Case report: Bitter vertigo.

BACKGROUND: There are many causes of episodes of vertigo and very few causes of episodes of changes in taste, and the combination of the two is very rare. Here, we describe a patient with recurrent short episodes of vertigo in combination with simultaneous episodes of recurrent paroxysmal dysgeusia and altered feeling on the left side of face. The symptoms were caused by compression of the vestibulocochlear nerve and the facial nerve due to dolichoectasia of the basilar artery. METHODS: The patient was diagnosed in our routine clinical practice and underwent a complete neurological and neuro-…

Vertigo Induced During Coitus.

Objectives: The aim of this study was to describe the clinical features of vertigo developed during sexual intercourse. Methods: We retrospectively reviewed the clinical and laboratory findings of seven patients who reported recurrent vertigo during sexual intercourse. Results: All the patients reported spinning sensation for a few minutes to 1 h, which developed during the coitus. Most patients (6/7, 86%) reported associated auditory symptoms including tinnitus (n = 4), ear fullness (n = 2), autophony (n = 1), hearing impairment (n = 1), or hyperacusis (n = 1). Four patients reported the vert…

[Benign paroxysmal positional vertigo].

Benign paroxysmal positional vertigo (BPPV) is the most common cause of vestibular vertigo. It is caused by dislodged otoconia that enters the semicircular canals. Its main symptom is relapsing positional vertigo. The provocative tests, in particular the Dix-Hallpike maneuver, are used to confirm the diagnosis. In case of positive result of the test, one of the maneuvers is peformed depending on the affected semicircular canal, for instance the Epley maneuver. It is important to differentiate BPPV from the vertigo of central origin.

[Vertigo: patient management tactics].

To avoid serious causes of dizziness and establishing preliminary diagnosis is usually enough sighting history taking and physical examination of the patient. Great importance is self-presenting patients with complaints of dizziness. In recent years, for treatment of various types vestybulopatiy, but also for the prevention of attacks, successfully used betahistyn hydrochloride, which improves the quality of care and well-being of the patient.

Malignant paroxysmal positional vertigo.

OBJECTIVE: An insidious percentage of paroxysmal positional vertigo appears to be intractable with canalith repositioning maneuver and also is not self-limiting. This type of positional vertigo is sustained by the action of intracranial tumors that mimics the clinical aspects of benign paroxysmal positional vertigo.Aim of this study is to clarify the features of these forms of positional vertigo, which we indicate as malignant paroxysmal positional vertigo. METHODS: We retrospectively reviewed the clinical records of all the patients who presented with vertigo spells and were managed at our t…

[Benign paroxysmal positional vertigo].

Benign paroxysmal positional vertigo is a common malfunction of the labyrinth, most frequently of the posterior canal, due to clot of debris in the endolymph. The attacks of vertigo are characterised by dependency on position, latency after positioning of the head, short duration, nystagmus, reversibility and fatiguableness. The diagnosis can be based on history, physical examination and provocative tilting of the head as described by Dix and Hallpike. In a typical case further investigations are not necessary. The symptoms often improve spontaneously. Half of the patients can be cured directl…

Benign paroxysmal positional vertigo.

Benign paroxysmal positional vertigo (BPPV) is a peripheral vestibular disorder affecting the posterior semicircular canal. It is most often idiopathic in nature, though it has been known to occur in patients following head injury, Meniere's disease, acoustic neuroma and viral neuronitis. Two basic theories of the pathology of BPPV include cupulolithiasis and canalithiasis. Diagnosis is made by instituting the Hallpike-Dix maneuver to elicit the positional vertigo and nystagmus. While many patients suffer from BPPV on a limited basis, treatment options are available for those individuals with …

[Non-organically-induced vertigo].

The symptom "vertigo" we find as a sign of many different diseases. A psychogenic cause must be considered, if organic failures are excluded by a complete examination. Such troubles we see as symptoms of endogenous psychosis and anxiety neurosis, especially aerophobia like agoraphobia, gephyrophia, and batophobia. In these cases dizziness appears as a covered form of anxiety.

The Enduring Controversy of Cervicogenic Vertigo, and Its Place among Positional Vertigo Syndromes.

The idea of cervicogenic vertigo (CV) was proposed nearly a century ago, yet despite considerable scrutiny and research, little progress has been made in clarifying the underlying mechanism of the disease, developing a confirmatory diagnostic test, or devising an appropriately targeted treatment. Given the history of this idea, we offer a review geared towards understanding why so many attempts at clarifying it have failed, with specific comments regarding how CV fits into the broader landscape of positional vertigo syndromes, what a successful diagnostic test might require, and some practical…

Psychological distress longitudinally mediates the effect of vertigo symptoms on vertigo-related handicap.

