Stenosing tenosynovitis of the flexor hallucis longus tendon at the sesamoid area.

The author presents a case of stenosing tenosynovitis of the flexor hallucis longus tendon at the sesamoid area of the great toe following injury of the hallux. Although stenosing tenosynovitis of the flexor hallucis longus tendon is not rare, occurring frequently in ballet dancers, its entrapment at the sesamoid area was rarely described in the literature. Early recognition of this condition is very important for successful treatment. This patient did not respond to nonoperative treatment and surgical tenolysis was very successful for relief of the symptoms.

Serous flexor tenosynovitis as an associated finding in meningococcal septic polyarthritis.

Flexor tenosynovitis complicating meningococcal septic polyarthritis may be sterile and could be misdiagnosed as disseminated gonococcal infection. Awareness of clinical findings associated with meningococcal polyarthritis is recommended to prevent onset of fulminant meningococcemia (Waterhouse-Friderichsen syndrome).

[An uncommon occupational accident: tuberculous tenosynovitis of the extensor tendons of the hand].

We present a case of tuberculous tenosynovitis of the extensor tendons of the hand. Our patient was a young doctor working in the respiratory medicine department. He was injured on the dorsal aspect of the hand with a needle used for pleural aspiration. The clinical features consisted of gradually swelling, mild pain and stiffness of the metacarpophalangeal joint. The diagnosis was made after synovectomy. Histological and bacteriological examinations revealed tuberculosis. Treatment consisted of synovectomy and appropriate antibiotics. The clinical course was excellent after one year follow-up…

Mycobacterial tenosynovitis of the flexor tendons of the hand. A report of five cases.

We present five cases of mycobacterial tenosynovitis of the flexor tendons of the fingers. These cases were observed during the last 12 years and treated by the same surgeon. This pathology is uncommon now, but it is becoming more frequent, especially in patients with diminished immunity. The diagnosis was most commonly made after synovectomy in patients presenting with carpal tunnel syndrome associated with slightly painful swelling at the wrist. Histological and bacteriological examinations are very important and revealed tuberculosis in four of our patients and mycobacterium in one, and the…

Rheumatoid factor and HLA antigens in wrist tenosynovitis and humeral epicondylitis.

A matched case-referent study was undertaken to assess whether rheumatoid factor (RF) is associated with repeating tenosynovitis or peritendinitis and whether HLA-B27 predisposes to epicondylitis. The study subjects consisted of 25 workers in manually strenuous jobs with a history of at least two episodes of tenosynovitis or peritendinitis in the wrist or forearm, or humeral epicondylitis, and their matched referents. The latex agglutination test was positive in seven of the 23 cases with tenosynovitis and in one of the referents (p = 0.03). The corresponding figures for IgM-RF by enzyme immun…

[Ultrastructure of the long flexor and extensor tendons of the hand in rheumatic tenosynovitis].

Tendon ruptures are a frequent problem in patients affected by rheumatoid arthritis. The aim of the present study was to further characterize the known macroscopic changes of tendon tissue by micromorphological methods. Flexor and extensor tendon samples (n = 28) in rheumatoid arthritis were investigated by light- and transmission electron microscopy. In all cases, marked alterations in fibril architecture and structure were found. The fibril diameter showed a higher variability (range = 20-490 nm, x = 98.9 nm, s = 49.1 nm) in comparison with the control group of normal tendons (range = 20-290

Management of Stenosing Tenosynovitis With the Eastwood Technique: Handling, Advantages, and Disadvantages.

Stenosing tenosynovitis, or trigger finger, is a common condition resulting from repetitive use of the finger tendons, causing inflammation and narrowing of the flexor sheaths. This causes difficulty in sliding the tendon, which can lead to locking or pain when extending the fingers. The condition is associated with chronic diseases, such as diabetes and rheumatoid arthritis, and has a higher incidence in people between the ages of 40 and 60 years. Treatments include conservative approaches, such as the use of anti-inflammatory drugs and corticosteroids, or definitive approaches, such as surge…

Gonococcal Tenosynovitis With Abscess Formation: A Rare Presentation of Disseminated Infection.

Gonococcal tenosynovitis is a rare but serious manifestation of disseminated Neisseria gonorrhoeae (N. gonorrhoeae) infection. It presents with localized inflammatory symptoms, including joint and tendon sheath involvement, and requires early recognition to prevent complications, including abscess formation, tendon rupture, and long-term functional impairment. We report the case of a 40-year-old male who developed progressive swelling, erythema, and pain in his left wrist over 20 days, without significant response to oral anti-inflammatory treatment. Laboratory tests showed marked leukocytosis…

Infectious Extensor Tenosynovitis of the Hallucis Longus Tendon: A Case Report.

Infectious tenosynovitis can involve both flexor and extensor tendons of the extremities. If left untreated, it can lead to high morbidity and mortality. Most emergency providers recognize the signs and symptoms of flexor and extensor tenosynovitis of the hand. However, extensor tenosynovitis of the hallucis longus tendon is a rare condition with a risk of complications similar to infectious tenosynovitis of the hand. This case report describes a presentation of extensor tenosynovitis of the hallucis longus tendon. Clinical suspicion is essential to help the provider not miss this rare conditi…

A Case Report of Surgical Approach in Managing De Quervain's Tenosynovitis.

De Quervain's tenosynovitis is characterized by swelling of muscles (abductor pollicis longus (APL) and extensor pollicis (EPB) brevis), and they are located at the base of the thumb. This is a very irritating and painful condition. In many cases, late detection causes an increase in inflammation, and due to prolonged ignorance and neglect, the patient suffers from pain and discomfort that affects and restricts their daily routine work. The disorder tenosynovitis is triggered by preexisting tendon degeneration induced by excessive twisting actions. Inflammatory arthritis is primarily associate…

Evaluating the Source and Content of Internet Information Regarding DeQuervain's Tenosynovitis.

Introduction Patients are increasingly relying on the internet for information about their medical diagnoses, but there is little regulation on the content of these websites. The purpose of this study was to investigate the quality of the informational content available on the internet regarding DeQuervain's tenosynovitis.  Methods The search phrasing "'DeQuervain' OR 'DeQuervain's' OR 'Radial styloid tenosynovitis' OR 'DeQuervain's synovitis' OR 'Wrist tendonitis'" was entered into the five most commonly used search engines, and the first 50 universal resource locators (URLs) were recorded, i…

Clinical characteristics and treatment outcomes of tuberculous tenosynovitis of the hand and wrist.

Objective: Vietnam is endemic with tuberculosis (TB), which is highly prevalent in the community. TB tenosynovitis of the wrist and hand is uncommon. Because of its insidious progression and atypical presentations, it is often difficult to diagnose, leading to treatment delays. This study investigates the characteristics of clinical and subclinical signs and treatment outcomes of patients with TB tenosynovitis in Vietnam. Patients and Methods: This prospective longitudinal cross-sectional study included 25 TB tenosynovitis patients in the Rheumatology Clinic at University Medical Center Ho Chi…

Importance of tenosynovitis in preventing the progression through rheumatoid arthritis continuum.

Rheumatoid arthritis (RA) has long been characterized by synovitis and bone erosions typically developing symmetrically in small joints. However, recent advances in imaging modalities have indicated frequent association of tenosynovitis with RA, and some consider tenosynovitis to be not just a complication but a major trait of RA. Furthermore, as there are cases with tenosynovitis preceding the clinical detection of inflammatory arthritis in predisposed individuals, tenosynovitis may constitute an important biomarker in defining the pre-RA phase of disease development. Tenosynovitis itself mus…
