Adoption of brain injury guidelines in mild traumatic brain injury: challenges at both referring and receiving trauma centers.

INTRODUCTION: Emerging data suggest Brain Injury Guidelines (BIG) can be used to manage patients with mild to moderate traumatic brain injury (TBI) without neurosurgical consultation. However, many Level 3 trauma centers without neurosurgical specialists continue transferring these patients to higher-level facilities. This study aimed to evaluate the transfer outcomes of TBIs and contribute to the discussion on the feasibility of observing mild TBI patients and evaluate barriers to BIG adoption at both smaller and larger hospitals. METHODS: A retrospective chart review was performed on patien…

Factors associated with survival in adult patients with traumatic arrest: a retrospective cohort study from US trauma centers.

BACKGROUND: Traumatic arrests increasingly affect young adults worldwide with low reported survival rates. This study examines factors associated with survival (to hospital discharge) in traumatic arrests transported to US trauma centers. METHODS: This retrospective cohort study used the US National Trauma Databank 2015 dataset and included patients who presented to trauma centers with "no signs of life". Univariate and bivariate analyses were done. Factors associated with survival were identified using multivariate regression analyses. RESULTS: The study included 5980 patients with traumati…

Effect of the Uniform Accident and Sickness Policy Provision Law on alcohol screening and intervention in trauma centers.

BACKGROUND: Alcohol screening and intervention in trauma centers are widely recommended. The Uniform Accident and Sickness Policy Provision Law (UPPL) exists in most states, and allows insurers to refuse payment for treatment of injuries in patients with a positive alcohol or drug test. This article analyzed the UPPL's impact on screening and reimbursement, measured the knowledge of legislators about substance use problems in trauma centers, and determined their opinions about substance use-related exclusions in insurance contracts for trauma care. METHODS: A nationwide survey of members of t…

Patient-centered Radiology: Implementing a Trauma-informed Care Framework.

The authors present a framework for integrating trauma-informed care principles into radiologic procedures. Trauma-informed care is an approach that acknowledges the widespread impact of psychological trauma, incorporating this understanding into health care practices to prioritize patient safety, trust, and support while minimizing the risk of retraumatization. Given the high prevalence of psychological trauma among patients, radiologic procedures-some of which can be physically and emotionally distressing, such as breast biopsies, hysterosalpingography, and fluoroscopic enemas-must be approa

Blood Product Management: A Comprehensive Analysis in a Trauma Center Hospital.

BACKGROUND: Today, excessive demand for blood products is a common issue across various medical departments. This demand can lead to improper distribution, waste of blood products, and poor availability for emergency patients. Additionally, it increases costs and the workload of blood banks, disrupting the proper distribution of products among medical centers. Ultimately, all these factors contribute to a decrease in the quality of blood products. This study was conducted to investigate the pattern of blood requests and utilization at 5-Azar Educational Hospital in the Golestan province in 202

Evaluation of incidental adrenal masses at a tertiary referral and trauma center.

BACKGROUND: Incidental adrenal masses are those that are found on imaging performed for any nonadrenal evaluation. Published guidelines define accepted follow-up criteria for incidental adrenal masses; however, adherence to these guidelines and barriers to appropriate follow-up are not well understood. We aimed to describe practice patterns for the discovery, evaluation, and follow-up of incidental adrenal masses. METHODS: Medical records of patients with an incidental adrenal mass underwent retrospective review at a tertiary referral and level-1 trauma center, as well as regional ambulatory

Influence of podiatry on orthopedic surgery at a level I trauma center.

Level I trauma centers frequently see trauma at or below the ankle, which requires consultation with the orthopedic surgery department. However, as podiatry programs begin to firmly establish themselves in more Level I trauma centers, their consultations increase, ultimately taking those once seen by orthopedic surgery. A review of the literature demonstrates that this paradigm shift has yet to be discussed. The purpose of this study was to determine how many, if any, lower extremity fracture consultations a newly developed podiatry program would take from the orthopedic surgery department. A …
