Determinants of stillbirth among reviewed perinatal deaths in Ethiopia.

BACKGROUND: The global burden of stillbirth has declined over time. However, the problem is still prominent in South Asian and Sub-Saharan African countries. Ethiopia is one of the top stillbirth-reporting countries worldwide. Despite several measures taken to reduce the burden of stillbirth; the pace of decline was not as good as the post-neonatal death. Thus, this study is aimed at identifying potential factors related to stillbirth in Ethiopia based on nationally reviewed perinatal deaths. METHOD: The national perinatal death surveillance data were used for this study. A total of 3,814 rev…

Candida chorioamnionitis associated with late stillbirth: A case report.

Vulvovaginal candidiasis during pregnancy is common, but serious complications, including chorioamnionitis, are infrequent. A 41-year-old woman presented at 37 weeks of gestation with reduced fetal movements, and fetal death in utero was subsequently confirmed on ultrasound. Histopathology of the cord and placenta revealed Candida infection and microabscesses on the umbilical cord. Overall, these features are suggestive of ascending infection, consistent with Candida as the causative organism. To the best of our knowledge, this is the first reported case of late stillbirth due to Candida chori…

Maternal Health after Stillbirth: Postpartum Hospital Readmission in California.

OBJECTIVE: The aim of this study is to assess whether the risk of postpartum readmission within 6 weeks of giving birth differs for women who had stillbirths compared with live births. STUDY DESIGN: Using data from the Office of Statewide Health Planning and Development in California, we performed a population-based cohort study of 7,398,640 births between 1999 and 2011. We identified diagnoses and procedures associated with the first postpartum hospital readmission that occurred within 6 weeks after giving birth. We used log-binomial models to estimate relative risk (RR) of postpartum readmi…

Maternal Risk Factors for Stillbirth: A Registry-Based Study.

Background and Objectives: The number of stillbirths has reduced more slowly than has maternal mortality or mortality in children younger than 5 years, which were explicitly targeted in the Millennium Development Goals. Placental pathologies and infection associated with preterm birth are linked to a substantial proportion of stillbirths. Appropriate preconception care and quality antenatal care that is accessible to all women has the potential to reduce stillbirth rates. The aim of the present study was to assess potential risk factors associated with stillbirth within maternal medical diseas…

Prediction of stillbirth from maternal demographic and pregnancy characteristics.

OBJECTIVES: To develop a model for prediction of stillbirth based on maternal characteristics and components of medical history and to evaluate the performance of screening with this model for all stillbirths and those due to impaired placentation and to unexplained causes. METHODS: This was a prospective screening study of 113 415 singleton pregnancies at 11 + 0 to 13 + 6 weeks' gestation and at 19 + 0 to 24 + 6 weeks. The study population included 113 019 live births and 396 (0.35%) antepartum stillbirths; 230 (58%) were secondary to impaired placentation and 166 (42%) were due to other or …

Thyroid function testing in women who had a stillbirth.

CONTEXT: Thyroid dysfunction is thought to be associated with stillbirth. Therefore, thyroid function is often recommended in the diagnostic investigations for stillbirth. OBJECTIVE: We aimed to evaluate the added value of thyroid function testing in the diagnostic investigations for stillbirth. DESIGN AND PATIENTS: A nationwide multicentre prospective cohort study in 1025 women who suffered stillbirth >20 weeks of gestation performed between 2002 and 2008. In each woman, an extensive diagnostic work-up was performed, including placental examination and autopsy. TSH and FT4 values below the …

Reclaiming a moral identity: stillbirth, stigma and 'moral mothers'.

OBJECTIVES: to understand the effect of expectant motherhood discourses on parents who suffer a stillbirth. DESIGN: a qualitative, exploratory study using in-depth interviews to understand parental experience of stillbirth. SETTING: interviews took place in the homes of bereaved parents across several English health authorities. PARTICIPANTS: 10 couples and 12 mothers who had experienced a stillbirth. FINDINGS: mothers were keen to distance themselves from behaviour that might be seen as stigmatising, that is, smoking, drinking, etc., while pregnant. Fathers, while keen to stress that thei…

Recurrence of stillbirth in sibships: Population-based cohort study.

