[Ischemic stroke registry].

Current knowledge of stroke risk factors and epidemiology is based mostly on USA or European studies; scarce data have been published from developing countries. Because epidemiological and clinical characteristics in stroke vary according to regional factors, we need to know the peculiarities of stroke on this subcontinent. The purpose is to describe the clinical subtypes and risk factors in patients with ischemic stroke. We analyzed all consecutive ischemic stroke in patients admitted at Hospital Italiano of Buenos Aires, between June 1, 2003 and June 1, 2005. Among 395 ischemic stroke patien…

[Post-stroke depression].

In recent years, post-stroke depression (PSD) has attracted a worldwide interest. This review addresses the major research issues of PSD. PSD is the most frequent psychic complication among stroke survivors. Depression has a significant impact on post-stroke recovery and mortality. The diagnosis of PSD is based on the structured mental state examination and DSM-IV criteria as well as on the scores of the clinical symptoms scale (the Hamilton Depression Rating Scale) and self-rated inventories. PSD is multifactorial in origin and develops under the influence of biopsychosocial factors. The anat…

Acute haemorrhagic stroke.

OBJECTIVE: To review the management and some of the recent advances in acute haemorrhagic stroke. DATA SOURCES: Articles and published reviews on acute haemorrhagic stroke. SUMMARY OF REVIEW: Hypertensive intracerebral haemorrhage or subarachnoid haemorrhage (SAH) from a ruptured intracranial saccular aneurysm are the commonest causes for an acute haemorrhagic stroke. Both lesions are often clinically characterised by a sudden severe headache and vomiting with the remaining neurological features dependent on the site of the lesion. The diagnosis requires an urgent non-contrast cerebral compu…

[Secondary stroke prevention].

The guidelines for secondary stroke prevention, graded following available scientific evidence, are presented. Stroke and TIA are defined and the indications for referral established. Basic assessment of stroke patients should include laboratory evaluation, ECG, brain CT, ultrasound examination of the extracranial vessels for events in the carotid distribution, and transthoracic or transesophageal echocardiogram if cardioembolism is suspected. The pharmacological and non-pharmacological reduction of blood pressure and serum cholesterol, stopping smoking and reducing alcohol intake are general …

Fractures after stroke.

Fractures are a serious complication after stroke. Among patients with femoral neck fractures, a large subgroup have had a previous stroke. This study aimed to investigate the incidence of fractures after stroke. Included in the study were 1139 patients consecutively admitted for acute stroke. Fractures occurring from stroke onset until the end of the study or death were registered retrospectively. Hip fracture incidence was compared with corresponding rates from the general population. Patients were followed up for a total of 4132 patient-years (median 2.9 years). There were 154 fractures in …

Recurrent alternating stroke.

Recurrent alternating stroke, i.e., one time ischemic and the other hemorrhagic or vice versa, is an uncommon event. We report a series of eight patients who had recurrent alternating strokes, which were diagnosed by CT scans during the last four years. Infarcts preceded hemorrhage in six patients. In the remaining two patients, hemorrhage developed first and infarct followed. All ischemic strokes were the lacunar infarcts. The lesions of the two attacks were located in different sites in all cases except one. The mean age of the patients was 56.6 years at the time of the first attack and 57.5…

[Rehabilitation after stroke].

Our hospital's stroke rehabilitation programme has been evaluated through a retrospective study of the record of 183 males (mean age 60 +/- 11 years) and 94 females (mean age 59 +/- 14 years) admitted in 1982/83. 87% of the patients had motoric impairments on admission; 45% had apraxia, 17% neglect, 38% aphasia and 57% other cognitive impairments. Most patients had both motoric and cognitive problems. 79% were able to ambulate freely at discharge as opposed to 55% on admission. 88% were independent in ADL when discharged; 67% on admission. The average length of stay (LOS) was shorter (p less t…

Dementia following stroke.

