PURPOSE: In the last decade interest has arisen in the use of ultrasound derived measurements of bladder wall thickness, detrusor wall thickness and ultrasound estimated bladder weight as potential diagnostic tools for conditions known to induce detrusor hypertrophy. Between December 1991 and January 1994, 134 patients underwent radical retropubic prostatectomy with bladder neck preservation. Forty-nine patients had a positive margin. Number and location of positive margins were analyzed. A tumor was identified at the bladder neck in 10 cases (7.5%). In all 10 patients with bladder neck involvement, a tumor was also identified at multiple other sites. PURPOSE: We determine whether long-term bladder dysfunction is more frequent in children with posterior urethral valves who undergo early supravesical urinary diversion (pyeloureterostomy) than in those who undergo valve ablation.

We present a case of acute urinary retention (AUR) with hyper-angulation of the urinary bladder neck secondary to uterine leiomyoma. Our patient is a 45-year-old female who presented with AUR and suprapubic pain requiring catheterization. CT images highlight the level of obstruction secondary to suspension of the urinary bladder rather than direct urethral compression. Of 25 upper tract urodynamic studies performed in 22 patients 9 positive results were obtained. The diagnosis of intrinsic ureterovesical junction obstruction was suspected in 11 children but only confirmed in 2. However, in 6 other children changes in bladder volume markedly affected upper tract function. While diuretic renography may confirm obstruction in many doubtful cases, when function is poor and the system is dilated massively a urodynamic study often will be the only definitive diagnostic procedure.
