Access site vascular complications remain relatively frequent following trans-femoral (TF) transcatheter aortic valve implantation (TAVI), and are associated with significant morbidity as well as increased mortality. Suture-based vascular closure devices (VCD) are widely used and have been demonstrated to reduce the rate of vascular complications. However, failure to achieve adequate hemostasis following their use occurs in some cases, and may necessitate surgical escalation. We report a case of a patient with complex ileo-femoral anatomy in whom the novel plug-based MANTA VCD was successfully…

We present the case of a 6-year old boy with a mid-muscular residual ventricular septal defect (VSD) of difficult surgical access, who underwent transcatheter closure using the Amplatzer(TM) VSD occluder. Transcatheter closure was guided by transthoracic echocardiography and successful closure was achieved with a 12 mm diameter VSD occluder, with no complications. Chest radiography showed a considerable decrease in cardiomegaly and normalization of pulmonary vascular markings before 24 hours, and echocardiography showed correct positioning of the Amplatzer(TM) VSD device without residual shunt

BACKGROUND: Transcatheter aortic valve implantation (TAVI) procedures have revolutionized the treatment of aortic stenosis. However, due to large sheaths, improperly deployed closure devices, and the comorbidities and challenges innate to this population, vascular access complications can be devastating. The objective of this study is to evaluate vascular access complications in one of the largest TAVI sites in North America.
METHODS: This was a retrospective single center review between January 2014 and December 2018 of vascular access complications necessitating operative intervention by va

Transcatheter closure of perimembranous ventricular septal defect is an alternative to surgery in selected small to medium defects with a reasonable distance between the defect and the aortic or tricuspid valves. Surgical closure is generally preferred for large defects with an inlet to outlet extension, aortic malalignment, or deficient aortic rim. We report two successful cases of percutaneous closure of large perimembranous ventricular septal defects with inlet to outlet extension using an Amplatzer Vascular Plug-II via a retrograde approach. Transcatheter closure of large perimembranous ve

Suture-based vascular closure devices have been shown to be effective in hemostasis for procedures with vascular access. However, iatrogenic vascular occlusion may occur. The cutting balloon (CB) is a noncompliant balloon wrapped with 3-4 microsurgical blades that are intended to modify vascular lesions, but it may also be utilized to cut and release endovascular sutures. We report two cases in which the CB was employed as a bailout strategy to alleviate suture-related vascular occlusion after transcatheter aortic valve replacement. The CB can be effectively utilized to resolve suture-related …

Transcatheter closure of the fossa ovalis atrial septal defect with adequate rims has become the procedure of choice in recent times. We report our experience with 439 patients who were selected for percutaneous device closure and 430 of whom underwent transcatheter closure of fossa ovalis atrial septal defects from May 1997 to May 2006. Device closure was not attempted in the remaining nine patients after initial evaluation due to high right atrial pressure following balloon occlusion (n=1 not parallel, high pulmonary vascular resistance (n=1), significant coronary artery disease (n=1), or th

BACKGROUND: Orienting the left atrial disc of an atrial septal defect (ASD) closure device parallel to the septum is important, particularly in cases of ASD with deficient retroaortic rim. Standard vascular sheaths can make this challenging. We report the use of an electrophysiologic sheath, the SL2™ St. Jude Vascular sheath (St. Jude Medical, St. Paul, MN) for closure of ASD.
METHODS: All patients undergoing ASD closure between July 2010 and February 2011 at The Children's Hospital at Montefiore in which the SL2 sheath was used were reviewed.
RESULTS: Nine patients underwent transcatheter A

BACKGROUND: Transcatheter closure of atrial septal defects (ASDs) has been feasible and successful. Two-dimensional echocardiography (2DE) was applied to patients before selection and during device deployment. Three-dimensional echocardiography (3DE) can provide unique anatomic perspectives that might aid in improving device closure of ASDs.
METHODS AND RESULTS: Twenty-two consecutive patients were enrolled in an initial protocol for ASD device closure by the new DAS-Angel Wings occluder device. On the basis of transesophageal (TEE) 2DE and 3DE, 13 patients were considered eligible for device

BACKGROUND: Use of percutaneous mechanical circulatory support has grown exponentially. Vascular complications remain a growing concern and best practices for device removal do not exist. We describe a novel post-closure technique for the next generation Impella CP removal and immediate hemostasis.
METHODS: This study is a single center, retrospective, exploratory analysis of 11 consecutive patients receiving an Impella CP for either high-risk PCI or cardiogenic shock and then referred for post-closure compared to 20 patients receiving manual compression for Impella CP removal between 2017 an

OBJECTIVE: Percutaneous access for endovascular aortic aneurysm repair (P-EVAR) is less invasive compared with surgical access for endovascular aortic aneurysm repair (S-EVAR). P-EVAR has been associated with shorter recovery and fewer wound complications. However, vascular closure devices (VCDs) are costly, and the economic effects of P-EVAR have important implications for resource allocation. The objective of our study was to estimate the differences in the costs between P-EVAR and S-EVAR.
METHODS: We used a decision tree to analyze the costs from a payer perspective throughout the course o…

BACKGROUND: With the advent of advanced structural procedures and circulatory devices, large-bore access has become increasingly common. However, vascular access site complications present serious prognostic risks. While suture-based and collagen-based devices have been described for vascular closure, these techniques require maintenance of wire access.
AIMS AND METHODS: We present a novel technique for achieving hemostasis once wire access has been lost and the perclose (standard suture-based device) has been deployed, utilizing a collagen plug; Tamponade On Perclose; Hemostasis Augmentation

Descending aorta obstruction is a known though uncommon event that can occur post patent ductus arteriosus device closure in premature infants. This adverse event has generally been managed with surgical repair or stent placement if the child is large enough for the vascular access needed. We present two cases of late aortic obstruction associated with device closure of a patent ductus arteriosus managed with the Renata Minima stent. Key Words: PDA, Coarctation, Renata Minima Stent.

OBJECTIVE: Femoral access site complications influence short term survival and outcomes in patients undergoing endovascular procedures. While ultrasound guided puncture is a reliable method to reduce such complications, ultrasound guidance is rarely used for closure device deployment.
DATA SOURCES: Web of Science, PubMed, and the Cochrane Library.
REVIEW METHODS: A systematic literature search was conducted to assess the safety and efficacy of ultrasound guided vascular closure device deployment compared with vascular closure device deployment without ultrasound guidance, referred to as conv

BACKGROUND: Electrophysiology (EP) procedures increasingly require large-bore venous access (LBVA), which is associated with a higher risk of vascular complications. While vascular closure devices (VCDs) are well established for arterial access, data on venous closure-particularly for LBVA-remain limited. The LockeT device is a suture-mediated venous closure system designed to achieve rapid hemostasis without intravascular components, but its performance in LBVA has not been well characterized.
METHODS: We conducted a single-center, retrospective observational study of patients undergoing EP …

Earlier attempts at percutaneous closure of perimembranous ventricular septal defects (Pm VSDs) were abandoned because of incidence of heart block likely as a result of device rigidity and/or oversizing. This is retrospective review and data reporting of patients who underwent percutaneous closure using the softer second-generation Amplatzer vascular occluders; namely the Amplatzer vascular plug, second generation, (AVP II) and the Amplatzer duct occluder, second generation (ADO II) in our institution. A total of 20 patients were identified; AVP II was used in 9 patients and ADO II in 11 patie
