BACKGROUND: In order to determine the short- and long-term influence of strabismus surgery on corneal curvature, a prospective study was carried out measuring corneal topography before and after operation.

Six patients were operated on for large subconjunctival cysts that developed up to 35 years after strabismus surgery. In four of these patients the cyst was found to arise between the anterior edge of the muscle and the site to which the muscle had been sutured during previous surgery. The paper deals with principles of surgical treatment of vertical squint caused by paresis of muscles of vertical action. The authors consider to be optimal one-stage surgical intervention consisting of enhancing the paretic muscle and weakening the antagonist of the same eye, i. e. the rectus and oblique muscles. The dosage of the surgery was calculated considering the angle of squint. A new method is proposed for calculating the position, on the sclera, of the point of fixation of inferior oblique muscle at its recession. Results of 73 operations are analysed. Orthotropia was achieved in 72.

Five adults with acute extraocular muscle palsies followed by limited recovery of muscle function were treated with injections of botulinum toxin A to the ipsilateral antagonist of the affected muscle. Three were cases of unilateral sixth nerve palsy, one of bilateral sixth nerve palsy, and one of third nerve palsy. After a period of paralysis, during which the strabismus was overcorrected, the injected muscle recovered and binocular sensory and motor functions were restored and have been maintained for periods of up to 21 months. There is no clinical test to detect a blockade. Under Anesthesia it is possible to discover a suspected blockade, so this test is quite helpful. Even in small babies this test can be applied in order to dected a pseudoparalysis of the external recti. We decide to operate after evaluation of the clinical facts and the findings under anesthesia according to the Zato test. We especially observe the positions of the eyes during normal breathing and the position under deep anesthesia.

Restrictions have been found to be a common cause of incomitant strabismus and limitation of ocular rotations. Different types of restrictions have been described and the technique of forced duction testing reviewed. Causes of restriction include endocrine eye disease, orbital floor and medical wall fracture, Brown's syndrome, tight lateral rectus syndrome, congenital tight muscles, scarring, muscle contracture, and orbital celluitis or tumors. The importance of recognizing restrictions in the management of these problems has been emphasized.

Retroequatorial fix-tion of a rectus muscle by means of nonabsorbable scleral sutures weakens this muscle in its primary field of action without disturbing the balance with its antagonist in other positions of gaze. This procedure has been employed in patients with dissociated vertical deviations, nystagmus compensation (blockage) syndrome, double-elevator paresis, nonaccommodative convergence excess, and Duane's retraction syndrome. Complications were not encountered, but the operation was ineffective in some patients and reoperation became necessary.

If the practitioner notices that a baby's eyes have started to turn in, or that they do not focus properly, the condition should be investigated. It might be a tumor or some other pathological condition and early diagnosis and treatment are important. If the child blinks, closes one eye in the sun, the eyes become red, or he appears to have difficulty in seeing, he should be referred for thorough examination and treatment.
