AI-based modality-agnostic classification system for vascular calcifications.

The importance of vascular calcification in major adverse cardiovascular events such as heart attacks or strokes has been established. However, calcifications have heterogeneous phenotypes, and their influence on diseased tissue stability remains poorly understood. Precise classification of calcification phenotypes is therefore essential for determining their impact on tissue stability through clinical and basic science studies. Here, we introduce a new classification system for phenotyping calcification along with a semi-automatic, non-destructive pipeline that can distinguish these phenotype…

The emerging role of iron in heart failure and vascular calcification in CKD.

Iron deficiency is a frequent comorbidity of cardiovascular (CV) diseases and nearly 50% of patients with heart failure (HF) with or without … rest in the utilization of iron in HF and CKD patients. One of the central players in CV disease is vascular calcification (VC), which has been recognized as a major independent risk factor for incident CV disease and overall mortality in chronic disease patients. In this review, we summarize the evidences generated by clinical trials aimed to study the effect of iron d …

The Role of NLRP3 Inflammasome Activities in Bone Diseases and Vascular Calcification.

Continuous stimulation of inflammation is harmful to tissues of an organism. Inflammatory mediators not only have an effect on metabolic and inflammatory bone diseases but also have an adverse effect on certain genetic and periodontal diseases associated with bone destruction. Inflammatory factors promote vascular calcification in various diseases. Vascular calcification is a pathological process similar to bone development, and vascular diseases play an important role in the loss of bone homeostasis. The NLRP3 inflammasome is an essential component of the natural immune system. It can recogni…

Cell-Matrix Interactions and Matricrine Signaling in the Pathogenesis of Vascular Calcification.

Vascular calcification is a complex pathological process occurring in patients with atherosclerosis, type 2 diabetes, and chronic kidney disease. The extracellular matrix, via matricrine-receptor signaling plays important roles in the pathogenesis of calcification. Calcification is mediated by osteochondrocytic-like cells that arise from transdifferentiating vascular smooth muscle cells. Recent advances in our understanding of the plasticity of vascular smooth muscle cell and other cells of mesenchymal origin have furthered our understanding of how these cells transdifferentiate into osteochon…

[Osteoporosis and vascular calcification in rheumatoid arthritis - the role of osteoprotegerin and sclerostin].

Disorders of bone tissue metabolism and increased frequency of cardiovascular diseases are among the well-known, extra-articular complicatio … cular systems. In the course of ra, lowering of the mineral density of bones and intensification of vascular calcification seem to be associated with the increase of plasma concentration of osteoprotegerin (OPG) and sclerostin - the regulatory proteins of the RANKL/RANK/OPG system and the Wnt/β-catenin pathway. Molecular mechanisms associated with the osteoblasts' activ …

[Vascular calcifications, the hidden side effects of vitamin K antagonists].

Despite the availability of new oral anticoagulants, vitamin K antagonists (VKA, such as fluindione, acenocoumarol or warfarin) remain curre … ase of matrix Gla-protein (MGP), a protein that plays a major inhibitory role in the development of vascular calcifications. Several experimental and epidemiological results suggest that the use of the VKA could promote the development of vascular calcifications increasing thus the cardiovascular risk. This risk seems to be higher in patients with chronic kidney disease …

Association of serum phosphate with vascular and valvular calcification in moderate CKD.

Within the normal range, higher serum phosphate concentrations are associated with cardiovascular events and mortality in individuals with chronic kidney disease (CKD) and in those with normal kidney function. Experimental models suggest that phosphate has a direct calcifying effect on vascular smooth muscle. We examined associations of serum phosphate concentrations with vascular and valvular calcification in 439 participants from the Multi-Ethnic Study of Atherosclerosis who had moderate CKD and no clinical cardiovascular disease. Serum phosphate concentrations were within the normal range (

High-dose calcitriol therapy and progression of cardiac vascular calcifications.

BACKGROUND: Dialysis patients show a very high prevalence of cardiovascular complications, affected as they are with abnormal and accelerated vascular calcifications and, eventually, calcium and phosphorous metabolism disorders. Multislice computed tomography (MSCT) provides a reproducible, high-resolution imaging of calcium contained in cardiac arteries, measured by Agatston score. The aim of the present study was to evaluate the influence of high-dose and low-dose calcitriol therapy on the progression of cardiac vascular calcifications in dialyzed patients. METHODS: We enrolled 36 dialyzed …

Optimising the treatment of hyperphosphatemia and vascular calcification in chronic kidney disease.

Accelerated atherosclerosis and vascular calcifications (VC) play a central role in the pathogenesis of cardiovascular disease in chronic kidney disease (CKD) patients. Mineral metabolism disorders and increased serum calcium-phosphate product have been recently investigated as inducing factors of cardiovascular calcification. In fact, cardiovascular disease in renal failure appears greatly associated with bone metabolism alterations. Recently, the treatment of hyperphosphatemia in CKD patients changed from either calcium- or aluminium-based phosphate-binders to new free-calcium and aluminium …

The role of vitamin D in vascular calcification in chronic kidney disease.

Vascular calcification is a significant cause of morbidity and mortality in patients with chronic kidney disease (CKD). Disorders of mineral metabolism are likely involved in the pathogenesis of vascular calcification. Calcitriol and its analogs are effective in suppressing parathyroid hormone levels in patients with secondary hyperparathyroidism and CKD, but experimental studies demonstrate that these drugs can act directly on vascular smooth muscle cells. In some in vitro studies and in animal models of CKD, calcitriol has induced vascular calcification. Newer analogs of vitamin D appear to …

[Functional disorder of the parathyroid gland and vascular calcification in hemodialysis patients].

