Treatment of hallux rigidus (HARD trial): study protocol of a prospective, randomised, controlled trial of arthrodesis versus watchful waiting in the treatment of a painful osteoarthritic first metatarsophalangeal joint.

INTRODUCTION: Hallux rigidus is a common problem of pain and stiffness of the first metatarsophalangeal joint (MTPJ) caused mainly by degenerative osteoarthritis. Several operative techniques have been introduced for the treatment of this condition without high-quality evidence comparing surgical to non-surgical care. In this trial, the most common surgical procedure, arthrodesis, will be compared with watchful waiting in the management of hallux rigidus. METHODS AND ANALYSIS: Ninety patients (40 years or older) with symptomatic first MTPJ osteoarthritis will be randomised to arthrodesis or w…

Prognostic role of prostate-specific antigen and prostate volume for the risk of invasive therapy in patients with benign prostatic hyperplasia initially managed with alpha1-blockers and watchful waiting.

OBJECTIVES: To investigate the prognostic role of prostate-specific antigen (PSA) level and prostate volume (PV) for the need for benign prostatic hyperplasia (BPH)-related invasive therapy among patients initially treated with an alpha1-blocker or watchful waiting (WW) in real-life clinical practice. METHODS: Data were collected from 2264 consecutive patients with clinical BPH. Patients initially treated with an alpha1-blocker or WW were included in this study. They were stratified by baseline PSA level (less than 1.5, 1.5 to less than 3.0, 3.0 to 10.0 ng/mL) and PV (less than 30 and 30 to 2…

Watch-and-Wait Policy to Defer Defunctioning Stoma After Low Anterior Resection: Preliminary Results From the Safe Anastomosis Project (KNUCRC-25CP02).

BACKGROUND: Anastomotic leakage remains a severe complication after low anterior resection for rectal cancer, often leading to the routine use of protective stomas, which can cause additional complications. Inspired by the watch-and-wait approach in rectal cancer treatment, we hypothesized that deferring stoma creation could be feasible through the Safe Anastomosis Project, which includes minimizing technical risk factors, enabling early detection of preclinical leakage, and intervening only when necessary. OBJECTIVE: This study aimed to evaluate whether technical modifications, comprehensive

Long-term survival outcomes of 'watch and wait' in patients with bronchus-associated lymphoid tissue lymphoma: a multicenter real-world data analysis in Korea.

Bronchus-associated lymphoid tissue (BALT) is a rare cause of extranodal marginal zone lymphoma (MZL). Although most patients with BALT lymphoma (BALToma) show an indolent clinical course and are monitored without treatment, there are limited real-world data on the long-term outcome of "watch-and-wait' strategy in comparison with other treatments. The survival outcomes of patients newly diagnosed with BALToma at three tertiary hospitals in Korea undergoing two treatment strategies were analyzed: group A, patients who were monitored without any treatment or received only radiotherapy after diag

Comparing local excision with watch and wait for the management of rectal cancer patients responding to neoadjuvant chemoradiotherapy: Composite endpoint analysis using the win ratio.

AIM: The aim of this work was to apply the 'win ratio' to compare the outcomes of rectal-sparing approaches [watch and wait (WW) and local excision (LE)] in the management of locally advanced rectal cancer responding to neoadjuvant chemoradiotherapy. METHOD: Patients enrolled in the ReSARCh study (NCT02710812) between 2016 and 2021 were divided into two cohorts (WW vs. LE). The win ratio was calculated by dividing the number of successes (or wins) in the WW group by the number of successes in the LE group on matched pairs. Oncological outcomes (overall survival, distant and local recurrence),

Long-term anorectal function in rectal cancer patients managed by a watch-and-wait strategy after total neoadjuvant treatment: a cross-sectional study.

