Short Report: Divergent Sleep-Wake Regularity in U.S. Children With Autism Spectrum Disorder: Bedtime vs Wake-up Regularity.

Sleep-wake regularity is an emerging dimension of sleep health, yet its association with autism spectrum disorder (ASD) remains understudied in nationally representative samples, particularly across distinct components of sleep timing. Using parent-reported data from 11,462 U.S. children aged 5-17 years in the 2022 and 2024 National Health Interview Survey, this study examined associations between ASD diagnosis and irregular bedtime and wake-up timing, including differences by age group. Multivariable logistic regression models were adjusted for sociodemographic characteristics, family context

Estimating the severity of obstructive sleep apnea during wakefulness using speech: A review.

Obstructive sleep apnea (OSA) is a chronic breathing disorder during sleep that affects 10-30% of adults in North America. The gold standard for diagnosing OSA is polysomnography (PSG). However, PSG has several drawbacks, for example, it is a cumbersome and expensive procedure, which can be quite inconvenient for patients. Additionally, patients often have to endure long waitlists before they can undergo PSG. As a result, other alternatives for screening OSA have gained attention. Speech, as an accessible modality, is generated by variations in the pharyngeal airway, vocal tract, and soft tiss

Methodology and theoretical basis of forward genetic screening for sleep/wakefulness in mice.

The regulatory network of genes and molecules in sleep/wakefulness remains to be elucidated. Here we describe the methodology and workflow of the dominant screening of randomly mutagenized mice and discuss theoretical basis of forward genetics research for sleep in mice. Our high-throughput screening employs electroencephalogram (EEG) and electromyogram (EMG) to stage vigilance states into a wake, rapid eye movement sleep (REMS) and non-REM sleep (NREMS). Based on their near-identical sleep/wake behavior, C57BL/6J (B6J) and C57BL/6N (B6N) are chosen as mutagenized and counter strains, respecti…

EEG Functional Connectivity Prior to Sleepwalking: Evidence of Interplay Between Sleep and Wakefulness.

STUDY OBJECTIVES: Although sleepwalking (somnambulism) affects up to 4% of adults, its pathophysiology remains poorly understood. Sleepwalking can be preceded by fluctuations in slow-wave sleep EEG signals, but the significance of these pre-episode changes remains unknown and methods based on EEG functional connectivity have yet to be used to better comprehend the disorder. METHODS: We investigated the sleep EEG of 27 adult sleepwalkers (mean age: 29 ± 7.6 years) who experienced a somnambulistic episode during slow-wave sleep. The 20-second segment of sleep EEG immediately preceding each pati

Cortical Drive to Breathe during Wakefulness in Patients with Obstructive Sleep Apnea Syndrome.

STUDY OBJECTIVES: The obstructive sleep apnea syndrome (OSAS) involves recurrent sleep-related upper airways (UA) collapse. UA mechanical properties and neural control are altered, imposing a mechanical load on inspiration. UA collapse does not occur during wakefulness, hence arousal-dependent compensation. Experimental inspiratory loading in normal subjects elicits respiratory-related cortical activity. The objective of this study was to test whether awake OSAS patients would exhibit a similar cortical activity. DESIGN: Descriptive physiology study. SETTING: Sleep laboratory in a large univ…

Statistical analysis of tracheal breath sounds during wakefulness for screening obstructive sleep apnea.

Obstructive sleep apnea (OSA) is a prevalent disorder. The accepted method of diagnosis in widespread clinical practice, polysomnography (PSG), is costly and very time consuming; therefore, quick screening methods, especially when there is a need for quick diagnosis, is of great interest. Diagnostic methods which exploit subtle differences in breath sounds recorded during wakefulness, such as our group's Awake-OSA technology, have shown their capability to diagnose OSA at the research stage. Simplifying the breath sound recording procedure employed in the Awake-OSA diagnostic method would incr…

From wakefulness to excessive sleepiness: what we know and still need to know.

The epidemiological study of hypersomnia symptoms is still in its infancy; most epidemiological surveys on this topic were published in the last decade. More than two dozen representative community studies can be found. These studies assessed two aspects of hypersomnia: excessive quantity of sleep and sleep propensity during wakefulness excessive daytime sleepiness. The prevalence of excessive quantity of sleep when referring to the subjective evaluation of sleep duration is around 4% of the population. Excessive daytime sleepiness has been mostly investigated in terms of frequency or severity…

Unilateral ablation of pre-Botzinger complex disrupts breathing during sleep but not wakefulness.

