[Cis-diamminedichloroplatinum penetration into the cerebrospinal fluid of the lateral ventricle, postoperative cavity, and lumbar subarachnoid space with or without pre-intravenous mannitol administration in patients with brain metastasis from lung cancer].

We have previously reported that repeated intravenous administration of cis-diamminedichloroplatinum(CDDP)prevented local recurrence of metastatic brain tumors after surgical total excision; however, data on CDDP distribution in the postoperative cavity after intravenous CDDP administration are not available. In the present study, we evaluated the penetration of total platinum(Pt)into the cerebrospinal fluid(CSF)and the effect of intravenous administration of 20%mannitolon total Pt distribution in the plasma and CSF. Total Pt levels in the plasma and CSF were determined immediately after intra

[Subarachnoid hemorrhage and circulatory disturbance of cerebrospinal fluid--scanning electron microscopid study in clinical and autopsy cases (author's transl)].

The scanning electron microscopy (SEM) gives intersting information about the changes of the subarachnoid space. In this study, specimens from 16 patients (one during surgery and others at autopsy) were examined by SEM: 11 cases of subarachnoid hemorrhage, 2 of posterior fossa brain tumor operated and 3 of control cases without CNS diseases. In cases of subarachnoid hemorrhage, there seemed to be three stages of the obstruction in the subarachnoid space: a) obstruction by blood clots mainly consisted of red blood cells, b) obstruction by subarachnoid fibrosis with thickening of arachnoid, c) o…

Circulation of marker substances in the cerebrospinal fluid of an amphibian, Rana pipiens.

Solutions of fluorescein-labelled dextran or Evans blue-albumin were infused into the lateral cerebral ventricle of Rana pipiens. The subsequent distribution in the cerebrospinal fluid (CSF) was investigated between 2 and 24 h after infusion by freezing and examination of the cut blocks of the head and vertebral column of the stage of a freezing microtome. These marker substances move out of the ventricles into the subarachnoid space at the caudal end of the fourth ventricle and spread rapidly along the subarachnoid space of the spinal cord. The spreading of marker substances is slower into th…

[Hypothesis of cerebrospinal fluid formation and absorption: from the clinical viewpoint of idiopathic normal pressure hydrocephalus].

The morphological features of disproportionately enlarged subarachnoid-space hydrocephalus (DESH), the core type of idiopathic normal pressure hydrocephalus, can not be explained by the classical theory of CSF absorption at the arachnoid villi and the hypothesis of CSF flow blocking at the convexity subarachnoid spaces. By using MRI Time-SLIP CSF flow imaging, we demonstrated that CSF freely move in the subarachnoid spaces below and in the Sylvian fissures. CSF does not move in the convexity subarachnoid spaces in healthy individuals and patients with DESH, indicating the presence of flow obst…

[Morphological findings in MRI to be considered in perfoming spinal and epidural anesthesia].

The spinal cord and cauda equina move in the subarachnoid space by changing positions. The subarachnoid space is compressed from the dorsal side by epidural injection. At the end of gestation, the subarachnoid space is compressed from the ventral side by the engorged venous plexus. Idiopathic epidural lipomatosis is observed almost exclusively in the obese population. In the case of lipomatosis, the subarachnoid space is compressed by increased extradural fat around the thecal sac. We have had some severe cases of spinal canal stenosis in whom it is difficult to perform the spinal puncture. Th…

Cerebrospinal fluid area and syringogenesis in Chiari malformation type I.

OBJECTIVE: Syringogenesis in Chiari malformation type I (CM-I) is thought to occur secondary to impaction of the cerebellar tonsils within the foramen magnum (FM). However, the correlation between the CSF area and syringogenesis has yet to be elucidated. The authors sought to determine whether the diminution in subarachnoid space is associated with syringogenesis. Further, the authors sought to determine if syrinx resolution was associated with the degree of expansion of subarachnoid spaces after surgery. METHODS: The authors performed a retrospective review of all patients undergoing posteri…

The Basal Subarachnoid Cisterns: Surgical and Anatomical Considerations.

The basal subarachnoid cisterns are expansions of the subarachnoid space and transmit cranial nerves and intracranial vessels. Providing neurosurgeons with key concepts, anatomical landmarks, and techniques can result in safer procedures and better patient outcomes. In this review, we discuss the major basal subarachnoid cisterns including their embryology, history, anatomical descriptions, and use during surgical approaches.

Development of the cerebrospinal fluid pathway in the normal and abnormal human embryos.

The subarachnoid space, the chorioid plexus and the arachnoid villi are microscopically studied in 60 normal human embryos and in 3 abnormal human embryos with rhombencephaloschisis and cervical myeloschisis. The subarachnoid space has been generally considered to be developed by outflow of cerebrospinal fluid (CSF) of the choroid-plexus origin from the IVth ventricle. This generally accepted concept does not meet with our findings: (1) cavity formation in the meninx primitiva is seen before appearance of the choroid plexus; (2) the primitive subarachnoid space is developed earlier in the prep…

The olfactory route for cerebrospinal fluid drainage into the peripheral lymphatic system.

Drainage of the cerebrospinal fluid through the olfactory nerves into the nasal lymphatics has been suggested repeatedly. To investigate precisely the morphology of this pathway, India ink was injected into the subarachnoidal space of the rat brain, and samples including the olfactory bulbs, olfactory tracts and the nasal mucosa were observed by light and electron microscopy. Under the dissecting microscope, ink particles were found within the subarachnoid space and along the olfactory nerves. At the nasal mucosa, a lymphatic network stained in black was identified near the olfactory nerves, w…

Distinct volumetric features of cerebrospinal fluid distribution in idiopathic normal-pressure hydrocephalus and Alzheimer's disease.