Knowledge of stillbirth recurrence risk is of clinical interest and may give etiological insight. The authors studied "gestational age-" and "weight-by-gestation-specific" stillbirth recurrence, and evaluated time trends in a population-based cohort study from the Medical Birth Registry of Norway, from 1967 to 2004. Singleton births, including stillbirths from 20 weeks' gestation, were linked to their mothers by national identification numbers. Stillbirth rates in second pregnancies among mothers with (N = 5,091) and without (N = 562,057; the reference group) a stillbirth in first pregnancies …

Management of subsequent pregnancy after antepartum stillbirth. A review.

In literature, there is a paucity of information about the management of the subsequent pregnancy after stillbirth (SB). we undertook a systematic review of the literature focusing on the evidence for antenatal interventions with the potential to prevent SB and we try to summarise the management of the pregnancy subsequent to a SB. The diverse interventions and their efficacy will be reported according to the possible causes and/or conditions associated to the previous SB. Few of the studies reported SB as an outcome and the evidence was frequently conflicting. Several interventions showed cle…

Stillbirth and early neonatal mortality in rural Central Africa.

OBJECTIVE: To develop a prospective perinatal registry that characterizes all deliveries, differentiates between stillbirths and early neonatal deaths (ENDs), and determines the ratio of fresh to macerated stillbirths in the northwest Democratic Republic of Congo. METHOD: Birth outcomes were obtained from 4 rural health districts. RESULTS: A total of 8230 women consented, END rate was 32 deaths per 1000 live births, and stillbirth rate was 33 deaths per 1000 deliveries. The majority (75%) of ENDs and stillbirths occurred in neonates weighing 1500 g or more. Odds of stillbirth and END increas…

Correlations between live and stillbirth outcomes in twin pairs.

A relationship has been proposed to exist between individual outcomes (live or stillbirth) of twins in the same set. Here, we analyze this association between live births and stillbirths among individuals in different twin pairs. When national birth registers are analyzed, individuals in opposite-sex twin sets can be identified and the correlation between individual outcomes estimated. However, full information about the individuals in same-sex twin sets is not, as a rule, available, and consequently, correlation coefficients cannot be estimated, but upper and lower limits of the correlation c…

Maternal hemoglobin concentration during pregnancy and risk of stillbirth.

CONTEXT: High and low maternal hemoglobin concentrations during pregnancy have been reported to increase risk of small-for-gestational-age (SGA) birth, which is a predictor of stillbirth. The relationship between hemoglobin concentration during pregnancy and risk of stillbirth is unclear. OBJECTIVE: To study the associations among hemoglobin concentration at first measurement during antenatal care, change in hemoglobin concentration during pregnancy, and risk of stillbirth. DESIGN, SETTING, AND PARTICIPANTS: Population-based, matched case-control study of births from 1987 through 1996 in Swe…

Value of CODAC classification in identifying causes of stillbirths.

PURPOSE: Stillbirth is a public health problem and one of the main obstetrical complications of pregnancy that practitioners can ever witness. A careful etiological investigation and a search for risk factors is necessary. The main objective of our study was to identify the causes of stillbirths using the CODAC classification system. METHODS: It was a monocentric retrospective descriptive study conducted over 5 years in our gynecology department. We collected 114 cases. RESULTS: The overall frequency was 8.26 ‰. The average age of women was 32.19 years. Women came from an urban environment i…

Hidden placental damage in stillbirths with idiopathic polyhydramnios.

Idiopathic polyhydramnios is clinically linked to stillbirth; however, its placental histopathology remains poorly characterized. This is the first study analyzing placentas from stillbirths with idiopathic polyhydramnios, revealing frequent lesions of fetal and maternal vascular malperfusion. Findings support a previously unrecognized role of placental dysfunction in fetal compromise.

Preventing Stillbirth: A Review of Screening and Prevention Strategies.

Stillbirth is a devastating pregnancy complication that still affects many women, particularly from low and middle-income countries. It is often labeled as "unexplained" and therefore unpreventable, despite the knowledge that placental dysfunction has been identified as a leading cause of antepartum stillbirth. Currently, screening for pregnancies at high-risk for placental dysfunction relies on checklists of maternal risk factors and serial measurement of symphyseal-fundal height to identify small for gestational age fetuses. More recently, the first-trimester combined screening algorithm dev…