RATIONALE: In adult rats, bilateral ablation of pre-Bötzinger complex (preBötC) neurokinin 1-expressing (NK1R) neurons leads to a progressive and irreversible disruption in breathing pattern, initially during sleep, eventually resulting in an ataxic breathing pattern during wakefulness. OBJECTIVES: Here we determine whether ablation of fewer preBötC NK1R neurons leads to a persistent pattern of disordered breathing during sleep but not during wakefulness. METHODS: Adult male Sprague-Dawley rats (n = 12) were instrumented to record diaphragmatic, abdominal, and neck EMG, and EEG. Fourteen day…

Individual differences in adult human sleep and wakefulness: Leitmotif for a research agenda.

This paper reviews the literature on interindividual variability in human sleep parameters, sleepiness, responses to sleep deprivation, and … s into sleep/wake regulation and sleep pathology. Understanding individual differences in sleep and wakefulness may also have provocative but important implications for health economics and clinical care, as well as for safety, productivity, and general well-being. This paper gives suggestions for a research agenda focusing on individual differences in sleep …

[Chaotic component of human high-frequency EEG in the state of quiet wakefulness].

Chaotic component of human EEG oscillations in the high-frequency band (14.7-100 Hz) was investigated. EEG was recorded from four points in symmetrical frontal and occipital scalp areas. The results of the non-linear analysis of the high-frequency EEG indicate the existence of the deterministic chaotic component with a high attractor correlation dimension. It was significantly different from the respective values of the Gaussian noise filtered in the same frequency band. In the state of quiet wakefulness (eyes closed), the dimensions of chaotic components of the EEG in all derivations did not …

Patterns of neuronal firing in the human lateral thalamus during sleep and wakefulness.

The firing patterns of thalamic neurons in mammals undergo a dramatic change as the animal's state changes between sleep and wakefulness. During sleep the normal tonic firing of thalamic neurons changes into a slower bursting mode characterized by repetitive activation of a low-threshold calcium (Ca2+) current. The present report describes the patterns of thalamic neuronal firing during sleep and wakefulness in one human patient. Extracellular single neuron activity was recorded during functional stereotactic surgery in the thalamus of a patient with chronic pain, who was observed to fall asle…

Ventilatory effects of specific carotid body hypocapnia in dogs during wakefulness and sleep.

We used extracorporeal perfusion of the vascularly isolated carotid sinus region to determine the effects of specific carotid body chemoreceptor hypocapnia-alkalosis on ventilatory control in the unanesthetized dog. Eight female dogs were studied during wakefulness, non-rapid-eye-movement (NREM) sleep, and rapid eye movement (REM) sleep. Carotid body perfusions lasted from 1 to 2 min, and each trial was preceded by a 1-min control period. Two levels of carotid body hypocapnia were employed, approximately 7 and 14 Torr below eupneic levels in a given dog. We found that 1) During wakefulness and…

[Central sleep apnea syndrome as a cause of impaired wakefulness in multiple myeloma].

Progressive day-time sleepiness developed in a 73-year-old man for 3 years known to have kappa-light-chain myeloma, treated with radio- and chemotherapy. His powers of concentration and intellectual performance were diminished. Neither clinical nor biochemically was there any indication of abnormal water and electrolyte metabolism or hyperviscosity syndrome. The neurological examination was unremarkable. His wife's observation of nocturnal breathing pauses suggested sleep-related abnormal breathing. Polysomnography showed severe central sleep apnoea: an apnoea index of 60/h and blood oxygen sa

The effect of lung inflation on breathing in man during wakefulness and sleep.

To determine whether the Hering-Breuer inflation reflex could be demonstrated in the presence and absence of behavioral influences on breathing, lung inflation was performed via the tracheal stoma in eleven laryngectomized subjects: seven seated at rest with their eyes closed and six recumbent during their deepest non-rapid eye movement sleep. Laryngectomized subjects were chosen for study since their permanent tracheal stoma and absence of a glottis abolished the sensation of inflation in the upper airways, avoided problems with glottic closure and allowed simple, airtight connection to respi
