CASE REPORTS: We report the cases of four males from four different families, who presented paroxysmal episodes from the 1st 2nd year. These episodes were characterised by asymmetrical bilateral dystonia of the upper limbs, predominantly in both hands, and were associated with orofacial dyskinesias, stereotipies (jumping, arm flapping, etc.), facial tics and, occasionally, phonic tics. Consciousness is not affected in any of the cases. These movements are triggered in situations where the patient is relaxed or excited. They occur daily and last from a few seconds to 30 minutes. Rett syndrome (RTT) is a neurodevelopmental disorder characterized by normal early psychomotor development followed by the loss of psychomotor and acquired purposeful hand skills and the onset of stereotyped movement of the hands and gait disturbance. The causative gene was discovered, and the disease was found to be caused by a mutation of methyl-CpG-binding protein 2. However, in many ways this clinically peculiar condition remains a mystery. I review the current status of clinical and basic research on RTT including data on the neurophysiology of the disease, neurotransmitter involvement, n

Autism is a complex neurodevelopmental disorder characterized by impairment in social interaction accompanied by a delay or lack of language, restricted interests, stereotyped behavior, and repetitive movement. Genetic predisposition to autism is evident from family and twin studies, and heritability in idiopathic autism is estimated at over 90%. Frequency of the disorder is approximately 1:2000 with a male to female ratio of 4:1. Affected individuals look normal at birth, and the symptoms manifest at the first 2-3 years of life. The spectrum of clinical symptoms and the severity of the disord

Recently, submicroscopic deletions of the 5q14.3 region have been described in patients with severe mental retardation (MR), stereotypic movements, epilepsy and cerebral malformations. Further delineation of a critical region of overlap in these patients pointed to MEF2C as the responsible gene. This finding was further reinforced by the identification of a nonsense mutation in a patient with a similar phenotype. In brain, MEF2C is essential for early neurogenesis, neuronal migration and differentiation. Here we present two additional patients with severe MR, autism spectrum disorder and epile

The male to female ratio is raised in autism spectrum disorders (ASD). Previous studies have suggested that girls with ASD have more problems with communication than boys, but boys show more repetitive behaviours than girls. In this study, 20 girls, 1.8-3.9 years of age were matched for chronological and developmental age with 20 boys with suspected ASD. All the children were recruited after population screening and referral by Child Health Care Services to a specialised neuropsychiatry clinic, where they underwent comprehensive neuropsychiatric assessments. Comparisons were made with regard t

Acral lick dermatitis is a common, frustrating disease. It is characterized by incessant licking behavior resulting in raised, thickened, ulcerative plaques, most commonly affecting the lower extremities of dogs. Underlying primary diseases, such as allergic disorders (atopic dermatitis, food allergy, flea allergy), orthopedic or neurologic disorders, parasitic diseases (eg, demodicosis), infections (fungal, bacterial), neoplasia or psychogenic disorders (compulsive/obsessive-compulsive disorder), and perpetuating factors (especially secondary infections) drive the itch-lick cycle. A 65-year-old woman who habitually made flexion-extension movements with her cervical vertebral column (so-called 'headbanging') was seen with complaints of the neck. X-rays of the neck revealed an odontoid fracture. Bone densitometry revealed lowered bone mineral density (osteopenia). The diagnosis was odontoid fracture due to headbanging, in the presence of osteopenia. Patient recovered after treatment with a neck collar worn for 6 months.

Access to a handicapped child with Rett syndrome may be attempted through its eyes and by means of music. The receptivity for rhythm and melodies must be located within a primitive region of the hierarchical structure of the brain because even severely retarded children do respond to music. If purposefully applied with several instruments music therapy can bring about at least a temporary cessation of the stereotypic movements in children with Rett syndrome. Many years of experience attest to the feasibility of this approach.

Bobble-head doll syndrome (BHDS) presents in childhood and is usually associated with lesions of the third ventricle. This disorder is characterized by stereotypical head movements of the type "yes-yes" (up and down) at a frequency of 2 to 3 Hz. Rarely, movements of the type "no-no" (side-to-side) are described. There are a few hypotheses to explain the mechanism responsible for BHDS, but its real pathophysiological characteristics are still unknown. The authors describe the case of a child born with hydrocephalus and Dandy-Walker syndrome. A ventriculoperitoneal shunt was implanted in the chi

A four-stage construction is presented for illustrating the characteristic clinical pattern and profile over the years in the average, 'classical' rett syndrome (RS) patient. The staging system was applied and evaluated in the 29 Swedish RS cases who now have passed age 13 (median 18). We also discuss the diagnostic pitfalls we have met and the conditions that need to be considered in the differential diagnosis.

