Management of Ruptured and Rapidly Progressive Mycotic Cerebral Aneurysms in the Setting of Unilateral Carotid Occlusion and Endocarditis with Valve Failure.

Introduction Congenital intracranial tumors are rare and often incidentally diagnosed on routine ultrasound. We report a case of a fetal demise with a massive intracranial teratoma at 25 weeks of gestation and the management of her delivery in the setting of macrocephaly, breech presentation, and polyhydramnios. Case A 31-year-old G3P1011 woman at 25 weeks' gestation presented with a recent fetal demise and a fetal intracranial tumor first identified at 16 weeks' gestational age. The patient had declined termination of pregnancy. Biometry was consistent with 24 weeks' gestation, except for a h

Effect of tocolytic drugs on fetal heart rate variability: a systematic review.

INTRODUCTION: Tocolytics may cause changes in fetal heart rate (HR) pattern, while fetal heart rate variability (HRV) is an important marker of fetal well-being. We aim to systematically review the literature on how tocolytic drugs affect fetal HRV. MATERIALS AND METHODS: We searched CENTRAL, PubMed and EMBASE up to June 2016. Studies published in English, using computerized or visual analysis to describe the effect of tocolytics on HRV in human fetuses were included. Studies describing tocolytics during labor, external cephalic version, pre-eclampsia and infection were excluded. Eventually,

Value and pregnancy outcome of external cephalic version.

To determine the efficacy and pregnancy outcome of external cephalic version at > or = 37 weeks gestation, 90 women with a singleton breech fetus in King Hussein Medical Centre who had the procedure were compared with 102 women in a control group in whom the procedure was not attempted. External cephalic version was successfully performed on 59 fetuses (64%). Caesarean section was performed on 39% of patients who underwent the procedure compared to 61% of the control group. Version was more successful in multiparous than nulliparous women. External cephalic version was effective in reducing th

Uterotonics and tocolytics for anesthesiologists.

PURPOSE OF REVIEW: Obstetric anesthesiologists are supposed to understand the uterotonics and tocolytics used in the perinatal period to provide a better clinical practice. This review describes current consensus of uterotonics and tocolytics used in the perinatal period that an obstetric anesthesiologist should know. RECENT FINDINGS: Rational use of uterotonics for cesarean section has been well studied in the past decades. Oxytocin remained as a first line uterotonics for cesarean section. For continuous infusion, it is reported that ED90 is higher for laboring parturients than for nonlabor

Determinants of pain perception after external cephalic version in pregnant women.

OBJECTIVE: A considerable proportion of pregnant women with a fetus in breech position refuses external cephalic version (ECV), with fear of pain as important barrier. As a consequence, they are at high risk for caesarean section at term. The current study investigated determinants of pain perception during ECV, with special attention to maternal mental state such as depression and fear of ECV. DESIGN: Prospective study of 249 third-trimester pregnant women with breech position with a request for an ECV attempt. SETTING: Department of Obstetrics and Gynaecology in a large teaching hospital i

Maternal thyroid function and the outcome of external cephalic version: a prospective cohort study.

BACKGROUND: To investigate the relation between maternal thyroid function and the outcome of external cephalic version (ECV) in breech presentation. METHODS: Prospective cohort study in 141 women (≥ 35 weeks gestation) with a singleton fetus in breech. Blood samples for assessing thyroid function were taken prior to ECV. Main outcome measure was the relation between maternal thyroid function and ECV outcome indicated by post ECV ultrasound. RESULTS: ECV success rate was 77/141 (55%), 41/48 (85%) in multipara and 36/93 (39%) in primipara. Women with a failed ECV attempt had significantly high

Obstetric maneuvers for shoulder dystocia and associated fetal morbidity.

OBJECTIVE: We sought to determine the fetal injury rate associated with shoulder dystocia and to determine whether there is a higher rate of brachial plexus injury or bone fracture when fetal manipulation techniques are required for delivery. STUDY DESIGN: A retrospective review of 285 cases of shoulder dystocia that occurred between January 1991 and December 1995 was performed. The type, sequence, and combination of obstetric maneuvers used to relieve the shoulder dystocia were noted. These cases were divided into two groups, as follows: (1) those resolved with McRoberts' maneuver, suprapubi

Singleton breach presentation planned for vaginal delivery.

A retrospective study of 156 consecutive cases of singleton breach presentation planned for vaginal delivery is presented. The incidence of caesarean section was not increased when compared with that for all Hospital and Clinic confinements. The perinatal mortality and prematurity rates were high. The incidence of cord prolapse was increased especially with the footling breech. The need for an experienced Obstetrician and Anaesthetist to conduct all deliveries was demonstrated. Factors influencing the assessment of feto-pelvic proportion and the occurrence of trauma and asphyxia in the managem

Obstetrical and neonatal outcomes after successful external cephalic version relative to those after spontaneous cephalic presentations.

OBJECTIVE: External cephalic version (ECV) is a procedure during which the fetus is rotated from breech or transversal to cephalic presentation. Our aim was to assess the outcomes of successful ECV in terms of obstetrical and immediate neonatal outcomes relative spontaneous cephalic presentation. METHODS: We performed a retrospective single-center observational study from January 2007 to December 2017. All included participants benefited from trial of labor with delivery of the fetus in cephalic presentation. They were divided into two groups depending on whether an ECV had been successfully

Value of routine ultrasound examination at 35-37 weeks' gestation in diagnosis of non-cephalic presentation.

OBJECTIVE: Undiagnosed non-cephalic presentation in labor carries increased risks for both the mother and baby. Routine pregnancy care based on maternal abdominal palpation fails to detect the majority of cases of non-cephalic presentation. The aim of this study was to report the incidence of non-cephalic presentation at a routine scan at 35 + 0 to 36 + 6 weeks' gestation and the subsequent management of such pregnancies. METHODS: This was a retrospective analysis of prospectively collected data in 45 847 singleton pregnancies that had undergone routine ultrasound examination at 35 + 0 to 36

A simple patient-initiated intervention to increase antenatal detection of breech presentation at term.

Antenatal detection of breech presentations at 35-37 weeks is necessary to allow adequate time for decision making about external cephalic version (ECV) and/or caesarean section. This study aimed to increase antenatal detection of breech presentation and referral for ECV using an inexpensive patient prompt (a simple brochure encouraging pregnant women to ask how their baby is presenting) and posters reminding clinicians to assess presentation. The interventions were evaluated using a before-after (single time series) study design. The records of women who had a breech presentation in late preg

[External version from breech to vertex presentation (author's transl)].

An analysis of 51 attempts at version was made of which 27 were successful (53%). In 3 cases an emergency cesarean section had to be performed following the attempt at version. In 2 of these cases there was partial premature detachment of the placenta. Attempts at version were begun in the 36th week of pregnancy. Up to the 38th week there was a 67% success rate, while from the 39th week onward it was only 41%. In cases of parity it can be shown clearly that the success rate increases with parity. When the pregnancy is the mother's first, 58% of attempts fail, while in fourth pregnancies this f
