Management of recurrent stress incontinence following a sling.

Management of recurrent or persistent stress urinary incontinence (RSI) following primary insertion of a synthetic midurethral sling (MUS) remains a challenge for the urologist since no consensus is available to favor one treatment over another. Complete workup should be carried out, including cystoscopy, urodynamics and potentially a pelvic floor ultrasound as a diagnostic adjunct. Various surgical options have been described for RSI, apart from another MUS, including tightening of or shortening a previously placed MUS, a mini-sling, a salvage spiral sling, a colposuspension, the ACT® system,

[Surgical treatment of female urinary stress incontinence].

Since the description of the sub urethral technique for genuine stress incontinence all the others have been abandoned. The surgical indications have not changed and are related to the poor quality of life of the patient induced by stress incontinence. A careful clinical examination and a pre operative urodynamique investigation should prevent from the failures due to a wrong indication. All the techniques are the same when it comes to the result except the mini slings which still have to be evaluated. There are two types of complications: the ones which are specific to the using of a hexogen

Recurrent urinary stress incontinence: an overview.

AIM: We review the literature regarding recurrent urinary stress incontinence (USI) following various anti-incontinence procedures and analyze possible risk factors for failure of different anti-incontinence procedures. Management options for recurrent USI and future trends are also covered. METHODS: A PubMed search was undertaken for English language literature using the MeSH terms 'stress urinary incontinence', 'recurrence', 'treatment', and 'failure'. Literature from the year 2001 to 2010 was selected. We only selected those studies including randomized control trials, meta-analysis, and p

Transobturator tension free vaginal tapes and bladder injury.

INTRODUCTION: Tension free vaginal tapes have transformed the surgical management of female urodynamic stress incontinence (USI). Transobturator approach of tension free vaginal tapes are emerging as one of the surgical options for USI. MATERIALS AND METHODS: A literature search to answer question of routine cystoscopy during transobturator tape procedures. CONCLUSION: Though, both TOT and TVT-O has been shown not to enter pelvis on anatomical dissection on female cadavers, many cases of bladder injury have been reported. Delorome and De Leval strongly suggest avoiding routine cystoscopy dur

[Vaginal mesh fixed by transobturator tape can lead to ureteral injury in cystocele surgery].

The lower third ureteral injuries are not rare during pelvic surgeries. We report for the first time a case of ureteral injury during a transvaginal cure of an anterior vaginal wall prolapse with a synthetic mesh, whose the four arms are fixed by transobturator way. This lesion is itself included in the lesions of the low urinary tract during vaginal surgery with transobturator tape. The difficult diagnosis of these iatrogenic injuries must be carried early in order to decrease their morbidity and in the case of ureteral injury to preserve the nephrogenic capital.

Tension-free vaginal tape obturator system in the treatment of stress urinary incontinence.

Surgical approach to the treatment of female stress urinary incontinence has changed significantly since 1995, when Ulmsten first described the procedure providing tension-free support in the middle part of urethra. Although retropubic placement of TVT tapes seemed to meet all the standards of minimally invasive surgical procedures, the occurrence of complications (bleeding, hematomas, and bladder, urethral of intestinal injuries) necessitated a new obturator approach. This study presents a transobturator approach to TVT tapes placement (Gynecare, Johnson-Johnson Company) with "inside-out" tec

Urinary stress incontinence treatment with Remeex in a patient with epispadias: a case report.

Epispadias is a rare condition in women. A postmenopausal 53-year-old woman with a known problem of epispadias came to our outpatient clinic complaining of persistent severe urinary stress incontinence without overactive bladder symptoms. She had previously undergone an inefficacious tension-free vaginal tape (TVT) procedure in another institution. We implanted a Remeex (Mechanic External Regulation) device without applying any tension to the sling. Two tape regulations were performed the day after surgery and 2 months later to gain continence. At 13 months of follow up, this woman was asympto

Comparison of 2 single incision slings on the vagina in an ovine model.

BACKGROUND: Stress urinary incontinence carries a significant healthcare burden for women worldwide. Single incision slings are minimally invasive mesh devices designed to treat stress urinary incontinence. For prolapse repair, meshes with higher porosity and lower structural stiffness have been associated with improved outcomes. OBJECTIVE: In this study, we compared the higher stiffness, lower porosity Altis sling with the lower stiffness, higher porosity Solyx sling in an ovine model. We hypothesized that SIS-B would have a negative impact on the host response. STUDY DESIGN: A total of Alt

Modifications of Anti-Incontinence Procedures for Radiated Patients: A Narrative Review.