Strokes can be due to ischemic or hemorrhagic vascular disorders. Ischemic strokes outnumber hemorrhagic strokes approximately 4:1. Although the mode of presentation and pathophysiology are different in the two conditions, the outcome is really dependent on the extent and location of brain injury. A CT scan helps in this regard and reveals surgically correctable lesions such as a subdural hematoma or normal pressure hydrocephalus. Effective rehabilitation of the stroke patient is dependent on motivation and cognitive ability even more than on remaining motor or sensory function. A team approac…

Jogger's heat stroke.

Two patients developed heat stroke following jogging. Transient disturbance of cerebral function was the most dramatic clinical feature. Although haemorrhagic complications were not seen, marked changes in the haemostatic system occurred with both thrombocytopenia and a reduction of clotting factors synthesised by the liver. No evidence of disseminated intravascular coagulation was found. Heat stroke must now be added to the list of jogging hazards.

The phenomenon of acute stroke and acute stroke care provision from the perspective of people with stroke: a descriptive phenomenological study.

PURPOSE: Experiencing a stroke is a distressing event, however little is known about the person with stroke's experience of the phenomenon of acute stroke, or of acute stroke care. Understanding the first 48 h of the acute stroke experience from the perspective of the person with stroke could inform the delivery of person-centred care and identify future acute stroke healthcare priorities. MATERIALS AND METHODS: A descriptive phenomenological approach using semi-structured interviews was conducted with fourteen participants in the acute hospital setting within seven days of their stroke. RES…

Rural Health and Atypical Stroke: Technology-Based Posterior Circulation Stroke Diagnosis.

Diagnostic errors cause an estimated 900,000 deaths and disabilities annually in the United States, and stroke is the leading cause of diagnostic error-related harm in US emergency departments. Posterior circulation strokes (PCS's) are frequently misdiagnosed due to atypical presentations such as dizziness. Key contributors to rural-urban care gaps in PCS diagnosis are highlighted for rural care (eg, challenges in bedside diagnosis and limited access to neurologists). Emerging technologies (eg, telestroke, video-oculography-based teledizzy programs, mobile stroke units, and smartphone tools) c…

Association of Acute Ischemic Stroke Volume With Post-Stroke Epilepsy Development.

BACKGROUND AND PURPOSE: Stroke is a leading cause of epilepsy, especially in older adults. The SeLECT score remains the standard among post-stroke epilepsy (PSE) prediction tools. However, its broader validation is limited by the need to manually extract neuroimaging predictors (cortical and middle cerebral artery [MCA] involvement). Unlike the CAVE score, SeLECT did not evaluate acute stroke volume, which can now be quantified automatically. We aimed to determine whether stroke volume independently predicts PSE and compare its predictive contribution to SeLECT's neuroimaging variables. METHO…

Stroke recurrence after transcatheter PFO closure in patients with cryptogenic stroke.

BACKGROUND: Patent foramen ovale (PFO) closure is recommended for secondary prevention of cryptogenic stroke. However, data on long-term results are limited. We aimed to evaluate safety and efficacy of transcatheter PFO closure and predict neurologic recurrence. METHODS: Data from patients undergoing PFO closure between 2010 and 2015 were collected to assess the combined endpoint of transient ischemic attack (TIA), stroke, or death from stroke at short- and long-term follow-up. RESULTS: 330 patients were included, mean age was 49 (±12) years, and 55.5% were male. Before PFO closure, 86% expe…

Trimethylamine N-Oxide and Stroke Recurrence Depends on Ischemic Stroke Subtypes.

BACKGROUND: Trimethylamine N-oxide (TMAO) has been recognized as a risk factor for cardiovascular disease. However, the role of TMAO in ischemic stroke remains unclear. As we know, ischemic stroke is a heterogeneous disease with variable pathogenesis. Hence, we aimed to investigate the association between TMAO and stroke recurrence according to etiology subtypes. METHODS: A total of 10 756 ischemic stroke/transient ischemic attack patients from the Third China National Stroke Registry were enrolled, and 1-year follow-up data for stroke recurrence were analyzed. TOAST (Trial of ORG 10172 in Ac…
