The effect of preoperative consumption of potatoes on succinylcholine-induced block and recovery from anesthesia.

Potatoes contain solanaceous glycoalkaloids (SGAs), which inhibit both butyrylcholinesterase (BuChE) and acetylcholinesterase (AChE). The present study investigated the effect of preoperative consumption of potatoes on succinylcholine-induced block and recovery from anesthesia. ASA I-II, adult patients, scheduled for elective surgery, were included in a randomized, blind and controlled study. Patients were randomly divided into two groups. Patients in Group P (n = 21) ate a standard portion of potatoes in their last meal prior to pre-operative fasting, while patients in Group C (n = 23) ate fo…

Effect of magnesium sulfate with propofol induction of anesthesia on succinylcholine-induced fasciculations and myalgia.

BACKGROUND: Magnesium sulfate and propofol have been found to be effective against succinylcholine-induced fasciculations and myalgia, respectively, in separate studies. A prospective randomized double blind controlled study was designed to assess the effect of a combination of magnesium sulfate with propofol for induction of anesthesia on succinylcholine-induced fasciculations and myalgia. MATERIALS AND METHODS: Randomly selected 60 adult patients scheduled for elective surgery under general anesthesia were allocated to one of the two equal groups by draw of lots. The patients of MG Group we…

Prolonged neuromuscular blockade following succinylcholine administration to a patient with a reduced butyrylcholinesterase activity.

This case is about a 48-year-old woman known with a reduced butyrylcholinesterase activity, who developed prolonged neuromuscular blockade following the unintentional administration of succinylcholine. We took the opportunity to monitor the development of neuromuscular function during this period and blood samples were taken for molecular genetic analysis and for quantitative and qualitative analysis since not all causative mutations are functionally characterized. Reduced butyrylcholinesterase activity is discussed in many aspects. Clinical considerations are suggested concerning genetic coun…

Effects of an intubating dose of succinylcholine and rocuronium on the larynx and diaphragm: an electromyographic study in humans.

BACKGROUND: Paralysis of the vocal cords is one objective of using relaxants to facilitate tracheal intubation. This study compares the neuromuscular blocking effect of succinylcholine and rocuronium on the larynx, the diaphragm, and the adductor pollicis muscle. METHODS: Electromyographic response was used to compare the neuromuscular blocking effect of succinylcholine and rocuronium on the laryngeal adductor muscles, the diaphragm, and the adductor pollicis muscle. Sixteen patients undergoing elective surgery were anesthetized with propofol and fentanyl, and their tracheas were intubated wi…

[Use of atracurium for the prevention of fasciculations and succinylcholine myalgia in athletes undergoing orthopedic surgery].

The Authors report their clinical experience about the use of atracurium besylate in preventing succinylcholine-induced fasciculations and postoperative myalgias in fifty athletes (ASA class 1), submitted to arthroscopic meniscectomy. The patients were pretreated with atracurium 5 mg i.v. or saline solution in a double-blind fashion. After 2.5 minutes, succinylcholine 1.3 mg/kg was administered, and fasciculations were recorded on a scale ranging from 0 to 3. Twenty-four hours after surgery all subjects were questioned about myalgias occurrence, scored by a 0-3 scale. Fasciculations occurred i…

Physiologic effects of succinylcholine chloride in mechanically ventilated horses anesthetized with halothane in oxygen.

Succinylcholine chloride, administered to horses anesthetized with halothane in oxygen and mechanically ventilated, caused slight but significant (P less than 0.01) increases in heart rate. Significant alteration in mean arterial blood pressure did not occur, and there were clinically insignificant increases in serum K+ and C1- concentration. Cardiac dysrhythmia and myoglobinuria did not occur. Thus, effects of halothane anesthesia and mechanical ventilation prevented cardiac dysrhythmia and hypertension and greatly reduced the tachycardia generally associated with siccinylcholine administrati…

Porcine malignant hyperthermia induced by halothane and succinylcholine: failure of treatment with procaine or procainamide.

Metabolic, hemodynamic and neuroendocrine responses to the combined use of halothane and succinylcholine (SCh) were measured in five normal swine and five swine susceptible to malignant hyperthermia (MH). Constant-volume ventilation was used, and no therapy was instituted. The overall response in susceptible swine was fulminant, in that it involved the rapid onset of SCh-induced MH combined with the more severe metabolic, endocrine, and cardiovascular effects of halothane-induced MH. Maximal changes in VO2 were equivalent with either drug or both combined, while changes in lactate, potassium (…

Hyperkalemic emergency department patients intubated with rocuronium or succinylcholine: Retrospective study of clinical outcomes.

STUDY OBJECTIVE: Emergency department (ED) clinicians often avoid succinylcholine due to concerns about potassium elevation leading to adverse events, though clinical evidence beyond case reports is limited. Rocuronium is a non-depolarizing paralytic that does not affect potassium levels. Therefore, it is perceived as safer despite causing longer paralysis than succinylcholine. This study aimed to compare adverse clinical outcomes in patients with pre-existing hyperkalemia who received either succinylcholine or rocuronium during rapid sequence intubation (RSI) in the ED. METHODS: We conducted…

Preoperative pregabalin prevents succinylcholine-induced fasciculation and myalgia: A meta-analysis of randomized trials.

Succinylcholine is the gold standard neuromuscular blocker for rapid sequence induction; however, its use is associated with fasciculation and myalgia. We performed a systematic review and meta-analysis of randomized controlled clinical trials comparing gabapentinoids versus placebo for the prevention of fasciculations and succinylcholine-induced myalgias. Six randomized clinical studies were included with a total of 481 patients - 241 in the intervention group and 240 in the placebo group. Gabapentinoids reduced the incidence of succinylcholine-induced myalgia (RR=0.69, 95% CI 0.56-0.84, P<.0…

Relationship between cardiac output and onset of succinylcholine chloride action in electroconvulsive therapy patients.

BACKGROUND: Succinylcholine chloride (Sch) is ideal for electroconvulsive therapy (ECT). However, the appropriate interval between Sch administration and electrical stimulation has not been reported. Cardiac output at the time of drug administration seems to be the major contributing factor for variability in onset time. The present study therefore investigated relationships between cardiac output before Sch administration and the onset of Sch action. METHODS: Cardiac output and cardiac index (CI) were continuously monitored in 24 patients using a noninvasive impedance cardiac output monitor.…

Succinylcholine-induced myalgia in obstetric patients scheduled for caesarean section--diclofenac vs placebo patches.

BACKGROUND: Succinylcholine-induced myalgia is a minor but frequent complication. Its incidence and severity is different according to the studied population. The aim of this study was evaluation of the diclofenac patch effect on postoperative succinylcholine-related myalgia in cesarean section. METHODS: The study was a prospective randomized double blind, placebo-controlled trial. One hundred twenty six participants undergoing elective cesarean section (previous cesarean section) were randomized in two equal groups (63 participants in each): the diclofenac patch (containing 180 mg of diclofe…

Cardiac arrest after succinylcholine administration in a pregnant patient recovered from Guillain-Barré syndrome.

This report describes a parturient who, within 1 month of recovering from a neurologic deficit, suffered cardiac arrest secondary to succinylcholine-induced hyperkalemia. This patient's course may not apply to all patients with GBS because the diagnosis of GBS, although supported on clinical grounds, was not supported by conclusive, objective data. Furthermore, pregnancy, drug abuse, and malnutrition may have had a unique impact on her muscle physiology. Nevertheless, it is important to note that, in any patient who is recovering or has clinically recovered from a neurologic deficit, a normal …