BACKGROUND: This study evaluates the long-term anorectal function and rectal toxicity in rectal cancer patients who achieved a clinical complete response (cCR) to total neoadjuvant treatment (TNT) and were managed with a watch-and-wait (W&W) approach. While oncological outcomes have been favorable, functional outcomes warrant further investigation. Additionally, this research identifies clinical risk factors of anorectal dysfunction post-treatment. METHODS: This was a single-center, cross-sectional study. Rectal cancer patients who underwent TNT followed by W&W between December 2014 and Novem

Baseline [18F]FDG PET/CT may predict the outcome of newly diagnosed follicular lymphoma in patients managed with initial "watch-and-wait" approach.

OBJECTIVES: To investigate if baseline [18F]FDG PET/CT can predict the outcome of follicular lymphoma (FL) in patients managed with an initial "watch-and-wait" approach. METHODS: Thirty-eight newly diagnosed FL patients who were managed with an initial "watch-and-wait" approach and undergone baseline [18F]FDG PET/CT were retrospectively enrolled. The standard uptake value (SUV), metabolic tumor volume (MTV), and total lesion glycolysis (TLG) of FL lesions were measured on PET/CT. Patients were followed up for at least 24 months or until initiation of FL therapy. The endpoint was the time to i

AAOS 2024 best paper in the shoulder and elbow classification: watchful waiting provides higher value with similar functional outcomes to physical therapy for frozen shoulder: a prospective randomized controlled trial.

BACKGROUND: Physical therapy remains a ubiquitous treatment modality for the management of frozen shoulder (adhesive capsulitis) despite limited high-level evidence supporting its use. Leveraging patient-reported outcome measures (PROMs) and healthcare costs, this study compared patient outcomes and value achieved through watchful waiting (WW) vs. physical therapy (PT) for conservative management of frozen shoulder. METHODS: Patients in this prospective, parallel randomized controlled trial who were diagnosed with frozen shoulder were randomized 1:1 to WW or PT. All patients were offered ther…

The value of MRI and flexible sigmoidoscopy in detecting tumor regrowth in watch and wait rectal cancer patients undergoing active surveillance after total neoadjuvant therapy.

AIM: Patients with locally advanced rectal cancer may avoid surgery if they develop a clinical complete response (cCR) to total neoadjuvant therapy (TNT). During surveillance, detection of local regrowth can be challenging. The ability of MRI to predict local regrowth is uncertain, especially when it differs from endoscopic findings. This study evaluates the ability of MRI and endoscopy to predict local regrowth in patients under surveillance for cCR after TNT. METHOD: All rectal cancer patients in our institution from 2006 to 2020 who achieved cCR following TNT, and entered watch and wait (W

Helicobacter pylori-positive Colorectal MALT Lymphoma with Repeated Metachronous Recurrence after a Complete Response to Eradication Therapy and Regression after a Watch-and-wait Strategy.

A 77-year-old man underwent a colonoscopy. A submucosal cecal tumor was found, and localized colorectal MALT lymphoma (Lugano stage I) was diagnosed with a Helicobacter pylori infection. The first-line eradication therapy was successful, and the cecal lesion regressed. However, a new lesion was observed in the lower rectum. The rectal lesion regressed after the second-line eradication therapy. Two new lesions were identified in the sigmoid colon. However, no additional treatment was administered and a watch-and-wait strategy was adopted. Both of the lesions regressed. There are no established

The importance of measuring baseline tumour volume (or alternatively tumour length along with its rectal circumferential extent) in the watch-and-wait strategy in rectal cancer: a review.

BACKGROUND: The impact of the tumour volume or size on achieving clinical complete response (cCR) after radio(chemo)therapy is poorly understood. MATERIALS AND METHODS: A literature search was performed to gather data on the predictive value of baseline tumour volume or size in achieving cCR. RESULTS: In total, nine reports were identified. In two of three studies evaluating the baseline tumour volumetry, the tumour volume was the most powerful predictor for cCR. In four of six studies evaluating baseline tumour size without volumetry, tumour dimension was significantly associated with cCR,

Rituximab versus a watch-and-wait approach in patients with advanced-stage, asymptomatic, non-bulky follicular lymphoma: an open-label randomised phase 3 trial.