Previous occupational health studies of diamond polishers have identified high rates of musculoskeletal disorders of the hands and arms in this population, specifically, ulnar neuropathy in the right elbow. These disorders may be due to the repetitive and forceful motions of the upper extremity required to manipulate the hand-held polishing tools. A video-based, biomechanical analysis of the upper extremities of polishers at several diamond-processing factories in Israel was conducted. Detailed motion measurements of the wrists, elbows, and shoulders of polishers were made while the subjects p

Rett' syndrome is a progressive disorder that occurs in females and is characterized by autistic behavior, dementia, ataxia, loss of purposeful use of the hands, and seizures. Patients with Rett' syndrome have been observed to have stereotyped hand movements (hand-washing) and to exhibit intermittent hyperventilation. To characterize more precisely the respiratory pattern associated with this disorder, SAO2 and pH studies were made during sleep and wakefulness in a girl with this syndrome. These studies showed abnormal respiratory pattern during wakefulness characterized by hyperventilation pe

Neuronal ceroid lipofuscinosis (CLN) 7 typically presents with motor and cognitive decline, seizures (myoclonus) and vision loss. Atypical manifestations such as, ataxia, Rett-like findings, microcephaly, personality disorders, extrapyramidal symptoms, stereotypical hand movements and autistic behaviors had been reported. A 7-year-old male patient referred with the diagnosis of sepsis and a medical history of afebrile seizure at the age of 3 years, and sleep problems and aggressive behavior at the age of 4 years. Dance-like movements were noted in his arms and legs. Laboratory tests identified

The purpose of this qualitative study was to explore the lived experience of how sensory-related behaviors of children with autism affected family routines. In-depth semi-structured interviews were conducted with four primary caregivers regarding the meaning and impact of their child's sensory-related behaviors on family routines that occurred inside and outside the home. Findings indicated that sensory behaviors are one factor that limited family participation in work, family and leisure activities; and that parents employed specific strategies to manage individual and family routines in ligh

Opioid receptor densities were measured in the hippocampus of chronically stressed (tethered) pigs to study the involvement of endogenous opioid systems in stereotypy performance. Three groups of animals were housed tethered for 2 (n=12), 5.5 (n=12) and 8-9 months (n=8), respectively, and the intensity of stereotypy performance was determined. Opioid receptor densities were measured post mortem using membrane binding assays with [(3)H]naloxone as a ligand. A negative correlation was found between the density of opioid receptors and the intensity of stereotypy performance in the animals that ha

We evaluated the effects of 3 types of noncontingent auditory stimulation (music, white noise, recordings of vocal stereotypy) on 2 children with autism who engaged in high rates of vocal stereotypy. For both participants, the music condition was the most effective in decreasing vocal stereotypy to near-zero levels, resulted in the highest parent social validity ratings, and was selected as most preferred in treatment preference evaluations.

We developed an iOS app, the iSTIM, designed to support parents of children with autism spectrum disorders (ASD) in reducing common repetitive vocal and motor behavior (i.e., stereotypy). The purpose of our study was to preliminarily test the decision-making algorithms of the iSTIM using trained university students to implement the assessments and interventions. Specifically, we examined the effects of the iSTIM on stereotypy and functional engagement in 11 children with ASD within alternating treatment designs. Using the iSTIM reduced engagement in stereotypy for eight participants and increa

Motor stereotypy behaviors are patterned, coordinated, repetitive behaviors that are particularly evident in those with an autistic spectrum disorder and intellectual disabilities. The extent to which motor stereotypy behavior severity is associated with motor skills and maladaptive behavior, measures of adaptive functioning, along with fundamental movement skills and degree of autistic spectrum disorder symptomology is assessed in this preliminary report. Twelve participants, aged 7 to 16 years, with a reported motor stereotypy behavior and either mild or severe intellectual disability compri
