Nine children with severe pectus excavatum deformity were operated on in the Department of Pediatric Surgery, Thoracic Unit, Ege University Faculty of Medicine, Izmir, Turkey. There were four boys and five girls, and the mean age at operation was 6.3 years. A modified Ravitch technique was performed by using two polytetrafluoroethylene bars placed retrosternally for fixation and stabilization of the reconstituted anterior chest wall. The bars were removed 6 months after the operation. Results were satisfactory in all cases, with no surgical complications.

We report on a case of thymic cyst which was successfully treated with mediastinoscopic resection combined with a sternum lift technique. A 62-year-old man was admitted to our hospital with a thymic cyst, which was causing chest discomfort and was increasing in size. Sternal fractures occurred in 78 out of 2097 consecutive victims of road traffic accidents who required to attend hospital; an incidence of 3.7 per cent. They were most common in restrained, front seat vehicle occupants involved in frontal collisions, and their frequency increased with age. There was a positive association with visceral chest injury but the latter was not thought to be the cause of death in any individual. Follow-up of survivors 2-4 years later revealed potential cardiac sequelae in 6 per cent.

Eight children (age 4 months - 9 years) were studied after surgical correction of congenital heart defects. Sternum closure reduced mean arterial pressure by 7% (p less than 0.05) and cardiac output by 14% (p less than 0.01), and increased central venous pressure by 2 mmHg (p less than 0.05). There was evidence of a decrease in intracardiac blood volume. It is suggested that the circulatory effects of sternum closure were due to increased pressure outside the heart, which reduced the pressure across the heart wall and the end-diastolic volume.
