Traumatic mesenteric arterioportal fistulas (APF) are rare but serious and life-threatening. This article reports the case of a 24-year-old female who suffered a gunshot wound to the abdomen and subsequently developed a superior mesenteric artery (SMA) pseudoaneurysm and APF, which caused secondary right heart failure and pulmonary hypertension. The patient underwent a successful endovascular stent-graft exclusion of the pseudoaneurysm/APF. The patient was asymptomatic 18 months post procedure and leading an active life with a normal arterial flow in the SMA confirmed by duplex ultrasonography

A case of large spontaneous splenorenal (S-R) shunt accompanied with splenic artery aneurysm is reported in this paper. The large blood vessel, a shunt, anatomizing the splenic vein and left renal vein is macroscopically found during the autopsy of a 71-year-old male, accompanied with liver cirrhosis and splenomegaly. Though there are many reports demonstrating medical imaging, anatomical findings are relatively poor besides describing and illustrating the appearances of the S-R shunt, the splenic artery aneurysm and related branches of the splenic vein and left renal vein, the embryologic and…

A blocked distal splenorenal shunt presents with torrential bleeding from gastric varices. Reoperation in the presence of portal hypertension in an unstable patient is difficult. Two patients with a blocked distal splenorenal shunt were subjected to splenic artery embolization. This stopped variceal bleeding from gastric varices resulting from the congested spleen due to thrombosis of the splenic vein. The procedure was successful in stopping the acute bleed in both patients. There were no serious complications or recurrent variceal bleeding and the varices were collapsed at one year on follow…

We report a case of splenic artery thrombosis developed in a 54-year old woman after prolonged use of oral contraceptives. The diagnosis of the disease, difficult on clinical grounds, was confirmed by computerized tomography and selective arteriography. The presence of anti-ethinyloestradiol antibodies in the serum is suspected to be a risk factor for thrombosis associated with oral contraception.

Splenic artery embolization with steel coils was performed in two patients who both had large splenic artery aneurysms and hepatic cirrhosis complicated by hypersplenism. A good clinical effect was noticed after the procedure. It was concluded that this treatment is safe and effective and decreases the risk of splenic artery rupture. It also corrects hypersplenism. Transcatheter embolization appears to be a preferable alternative to surgery in such cases.

High risk splenectomy is often encountered in cases of hypersplenism with massive splenomegaly (10 times usual weight of 150-200 g) resulting from myelophthisic processes. Intra-operative ligation of the splenic artery through the lesser sac is a technically useful method of gaining vascular control prior to mobilizing the challenging spleen. However, a massive or inaccessible spleen imposes mechanical limitations during surgery and may be complicated by torrential intra-operative hemorrhage in the setting of severe thrombocytopenia refractile to platelet transfusions. The authors describe pre…

Five patients in the blastic phase of chronic granulocytic leukemia with massive splenomegaly were treated by intraarterial splenic artery infusion of cytosine arabinoside. All patients had massive splenomegaly associated with pain and/or hypersplenism and were refractory to previous therapy. All 5 patients demonstrated responses to treatment, with reduction in spleen size as well as symptomatic relief. Systemic toxicity was minimal in 4 of the 5 patients.

Vascular Ehlers-Danlos Syndrome (VEDS) is a rare genetic disorder characterized by arterial fragility and a high risk of spontaneous vascular rupture. Splenic artery rupture is an uncommon but life-threatening manifestation of this condition. We report the case of a 34-year-old male with VEDS who presented with sudden abdominal pain and rapid progression to hemorrhagic shock. Imaging confirmed active bleeding from the distal splenic artery. After multidisciplinary evaluation, endovascular embolization via a left humeral approach was performed using multiple coils. The procedure achieved hemost…

BACKGROUND: The spleen is commonly injured in trauma and this may be managed with a conservative approach, embolisation or splenectomy. There is uncertainty how splenic embolisation fits into the treatment paradigm and the delivery of IR services remains variable.
AIMS AND OBJECTIVES: The primary objectives are to determine if service design significantly affects splenic embolisation (SE) rates in AAST grade 2-5 acute traumatic splenic injuries (ATSI) across the Major Trauma Centres (MTCs) in England and to determine if variation in treatment affects SE outcomes in ATSI.
METHODS: We will inc

The authors report a case of a 74-year-old woman found to have an extremely rare case highlighted by multidetector computed tomography (MDCT) angiography, with the presence of a replaced right hepatic artery (RRHA) arising from the splenic artery (SA). In this case, the SA arose from a gastrosplenic trunk (GST). The GST had an endoluminal diameter of 9.2 mm at its origin and a length of 9.3 mm. It arose directly from the anterior abdominal aortic wall, at the level of the T12-L1 intervertebral disc. The SA branched off from the GST and travelled in front of the abdominal aorta (AA) for 18.2 mm…

Hepatic artery aneurysms are responsible for 12% to 20% of all visceral arterial aneurysms. Because most patients are asymptomatic, this dis … hepatic arteries, followed by an end-to-end anastomosis formed between the right hepatic artery and splenic artery using a splenic artery transposition graft. Postoperative follow-up examinations showed that both hepatic arterial circulations were good, and no splenic infraction had developed.

AIM: The natural history and optimal management of splenic artery aneurysms (SAAs) remain uncertain. We combined a systematic literature review (2000-2025) with a local single-centre retrospective cohort to evaluate the natural history, predictors of growth, and rupture risk of incidental splenic artery aneurysms.
MATERIALS AND METHODS: A systematic review of 5 databases (2000-2025) identified longitudinal studies of true SAAs and were quality assessed. A parallel local retrospective cohort of untreated SAA diagnosed on computed tomography (CT) (2008-2023) was analysed. Demographic, imaging, …

Splenic artery aneurysm (SAA) is an uncommon and life-threatening vascular disease. This pathology is frequently diagnosed during pregnancy. The major complication of this aneurysm is rupture, which happen frequently in pregnant women due to physiological changes. We report, two rare cases of sudden death due to SAA rupture occurring during the first two trimesters of pregnancy and discovered during autopsy. Case 1: A 26-weeks pregnant woman consulted the Emergency for a sudden onset of abdominal pain and dizziness. An ultrasound showed fetal death. She underwent two surgeries, and the origin …

Hemosuccus pancreaticus (HP) is a rare but potentially life-threatening condition, characterized by upper gastrointestinal bleeding from the ampulla of Vater, often originating from a ruptured pseudoaneurysm of the peripancreatic arteries. Despite its rarity, HP presents a diagnostic and therapeutic challenge due to its elusive clinical presentation and complex underlying pathophysiology. In this case report, we presented a compelling instance of HP, diagnosed in a 48-year-old man, complicated with gastrointestinal bleeding and severe anemia successfully managed with urgent endovascular interv
