Spontaneous Splenic Ruptures: A Contemporary Systematic Review.

INTRODUCTION: Spontaneous splenic rupture (SSR) is a rare, life-threatening condition. The current study aims to provide a comprehensive recent overview of SSR. METHODS: A comprehensive search was performed in PubMed (MEDLINE), Google Scholar, and Scopus to identify relevant case reports and case series published during the last decade (January 1, 2015-July 1, 2025). RESULTS: We identified 351 eligible studies with 400 cases of SSR. The two most common etiologies for SSR were infectious (25.6%) and neoplastic (22.9%) conditions. Drug- or treatment-related etiologies were reported in 19% of c…

It must be mono: An atypical case of splenic rupture.

Splenic rupture is a rare but life-threatening complication of infectious mononucleosis, typically associated with Epstein-Barr virus. Early diagnosis of infectious mononucleosis is essential for appropriate management but may be delayed by false-negative early testing and atypical presentations. We report an atypical case of infectious mononucleosis in a 17-year-old male who presented with an acute abdomen secondary to atraumatic splenic rupture confirmed on CT. Initial mononucleosis testing was negative, delaying diagnosis, but repeat testing and subsequent laboratory findings strongly suppo…

Spontaneous Splenic Rupture Leading to the Diagnosis of CML.

BACKGROUND: Spontaneous splenic rupture (SSR) is a known complication of hematologic malignancy. Rare cases have been reported in which patients presented with SSR prior to diagnosis of chronic myeloid leukemia (CML). We present a case of atraumatic SSR due to CML presenting as persistent abdominal pain. CASE PRESENTATION: A 42-year-old man presented with persistent left upper quadrant pain radiating to the lower quadrants ongoing for many months. He additionally had many other systemic symptoms including weight loss, night sweats, dark tarry stools, and progressive fatigue. Initial laborator…

A case report of splenic rupture related to colonoscopy.

Splenic rupture is a rare but serious iatrogenic complication of colonoscopy, potentially leading to hemodynamic instability. When common complications such as post-polypectomy syndrome and perforation are excluded, physicians must maintain a high level of suspicion for splenic injury, particularly in patients presenting with abdominal pain after the procedure. This report describes an emergency splenectomy performed on an 80-year-old male following a routine colonoscopy. Seven hours of post-procedure, the patient presented abdominal pain, syncope, sweating, and hypotension. Computed tomograph…

Spontaneous splenic rupture as a complication of dengue fever.

Dengue is a mosquito-transmitted virus and the leading cause of arthropod-borne viral disease in the world. Clinical manifestations can vary from asymptomatic disease to severe illness and death. Dengue is also known to cause myocarditis, hepatitis, and neurological manifestations, and rarely to cause splenic rupture. We report a 34-year-old man with splenic rupture complicating dengue fever which was managed conservatively with excellent outcome.

Ruptured splenic abscess: A rare case entity.

Vancomycin-resistant enterococci (VRE) are a serious public health threat and a leading cause of healthcare-associated infections. Bacterial resistance to antibiotics recommended for the treatment of enterococcal infections complicates the management of these infections. Hence, there is a critical need for the discovery of new anti-VRE agents. We previously reported carbonic anhydrase inhibitors (CAIs) as new potent VRE inhibitors. In the present study, the activity of the CAI, dorzolamide was evaluated against VRE both in vitro and in vivo. Dorzolamide exhibited potent activity against a pane

A case of splenic rupture a week after appendectomy.

A 52-year-old woman developed atraumatic splenic rupture 1 week after appendectomy for perforated appendicitis. The emergency computed tomography (CT) revealed abscessed appendicitis. We performed a laparoscopic appendectomy and meticulous peritoneal lavage of the right lower quadrant peritonitis. Intravenous antibiotics were prolonged after surgery. Six days after appendectomy, she presented acute signs of hypotensive shock associated with abdominal pain and blood in the pelvic drain. Emergency CT scan revealed splenic rupture with major hemoperitoneum and active splenic bleeding. Embolizatio…

Spontaneous Splenic Rupture in a Case of Infectious Mononucleosis.

Infectious Mononucleosis is a common viral illness mainly of adolescent-young adult age group. Spontaneous splenic rupture is a rare but potentially fatal complication of Infectious Mononucleosis occurring in less than 0.5 % of the cases. A high index of suspicion especially if abdominal pain develops during Infectious Mononucleosis is very crucial to early diagnosis and intervention in the case of rupture. Here we discuss the case of a 24 year old male with no previous comorbidities and stable vitals who presented with febrile illness of one week duration associated with abdominal discomfort.…

Splenic rupture in a patient with pelvic abscess and sepsis.

A 49-year-old man underwent appendicectomy through a Lanz incision for necrotic appendicitis. He subsequently developed pelvic abscess in the postoperative period, which was drained under computed tomography (CT) guidance. The bacteriology of pus swabs taken during appendicectomy and abscess drainage revealed coliforms. Six days after appendicectomy, the patient had an acute collapse due to rupture of the spleen, which was treated by splenectomy. CT of the abdomen at the time of abscess drainage had confirmed a normal spleen. Histopathological examination of the spleen revealed nonspecific acu

[The rupture of splenic artery aneurysm in Erdheim disease].

Splenic vessel aneurysm is often a casual pathology during strumental imaging or laparotomy. The first cause for the formation of the aneurysm is a vessel primitive degenerative pathology of the medium wall, or idiopathic cystic medionecrosis, that to develop in aneurysmatic dissection. In the early diagnosis, in the selective patients, resulted indicate an endovascular therapy, while in emergency the laparotomy is a gold strandard for reduction morbility and mortality. In this paper, the Authors described a rare case of rupture of the lienal vessel aneurysm in Erdheim disease, and reported cl

Pathologic splenic rupture in a patient with follicular lymphoma.

A middle aged man presented with abdominal pain and fever, with progressive dyspnea for the past one week. He had generalized lymphadenopathy with hepatosplenomegaly and a left sided pleural effusion on admission. Further evaluation revealed that he had lymphocytosis on peripheral blood. He then developed increasing abdominal pain and fall in hemoglobin which was confirmed on imaging to be due to a splenic rupture and he underwent a splenectomy. The diagnosis on lymph node biopsy and peripheral blood immunophenotyping was grade 1 follicular lymphoma. He has completed his 6 cycles of chemothera…

[Spontaneous splenic rupture in the course of infectious mononucleosis].

Spontaneous splenic rupture (SSR) in the course of infectious mononucleosis (IM) is a rare but potentially fatal complication. Mortality rate is relatively high, therefore emergency splenectomy is a life-saving intervention. In case of undergoing urgent operation there is no possibility to initiate proper prophylaxis of overwhelming infection. The humoral and cellular immunologic response impairment is a reason for life-threatening complications of splenectomised person. Asplenic children should receive infection prophylaxis immediately post splenectomy. We report two cases of splenic rupture …