OBJECTIVE: The aims of the study were to measure the cerebrospinal fluid (CSF) volumes in the lateral ventricle, high-convexity subarachnoid space, and Sylvian fissure region in patients with idiopathic normal-pressure hydrocephalus (INPH) and Alzheimer's disease (AD), and to evaluate differences in these volumes between INPH and AD groups and healthy controls. METHODS: Forty-nine INPH patients, 59 AD patients, and 26 healthy controls were imaged with automated three-dimensional volumetric MRI. RESULTS: INPH patients had larger lateral ventricles and CSF spaces of the Sylvian fissure region …

Indirect imaging of the canine optic nerve, using metrizamide (optic thecography).

Between the optic nerve and its sheath is a small, CSF-filled space that communicates with the subarachnoid space of the brain. A technique for the radiographic opacification of the optic nerve subarachnoid space following the intrathecal injection of metrizamide was investigated in 8 dogs. The technique enabled indirect visualization of the optic nerves from the optic chiasm to the eyeball and of structures within the subarachnoid basal cisterns of the brain, including the optic chiasm, hypophysis, and blood vessels of the cerebral arterial circle. Displacement and obstruction of the optic ne…

Spinal subarachnoid hemorrhage accompanied with intraventricular hemorrhage.

Spinal hematoma is a rare and usually severe neurological disorder that, without adequate treatment, often leads to death or permanent neurological deficit. Epidural as well as subdural and subarachnoid hematomas have been investigated in some studies. A 66-year-old man referred to our hospital because of acute onset paraplegia and incontinency started 3 h before admission. With impression of spinal hemorrhage, emergent cervicothoracic spinal MRI performed. On magnetic resonance imagination (MRI) mixed hyper/iso intense lesion in anterior subarachnoid space from C7 to T5 was seen. On brain A c…

MR of cranial and spinal meningeal carcinomatosis: comparison with CT and myelography.

Thirty-nine patients with histologically proved primary neoplasms, focal neurologic deficits, and positive CSF cytology were evaluated by enhanced cranial CT and MR, or complete myelography and MR of the spine. Intracranial abnormalities were noted on CT in 56% of cases and included abnormal enhancement of subarachnoid space and ventricular walls, ventricular dilatation, obliteration of cortical sulci, and enhancing nodules within the subarachnoid cisterns and lumen of the lateral ventricles. Although the degree of ventricular enlargement and intraventricular tumor deposits were equally well s…

Prevalence of subdural collections in children with macrocrania.

BACKGROUND AND PURPOSE: The relationship between enlarged subarachnoid spaces and subdural collections is poorly understood and creates challenges for clinicians investigating the etiology of subdural collections. The purpose of this study was to determine the prevalence of subdural collections on cross sectional imaging in children with macrocephaly correlating with subarachnoid space enlargement. MATERIALS AND METHODS: The radiology information system of a large pediatric medical center was reviewed for "macrocrania" and "macrocephaly" on reports of cranial MRI/CT examinations in children <…

Magnetic resonance imaging characteristics of benign macrocephaly in children.

Benign macrocephaly of infancy is a common problem in the child neurology practice. The radiologic features of this entity are not well defined. In most of the previous studies, macrocephalic patients were evaluated by computed tomography. To define the radiologic characteristics of this entity, 20 children with macrocephaly with normal neurologic examinations were enrolled in the study. All the patients were evaluated by magnetic resonance imaging studies. Sixty-five percent of patients had enlargement of the subarachnoid space and 35% of patients had megalencephaly. None of the patients had …

Histologic characteristics of normal perivascular spaces along the optic tract: new pathogenetic mechanism for edema in tumors in the pituitary region.

BACKGROUND AND PURPOSE: Perivascular (PV) spaces are known to distend and cause edema along the optic tract (OT) in pituitary-region tumors. Interstitial fluid may be retained in PV spaces when tumors block their drainage outlets to subarachnoid spaces. However, these spaces and their outlets have not been anatomically elucidated. Our purpose was to evaluate how often large PV spaces are present along the OT and demonstrate their superficial communication points to adjacent subarachnoid spaces. METHODS: We examined serial histologic sections of 10 hemispheric blocks obtained from cadavers wit…

Neuroimaging of Adult Hydrocephalus.

Ventricle expansion and disproportionately enlarged subarachnoid-space hydrocephalus (DESH) are the hallmark features of idiopathic normal-pressure hydrocephalus, which has recently also been referred to as Hakim's disease. DESH refers to the unbalanced distribution of CSF in the subarachnoid space characterized by tightened sulci in the high convexities and Sylvian fissure dilation. The ventricles and Sylvian fissure expand in the parietal direction. This process compresses the brain and subarachnoid space around the parietal region. In addition to 2-dimensional imaging indicators, this study…

MRI appearances mimicking the dural tail sign: a report of two cases.

We report two cases in which the MRI appearances mimicked the dural tail sign; a glioma extending into the subarachnoid space, and a meningioma extending to the subdural space. They indicate that tumour invasion into the subarachnoid or subdural space, should be considered when prominent linear enhancement is observed along the dura mater adjacent to tumours.

Intracavernous carotid artery aneurysms: the possible importance of angiographic dural waisting. A case report.

BACKGROUND: Cavernous carotid aneurysms are generally benign entities. Certain indications exist for their treatment, however, including transient ischemic events, subarachnoid hemorrhage or risk of subarachnoid hemorrhage, epistaxis or its risk, ophthalmoplegia, pain, and progressive visual loss. We feel certain angiographic features may indicate a greater likelihood that cavernous carotid aneurysms extend into the subarachnoid space, thus making their rupture a life-threatening event. METHODS: A case report of an intracavernous carotid aneurysm, which at surgery extended into the subarachno…