BACKGROUND: Patients with advanced-stage, low-tumour-burden follicular lymphoma have conventionally undergone watchful waiting until disease progression. We assessed whether rituximab use could delay the need for chemotherapy or radiotherapy compared with watchful waiting and the effect of this strategy on quality of life (QoL). METHODS: Asymptomatic patients (aged ≥18 years) with low-tumour-burden follicular lymphoma (grades 1, 2, and 3a) were randomly assigned centrally (1:1:1), by the minimisation approach stratified by institution, grade, stage, and age, to watchful waiting, rituximab 375…

A Case of Local Resection of Secondary Perianal Paget Disease Appearing during "Watch and Wait" Management of Locally Advanced Rectal Cancer after Preoperative Chemoradiotherapy.

A 78-year-old woman was diagnosed with lower rectal cancer extending to the dentate line. She underwent preoperative chemoradiotherapy, which resulted in tumor shrinkage and clinical complete response (cCR), leading to the implementation of the "Watch and Wait" strategy. At the 21-month surveillance mark, pruritus and erythematous lesions appeared around her anus, which were histologically diagnosed as secondary Paget disease with invasion based on biopsy specimens. Because the primary lesion maintained cCR, local excision of the skin lesion was performed, and negative resection margins were c

Local Regrowth and the Risk of Distant Metastases Among Patients Undergoing Watch-and-Wait for Rectal Cancer: What Is the Best Control Group? Multicenter Retrospective Study.

BACKGROUND: A proportion of rectal cancer patients who achieve a clinical complete response may develop local regrowth. Although salvage appears to provide appropriate local control, the risk of distant metastases is less known. OBJECTIVE: To compare the risk of distant metastases between patients who achieve a clinical complete response (watch-and-wait strategy) and subsequent local regrowth and patients managed by surgery after chemoradiation. DESIGN: Retrospective multicenter cohort study. SETTINGS: This study used data of patients from 3 institutions who were treated between 1993 and 20

Early treatment with FCR versus watch and wait in patients with stage Binet A high-risk chronic lymphocytic leukemia (CLL): a randomized phase 3 trial.

We report a randomized prospective phase 3 study (CLL7), designed to evaluate the efficacy of fludarabine, cyclophosphamide, and rituximab (FCR) in patients with an early-stage high-risk chronic lymphocytic leukemia (CLL). Eight hundred patients with untreated-stage Binet A disease were enrolled as intent-to-treat population and assessed for four prognostic markers: lymphocyte doubling time <12 months, serum thymidine kinase >10 U/L, unmutated IGHV genes, and unfavorable cytogenetics (del(11q)/del(17p)/trisomy 12). Two hundred and one patients with ≥2 risk features were classified as high-risk

Patients with gastric MALT lymphoma revealing persisting endoscopic abnormalities after successful eradication of Helicobacter pylori can be safely managed by a watch-and-wait strategy.

BACKGROUND: In current guidelines, Helicobacter pylori eradication is recommended as first-line therapy in patients with gastric MALT lymphoma. This leads to complete lymphoma regression in the majority of patients. Those who show persisting histological residuals of lymphoma after eradication of the bacterium and normalization of endoscopic findings are managed by a watch-and-wait strategy. We aim to show that such an approach can be extended to patients with persisting endoscopic abnormalities. METHODS: Thirteen patients (7 female and 6 male; 62 years, range: 43 - 80) with gastric MALT lymp

Pathologic Complete Response in Rectal Cancer: Can We Detect It? Lessons Learned From a Proposed Randomized Trial of Watch-and-Wait Treatment of Rectal Cancer.

BACKGROUND: Chemoradiotherapy has the potential to downsize and downstage tumors before surgery, decrease locoregional recurrence, and induce a complete sterilization of tumor cells for middle and low locally advanced rectal cancer. A watch-and-wait tactic has been proposed for patients with clinical complete response. OBJECTIVE: The purpose of this study was to verify our ability to identify complete clinical response in patients with rectal cancer based on clinical and radiologic criteria.