In uremia, hyperparathyroidism induces hypercalcemia and hyperphosphatemia. Relative hypoparathyroidism also causes hypercalcemia and hyperphosphatemia due to the low buffering capacity of the bone. Thus control of PTH within target range is very important.

[Disturbances of mineral metabolism and vascular calcifications in dialysis patients (review)].

Vascular calcifications are more frequent in dialysis patients than in the general population or in patients with cardiovascular disease and normal renal function. The reasons for this high incidence are multiple. They include traditional factors such as hypertension, diabetes, dyslipidaemia, and specific factors such as sodium overload, hyperomocysteinaemia, chronic inflammation, oxidative stress as well as disturbance of mineral metabolism. Specifically, hyperphosphataemia and the elevated calcium (Ca) x phosphate product have been associated with an increased risk for development of vascula…

Vericiguat improves CKD-related vascular calcification through the NO/cGMP/PKG pathway.

Chronic kidney disease (CKD) is a condition characterized by progressive loss of kidney function. Vascular calcification is not only a common late-stage complication of CKD but also serves as an independent predictor of cardiovascular events and mortality in CKD patients. This study investigates the mechanism by which vericiguat (VER), a soluble guanylyl cyclase (sGC) sensitizer, attenuates CKD-associated vascular calcification. The hypothesis that vericiguat alleviates the decrease of sGC activity mediated by calcium and phosphorus metabolism was verified in both in vitro and in vivo experime…

Abdominal vascular calcification at CT: opportunistic quantification and imaging-based risk stratification.

Abdominal vascular calcification is increasingly recognized as a clinically relevant marker of systemic atherosclerotic burden and regional vascular disease. Because the abdominal aorta and its major branches are frequently captured on routine abdominal, oncologic, vascular, and preoperative CT examinations, CT provides a practical opportunity for opportunistic assessment of vascular calcium beyond the coronary circulation. This review summarizes current approaches to CT-based evaluation of abdominal vascular calcification, with a focus on abdominal aortic, renal artery, mesenteric and celiac …

Chronic Kidney Disease with Mineral Bone Disorder and Vascular Calcification: An Overview.

Chronic kidney disease (CKD) is a global health issue with a rising prevalence, affecting 697.5 million people worldwide. It imposes a subst … olving dysregulation of bone turnover, mineralization, and strength, accompanied by soft tissue and vascular calcification. Alterations in mineral metabolism, including calcium, phosphate, parathyroid hormone (PTH), vitamin D, fibroblast growth factor-23 (FGF-23), and Klotho, play pivotal roles in CKD-MBD. These disturbances, observed early in CKD, contribute to the pro …

Associated factors of osteoporosis and vascular calcification in patients awaiting kidney transplantation.

INTRODUCTION: Pretransplant osteoporosis and vascular calcification probably increase the risk of fractures and cardiovascular events after kidney transplantation. In the present study, we investigated the related risk factors of osteoporosis and vascular calcification among end-stage renal disease (ESRD) patients awaiting kidney transplantation. METHODS: A total of 221 ESRD patients (age, 43.4 ± 14.3 years; 125 males and 96 females; median dialysis duration, 61.0 m) awaiting kidney transplantation were enrolled in this cross-sectional study. Serum levels of bone turnover markers and intact p…

IL-17A promotes vascular calcification in an ex vivo murine aorta culture.

BACKGROUND: Vascular calcification is characterized by mineral deposition in the vasculature, which is triggered by chronic systemic inflammation, including psoriasis. Psoriasis is an IL-17A-mediated inflammatory skin disease that is associated with exacerbated vascular calcification and high cardiovascular mortality. Although previous studies have shown that IL-17A induces vascular dysfunction in murine psoriasis models, it has not been clarified whether IL-17A induces vascular calcification. In this study, we investigated the potential vascular calcification-inducing effect of IL-17A in an e…

Mechanistic insights into CKD-MBD-related vascular calcification and its clinical implications.

Chronic Kidney Disease (CKD) is linked to a rising prevalence of morbidity and mortality primarily from cardiovascular complications and is considered a high or very high-risk associated cardiovascular state. These poor outcomes are associated with CKD-mineral and bone disorders (CKD-MBD) in CKD patients, which are manifested by vascular calcification. Patients with chronic renal illness have not only accelerated intimal and medial calcification, but heart valves, and possibly the myocardium get also calcified, as well as the uncommon state of calcific uremic arteriolopathy (calciphylaxis). So…

Circulating uromodulin inhibits vascular calcification by interfering with pro-inflammatory cytokine signalling.

AIMS: Uromodulin is produced exclusively in the kidney and secreted into both urine and blood. Serum levels of uromodulin are correlated with kidney function and reduced in chronic kidney disease (CKD) patients, but physiological functions of serum uromodulin are still elusive. This study investigated the role of uromodulin in medial vascular calcification, a key factor associated with cardiovascular events and mortality in CKD patients. METHODS AND RESULTS: Experiments were performed in primary human (HAoSMCs) and mouse (MOVAS) aortic smooth muscle cells, cholecalciferol overload and subtota…

Uremic Vascular Calcification: The Pathogenic Roles and Gastrointestinal Decontamination of Uremic Toxins.

Uremic vascular calcification (VC) commonly occurs during advanced chronic kidney disease (CKD) and significantly increases cardiovascular morbidity and mortality. Uremic toxins are integral within VC pathogenesis, as they exhibit adverse vascular influences ranging from atherosclerosis, vascular inflammation, to VC. Experimental removal of these toxins, including small molecular (phosphate, trimethylamine-N-oxide), large molecular (fibroblast growth factor-23, cytokines), and protein-bound ones (indoxyl sulfate, p-cresyl sulfate), ameliorates VC. As most uremic toxins share a gut origin, inte…