PURPOSE OF REVIEW: The mainstay of surgical management for male stress urinary incontinence (SUI) for many years has been the male urethral sling and the artificial urinary sphincter. However, patients with pelvic radiation history are at increased risk for poorer outcomes. This review examines the surgical options for radiated men with SUI, how this history may affect outcomes, and surgical modifications which may optimize results. RECENT FINDINGS: Male urethral slings demonstrate diminished success but may be considered in carefully selected radiated patients. The artificial urinary sphincte

More With Less: Single-Incision Sling Insertion Techniques.

STUDY OBJECTIVE: To demonstrate reproducible procedures for efficient single-incision sling insertion and troubleshooting. DESIGN: Narrated video footage with stepwise demonstration of single-incision sling insertion technique with anchor system. SETTING: The mesh midurethral sling is a highly effective and safe procedure that is considered the gold standard for surgical treatment of stress urinary incontinence. Retropubic and transobturator approaches for midurethral slings have similar subjective cure rates with differing surgical risk profiles [1,2]. The retropubic route has a higher risk

Retreatment of Stress Urinary Incontinence After Midurethral Sling With Prior Urethral Bulking.

IMPORTANCE: There is limited current literature regarding the retreatment of stress urinary incontinence (SUI) after midurethral sling (MUS) placement with prior urethral bulking. OBJECTIVE: The objective was to evaluate the retreatment and perioperative complications of MUS placement with prior urethral bulking compared with MUS placement without prior urethral bulking. STUDY DESIGN: This was a retrospective cohort study of patients within the Southern California Permanente Medical Group who underwent MUS placement from January 2009 to December 2020. Patients who underwent prior urethral bu

Should We Call It a Prostate? A Review of the Female Periurethral Glandular Tissue Morphology, Histochemistry, Nomenclature, and Role in Iatrogenic Sexual Dysfunction.

INTRODUCTION: There is evidence of glandular tissue within the region of the anterior vaginal wall-female periurethral tissue (AVW-FPT) having similar morphology and immunohistochemistry to the prostate in men and having physiological roles in the female sexual response (FSR). Whether this tissue should be called a prostate in women has been debated. Iatrogenic injury to structures of the AVW-FPT, including these glands and the associated neurovasculature, could be a cause of female sexual dysfunction (FSD). OBJECTIVES: To consolidate the current knowledge concerning the glandular tissue surr

Current treatments for female primary stress urinary incontinence.

Stress urinary incontinence (SUI) affects millions of women worldwide. Pelvic floor muscle training is the first-line treatment for SUI, and if this fails, midurethral sling surgery has become the gold-standard treatment. More recently, complications from midurethral mesh slings, particularly chronic pain and dyspareunia, have become a major concern. Although traditional SUI treatments, such as colposuspension and fascia slings, are used, the future of SUI treatment likely will rely on less invasive alternatives. Modern bulking agents could have the potential to become a first-line treatment f

Informed consent checklists for midurethral slings: a common-sense approach.

INTRODUCTION AND HYPOTHESIS: Following the US Food and Drug Administration's (FDA's) warning about the use of transvaginal mesh to treat pelvic organ prolapse (POP) and the use of single-incision slings to treat incontinence, the number of lawsuits for medical negligence regarding the use of any polypropylene mesh in the vagina has increased tremendously. METHODS: This same FDA document did not question the use of polypropylene midurethral slings and polypropylene for sacrocolpopexies. Surprisingly, despite all the evidence and recommendations from respected international scientific societies

TOT 8/4: A Way to Standardize the Surgical Procedure of a Transobturator Tape.

Suburethral tapes are placed "tension-free" below the urethra. Several studies reported considerable differences of the distance between urethra and tape. These distances ranged from 1 to 10 mm amongst different patients. This either caused urethral obstruction or had no effect on urinary incontinence. Therefore, we decided to standardize the procedure by placing a Hegar dilator of 8-millimeter diameter in the urethra and another Hegar dilator of 4-millimeter diameter between the urethra and the tape during transobturator tape placement. Using that simple technique, which we named "TOT 8/4," w
