in Parkinson's disease treated with spinal corrective surgery. Despite prolonged postoperative courses, including a high complication rate and the need for multiple revisions, both patients benefited from the procedures.

Adult spinal deformity(ASD) is a condition in which the spinopelvic alignment changes owing to age-related degeneration, making it difficult to maintain a standing position. The goal of surgery for ASD is to correct the spine and obtain normal alignment. Here, we discuss the pathophysiology of ASD, spinopelvic alignment, surgical methods, and complications. Circumferential minimally invasive surgery for ASD comprises an anterior approach using lateral lumbar interbody fusion and a posterior approach using percutaneous pedicle screws. Complications include perioperative and implant-related comp

BACKGROUND: Spinal cord stimulation (SCS) has recently been reported to be effective for truncal postural abnormalities such as camptocormia and Pisa syndrome in Parkinson's disease. In this case report, we describe a case of a woman with Parkinson's disease in whom SCS was effective for painful camptocormia with Pisa syndrome.
CASE PRESENTATION: A 65-year-old woman was admitted to our hospital because of painful camptocormia. She had noticed resting tremor in the left upper limb and aprosody at 48 years of age. She was diagnosed as having Parkinson's disease (Hoehn & Yahr stage 1) at 53 year

BACKGROUND: The geometric alignment of the spine plays an integral role in stability, biomechanical loading, and consequently, pain, and a range of healthy sagittal curvatures has been identified. Spinal biomechanics when sagittal curvature is outside the optimal range remains a debate and may provide insight into the load distribution throughout the spinal column.
METHOD: A thoracolumbar spine model (Healthy) was developed. Thoracic and lumbar curvatures were adjusted by 50% to create models with varying sagittal profiles: hypolordotic (HypoL), hyperlordotic (HyperL), hypokyphotic (HypoK), a

Adolescent idiopathic scoliosis (AIS) affects 3% to 4% of children between the ages of 11 and 18 [1, 2]. This disorder, characterized by abnormal three-dimensional spinal curvatures that typically develop during periods of rapid growth, occurs in the absence of congenital vertebral malformations or neuromuscular defects [1]. Genetic heterogeneity [3] and a historical lack of appropriate animal models [4] have confounded basic understanding of AIS biology; thus, treatment options remain limited [5, 6]. Recently, genetic studies using zebrafish have linked idiopathic-like scoliosis to irregulari…

The ability to adopt the properly corrected body posture is one of the factors determining the effectiveness of therapeutic programmes. This study determined the active self-correction expressed by the change of sagittal spinal curvatures (in standing and sitting positions) in 249 children (136 females, 113 males, aged 10-14 years) instructed with 'straighten your back' command (SYB). Spinal curvatures (sacral slope-SS, lumbar lordosis-LL, global, lower and upper thoracic kyphosis-TK, LK, UK, respectively) were assessed using Saunders inclinometer. The assessment was done in spontaneous standi…

BACKGROUND: Small and relatively balanced anteroposterior curvatures and symmetric positioning of the shoulders, scapulae, waist triangles, anterior superior iliac spines, knees and feet are the elements of a good posture. The aim of our study was to determine which somatic features and parameters of spinal curvatures in the sagittal plane show statistically significant differences among children with particular types of body posture.
MATERIAL AND METHODS: The study was carried out in selected primary schools of the Subcarpathian and Silesian regions. We examined a total of 563 randomly selec…

Using a special software program we measured spinopelvic parameters on digitised radiographs of the entire spine and the pelvis of 50 patients with lumbar disc herniation and compared these with the same measurements on 30 healthy subjects. In the disc hernia group the patients had a relatively straight spine in the sagittal plane. The sacrum was more vertical, and the value of the lumbar lordosis was lower, as was the amplitude of the spinal curvatures, when compared with those of the healthy group. This results in a higher gravitational compressive force which may, in turn, lead to progressi…

STUDY DESIGN: Multicenter database review of consecutive adult spinal deformity (ASD) patients.
OBJECTIVE: The aim of this study was to identify associations between changes in spinopelvic parameters and cervical alignment after thoracolumbar arthrodesis for ASD.
SUMMARY OF BACKGROUND DATA: Reciprocal cervical changes occur after instrumented thoracic spinal arthrodesis. The timing and relationship of these changes to sagittal alignment and upper instrumented vertebra (UIV) selection are unknown.
METHODS: In 171 ASD patients treated with thoracolumbar arthrodesis from 2008 to 2012, we asses

PURPOSE: To investigate the different cervical strategies for maintaining horizontal gaze in asymptomatic subjects.
METHODS: One hundred and forty-four asymptomatic adults filled the SF-36 quality of life questionnaire and underwent full-body biplanar radiographs. Chin brow vertical angle (CBVA) and postural and cervical parameters were measured. Subjects were grouped according to cervical spine curvature (C2-C7 angle): kyphotic (< - 5°), straight [- 5°, 5°], lordotic (> 5°). Demographics, SF-36 component scores and CBVA were compared between groups. All other parameters were compared between

Spinal deformities can result in increasing thoracic kyphosis or loss of lumbar lordosis, leading to imbalance in the sagittal plane. Such deformities can be functionally and psychologically debilitating. The Smith-Petersen osteotomy can achieve approximately 10 degrees of correction in the sagittal plane at each spinal level at which it is performed. This osteotomy is beneficial for patients who have a degenerative imbalance in the sagittal plane. The pedicle subtraction osteotomy can achieve approximately 30 degrees to 40 degrees of correction in the sagittal plane at each spinal level at wh

STUDY DESIGN: Author experience and literature review.
OBJECTIVES: To investigate and discuss decision-making on when to perform a Smith-Petersen osteotomy as opposed to a pedicle subtraction procedure and/or a vertebral column resection.
SUMMARY OF BACKGROUND DATA: Articles have been published regarding Smith-Petersen osteotomies, pedicle subtraction procedures, and vertebral column resections. Expectations and complications have been reviewed. However, decision-making regarding which of the 3 procedures is most useful for a particular spinal deformity case is not clearly investigated.

During spinal fusion procedures, spinal rods are bent to match the current and desired curvature of a spinal segment. Following bending, the rods are attached to the heads of pedicle screws that have been driven into the vertebral pedicles. A mismatch between the curvature of the spine and the curvature of the rod can lead to increased loads at the screw-bone interfaces. These increased loads have been associated with screw loosening and pull-out. We have developed a novel and inexpensive spinal fusion simulation device to explore how variables such as surgical techniques, tools, and hardware

Hyperkyphosis is a spinal deformity characterized by an excessive posterior curvature of the thoracic spine, most commonly developing during adolescence. If untreated, it may cause postural imbalance, pain, reduced spinal mobility, and functional limitations. Conservative treatment is essential during growth. The Schroth method uses three-dimensional exercises and breathing techniques to improve spinal alignment and postural control, while the Schroth brace supports corrective positioning and enhances therapy. This case study evaluates the effects of a one-year Schroth treatment combined with

PURPOSE: To improve the setup reproducibility of neck curvature using real-time optical surface imaging (OSI) guidance on 2 regions of interest (ROIs) to infer cervical spine (c-spine) curvature for surface-guided radiotherapy (SGRT) of head-and-neck (HN) and c-spine cancer.
METHODS: A novel SGRT setup approach was designed to reproduce neck curvature with 2 ROIs: upper-chest ROI and open-face ROI. It was hypothesized that the neck curvature could be reproduced if both ROIs were aligned within ±3 mm/2˚ tolerance. This was tested prospectively in 7 volunteers using real-time 3D-OSI guidance an