OBJECTIVE: Vertigo symptoms can lead to more or less vertigo-related handicap. This longitudinal study investigated whether depression, anxiety, and/or somatization mediate the relationship between vertigo symptoms and vertigo-related handicap. METHODS: N=111 patients with vertigo/dizziness provided complete data on the following measures: Vertigo symptoms at baseline, depression at 6-month follow-up, anxiety at 6-month follow-up, somatization at 6-month follow-up, and vertigo handicap at 12-month follow-up. Mediation analyses with bootstrapping were performed to investigate the mediating rol…

Vertigo and dizziness from environmental motion: visual vertigo, motion sickness, and drivers' disorientation.

The normal vestibular system may be adversely affected by environmental challenges which have characteristics that are unfamiliar or ambiguous in the patterns of sensory stimulation they provide. A disordered vestibular system lends susceptibility even to quotidian environmental experiences as the sufferer becomes dependent on potentially misleading, nonvestibular sensory stimuli. In both cases, the sequelae may be vertigo, incoordination, imbalance, and unpleasant autonomic responses. Common environmental motion conditions include visual vertigo, motion sickness, and motorists' disorientation…

Rotational vertigo caused by cerebral lesions: vertigo and areas 3av, 2v, and 7.

We describe two patients complaining of vertigo associated with a small supra-tentorial convexity meningioma. Symptoms disappeared after tumor removal, providing evidence for an association between the vertigo and the cerebral cortical lesions. Tumors were located in the central and parietal areas, respectively, which are probably analogous to the vestibular cortex in the areas designated 3av, 2v, and 7 in animal studies.

A newly recognised cause of vertigo: horizontal canal variant of benign positional vertigo.

AIMS: To describe the presentation, causes, treatment, and outcomes of patients presenting with the newly recognised horizontal canal variant of benign positional vertigo (BPV); and to emphasise the importance of performing a positional test on all patients being assessed for vertigo. METHODS: The records on 400 patients presenting with BPV were analysed. Two detailed patient histories from 2004 are included. RESULTS: Forty-nine patients (12%) had horizontal canal BPV. The median presentation age was 59 years. In 17 patients, it presented de novo during repositioning treatment for 'classical…

[Vertigo and fluctuating hearing-loss. Differential diagnosis: Ménière's disease, migraine, and psychogenic vertigo].

We present the case of a female patient suffering from recurrent vertigo and low-frequency hearing loss who was admitted for inpatient treatment with the diagnosis of Ménière's disease. After evaluation of all diagnostic examinations, including psychosomatic evaluation, a diagnosis of vestibular migraine with accompanying psychogenic vertigo could be confirmed and was treated accordingly. Neurotologic findings and the corresponding literature are reported.

The efficacy of piracetam in vertigo. A double-blind study in patients with vertigo of central origin.

In a double-blind trial according to a switchback design with 4 periods of one week each a comparison was made between the effects of piracetam and a placebo. In 22 patients with vertigo of central origin (posttraumatic, psychogenic, ecileptogenic and hypertensive vertigo were excluded) piracetam was found to significantly reduce symptoms. On anamnestic examination the patients noted the effect of both substances on vertigo, motility disturbances, vitality and sleep. Piracetam was found to have a significant effect on the first three. The effect of piracetam is explained by an enhanced control…

Cerebellar infarction presenting with isolated positional vertigo: differentiating factors for benign paroxysmal positional vertigo.

BACKGROUND: Isolated central positional vertigo (CPV) due to cerebellar infarction is often difficult to differentiate from benign paroxysmal positional vertigo (BPPV). Here, we aimed to evaluate whether vascular risk factors and serum vitamin D level can differentiate between positional vertigo types. METHODS: A total of 78 consecutive patients were consecutively enrolled from January 2017. All CPV patients had a National Institutes of Health Stroke Scale score of 0 and cerebellar infarctions confirmed by brain MR imaging. Vascular risk factors and serum 25-hydroxyvitamin D levels were compa…

[Benign paroxysmal vertigo of childhood: categorization and comparison with benign positional paroxysmal vertigo in adult].

INTRODUCTION: The differential diagnosis of vertigo in children is extensive. This implies an additional difficulty in diagnosing dizziness in paediatric population. PATIENTS AND METHOD: Twenty-three children consecutively examined for paroxysmal attacks of dizziness and/or vertigo attacks entered our study, and were compared to a 15 adults group with benign positional paroxysmal vertigo. Fifteen healthy paediatric subjects and 18 adults were selected as control groups. The clinical characteristics of vertigo, presence of triggering factors, family history of migraine, presence of motion sick…
