Fifteen patients are described who attended out Breast Screening Programme, and were found to have sclerosed fibroadenomas, the imaging of which raised the possibility of carcinoma. In six of these a reflective zone between a mass lesion and distal acoustic shadowing might have been used to infer the benign diagnosis.

Twenty megahertz ultrasound B-scanners achieve an axial resolution of 80 microns. The range of 7 mm in soft tissue suffices for imaging of dermal and cartilaginous structures with anatomical precision, and the form, position and diameter of the cartilaginous structures of the nose can be assessed. Distinct echoes within the cartilage indicate inflammation or trauma. The position, scarring and rate of resorption of implants can be observed non-invasively.

Intussusception is a rare condition in adults. In the large intestine, intussusception is most frequently caused by malignant tumours. The case history of a man aged 35 years is presented here. In connection with operation for colo-colic intussusception, familial polyposis was demonstrated. The diagnosis of colo-colic intussusception was established by ultrasonic scanning.

In 36 subjects with pancreas divisum an ultrasonic examinations of the pancreas were made after filling the stomach with 600 ml of fluid. In 27 subjects (75%) it was possible to evaluate the entire pancreas reliably, incl. the area of the dorsal segment. The gastric filling and positioning of the patients during examination improved in 47% the quality of the visualization, in particular in the left half of the pancreas. Ultrasonically guided percutaneous nephrostomy was planned in 98 patients with a total of 128 nephrostomies. The success rate was over 90%. The catheters were maintained from a 1/2 day to 150 days with a median duration of 13 days. 26% of the patients had the catheter replaced, mainly because of displacement. Two major complications were seen, one perforation of the gallbladder and one case of major haemorrhage resulting in a nephrectomy. Two of the 26 patients infected at the puncture died from sepsis. Ocular cysticercosis, a platyhelminth infection, involves various ocular and adnexal sites. Orbital cysticercosis is a relatively rare site of presentation. We illustrate that this parasitic cyst, when present in the orbit, mimics an orbital pseudotumour. We have used ultrasound as an effective pre-operative diagnostic modality.

The paper is concerned with a study of an ultrasound picture of verified hydatidosis in 62 patients. The symptoms of hydatid cysts are the presence of daughter cysts and "hydatid sand", cuticular membrane desquamation, local cyst wall thickening and parietal layers, a necrotic zone in a preparasitic space and cyst calcification. The detection of these symptoms helps to estimate not only the type of a hydatid cyst but also the state and character of parasite activity permitting a choice of therapeutic tactics.

A prospective study was carried out to evaluate the sensitivity of ultrasonography in diagnosis of peritonsillar abscess (Quinsy). In 1986 through 1989 all cases of doubtful peritonsillitis were subjected to B-mode ultrasonography of tonsils before tonsillectomy was carried out. 36 patients were included in this clinical study. In cases of clinically uncertain peritonsillar abscesses the sensitivity of the method was 82%. However, only four false positive cases ("abscess in the scan but no pus during surgery") occurred.

It can be assumed that among the most frequent pointers to triploidy are the sonographic proof of the existence of a voluminous and vacuolic placenta, early retardation of growth, and the presence of an oligohydramnios. Occasionally, other malformations are also observed, such as hydrocephalus or neural tube defects. Sonographic findings are demonstrated in two cases of triploidy found to exist before the 24 weeks of pregnancy in the living foetus. The clinical signs and patterns are discussed.

The ranking of sonography in the diagnosis of acute appendicitis and its complications was examined in 208 patients in a prospective study. With a prevalence of the disease of 31.25% the sensitivity of sonography was 91.5, and the specificity more than 96% with a low negative laparotomy rate of 12.9%. In a patient with acute cholecystitis whose body habitus precluded cholecystectomy, we used percutaneous transhepatic ultrasonic cholelithotripsy. We compare it with other modalities applicable in such cases. Although not commonly used, these alternatives may be the most appropriate therapy in certain unusual circumstances.

Todays topics on ultrasonic imaging such as ultrasounds of small cancers, noninvasive carcinoma and microcalcifications in breast cancers are discussed. Two cases of early ultrasonic diagnosis of conjoined twins (thoracopagi) in the 13th and 21th week of pregnancy, respectively, are reported. Heart and liver were fused in both and the upper gastrointestinal tract in one of the pairs. Once an ophalocele was seen. The most important ultrasonic criteria were close and fixed position of the twins to each other, ventral fusion of the thoracal and abdominal contours and detection of joint organs. Since hearts and livers were not paired, there was no chance of any successful therapy and on the parent's request, both pregnancies were terminated.

Peyronie disease, or Induratio Penis Plastica, is characterized by the presence of one or more fibrous plaques at the albuginea penis, on the cavernous bodies or on the intercavernous septum. First of all, Induratio Penis Plastica etiology is described, and its clinics and therapy. Past imaging methods are then considered (i.e. conventional radiology, xerography), and current ones (i.e. conventional radiology, cavernosography, CT and US). The authors report on their 4-year (1983-1987) experience with US in 62 males. Various different probes were employed, especially small-part 7.5 MHz probes.

The role of sonographic examination in echinococcal cholangitis is stressed based on a clinical case. Due to perforation of the hepatic hydatid cyst into the bile ducts the clinical picture of acute cholangitis occurred, this being difficult to differentiate from that of choledochal stones. The correct diagnosis was made by sonographic examination and was confirmed intraoperatively. The importance of emergency sonographic examination in severely ill patients is stressed.
