Pertussis in Italy: how to protect the "unprotectable"?

Whooping cough continues to be an important public health issue despite high levels of vaccination coverage with acellular pertussis vaccine. Young unimmunized infants represent the most vulnerable group with the highest rates of complications and death. As infant-specific pertussis epidemiologic data, especially among neonates, in Italy were limited, a retrospective observational study of hospitalizations for whooping cough in Italian infants aged <12 months between 2007 and 2018 was conducted to address this knowledge gap. The temporal trend of rates, also stratified for age classes accordin…

[More and faster notification of infectious disease if notification is carried out by the laboratories instead of the diagnosing physician].

OBJECTIVE: To investigate if notification by the laboratories of a number of Group B diseases (meningococcal disease, whooping cough and hepatitis A) which according to the Communicable Disease Law of 999 must be reported to the local public health authority, would increase the number of reports and the speed at which the reports were made. DESIGN: Comparative study. METHOD: In the participating regions (Den Bosch and north-east Brabant) it was decided that the laboratories would notify the authorities of cases ofmeningococcal disease, whooping cough and hepatitis A. The notifications were c…

A 10-year review of morbidity from childhood preventable diseases in Nigeria: how successful is the expanded programme on immunization (EPI)? An update.

Morbidity and mortality in children of developing countries are primarily due to preventable infectious diseases such as measles, poliomyelitis, tuberculosis, whooping cough, diphtheria, and tetanus. By 1990 WHO hopes to have every child in the world immunized against these six diseases, that was why the Expanded Programme on Immunization (EPI) was launched. In Nigeria, a nationwide execution of EPI began in 1979. In view of the huge population of Nigeria, an evaluation of the efficiency of the EPI programme at reducing morbidity and mortality from the six target diseases has national and glob…

[Progress in the development of pertussis-containing vaccines and immunization strategies].

The resurgence of pertussis has become a global public health challenge. Consequently, pertussis prevention and control face new challenges. Despite high vaccination coverage with pertussis-containing vaccines, the incidence rate of pertussis has increased significantly in recent years. This article reviews the research and development (R&D) progress of whole-cell pertussis vaccines, acellular pertussis vaccines (including co-purified and component-purified pertussis vaccines), and novel pertussis-containing vaccines. It also reviews the evolution and research progress of immunization strategi

[A case of Reye's-like syndrome due to suspected Bordetella pertussis infection in an adult].

We report a rare case of Reye's-like syndrome associated with suspected pertussis infection. A 26-year-old woman admitted comatose and found in laboratory studies to have acute liver dysfunction, severe hypoglycemia and prolonged prothrombin time, was diagnosed with clinical Reye's-like syndrome due to aspirin use. Her child was probably infected with pertussis, which she contracted and which, in turn, triggered Reye's-like syndrome.

[Adult pertussis: an emerging clinical problem requiring urgent attention].

Against the backdrop of a global resurgence of pertussis, China has also experienced an increase in reported pertussis cases over the past decade. Pertussis poses a significant threat to children, particularly young infants, with severe cases and fatalities predominantly occurring in babies under three months old. Transmission to this vulnerable age group is frequently traced back to adults with pertussis within household settings. However, pertussis in adults is often underdiagnosed and overlooked, meaning they are a primary source of infection for infants. To enhance clinicians' awareness of

Attack Rate, Case Fatality Rate and Predictors of Pertussis Outbreak During Pertussis Outbreak Investigation in Ethiopia: Systematic Review and Meta-Analysis.

BACKGROUND: Pertussis, a highly contagious, vaccine-preventable respiratory infection caused by Bordetella pertussis, is a leading global public health issue. Ethiopia is currently conducting multiple pertussis outbreak investigations, but there is a lack of comprehensive information on attack rate, case fatality rate, and infection predictors. This study aimed to measure attack rates, case fatality rates, and factors associated with pertussis outbreak. METHODS: This study conducted a systematic review and meta-analysis of published and unpublished studies on pertussis outbreaks in Ethiopia f

[A pertussis outbreak in a ward for severely retarded].

In June 2000, many cases with persistent cough were observed among inpatients and the staff of a ward for severely retarded. Some of them had symptoms suggestive of pertussis, such as whooping, post-tussive apnea. We performed a retrospective investigation to assess symptoms and serological findings suspicious of pertussis. There were a total of 14 cases of persistent cough over 3 weeks (4 to 9 weeks). 6 cases were inpatients and 8 were hospital staff. Of those, serological test for pertussis infection was performed in 10 cases and 6 cases were diagnosed as serologically confirmed pertussis. T

Acute Bronchitis.

Cough is the most common illness-related reason for ambulatory care visits in the United States. Acute bronchitis is a clinical diagnosis characterized by cough due to acute inflammation of the trachea and large airways without evidence of pneumonia. Pneumonia should be suspected in patients with tachypnea, tachycardia, dyspnea, or lung findings suggestive of pneumonia, and radiography is warranted. Pertussis should be suspected in patients with cough persisting for more than two weeks that is accompanied by symptoms such as paroxysmal cough, whooping cough, and post-tussive emesis, or recent …

[Humoral factors of general and local immunity in diabetes mellitus].

Considerable changes in the humoral factors of the general and local immunity were revealed during examination of 53 patients. A raised IgA and IgM level and a lowered titer of normal antibodies to antitoxins of diphtheria and tetanus and to staphylococcus antigen were revealed in the patients' blood. The content for IgG, lysozyme and normal antibodies to the antigens of dysentery and whooping cough was normal. A significant rise of the level of IgA, IgG, titers of normal antibodies and a decrease in the lysozyme activity were observed in the saliva.

Genomic evidence and virulence properties decipher the extra-host origin of Bordetella bronchiseptica.

Until recently, members of the classical Bordetella species comprised only pathogenic bacteria that were thought to live exclusively in warm … um of mammals, including humans. The first identified member was Bordetella pertussis, which causes whooping cough, a fatal disease in young children. In due course, B. bronchiseptica was recovered from the trachea and bronchi of dogs with distemper. Later, a second closely related human pathogen, B. parapertussis, was described as causing milder whooping cough. …

The O antigen is a critical antigen for the development of a protective immune response to Bordetella parapertussis.

Despite excellent vaccine coverage in developed countries, whooping cough is a reemerging disease that can be caused by two closely related pathogens, Bordetella pertussis and B. parapertussis. The two are antigenically distinct, and current vaccines, containing only B. pertussis-derived antigens, confer efficient protection against B. pertussis but not against B. parapertussis. B. pertussis does not express the O antigen, while B. parapertussis retains it as a dominant surface antigen. Since the O antigen is a protective antigen for many pathogenic bacteria, we examined whether this factor is…

A newborn with pertussis accompanying nephrotic syndrome.

Pertussis or whooping cough is a vaccine-preventable disease that still remains a serious infection in neonates and young infants. The disease is particularly severe in infants less than three months old, who are often infected by their parents. Congenital nephrotic syndrome is a rare entity presenting within the first three months. It encompasses a heterogeneous group of entities with genetic, infectious and idiopathic etiologies. In this report we describe a newborn infant who presented with congenital nephrotic syndrome secondary to Bordetella pertussis infection.

Clinical progress note: Pertussis.

Pertussis, commonly known as whooping cough, is a vaccine-preventable respiratory disease with rising incidence due to declining vaccination rates and waning immunity. The most severe disease course is seen in unvaccinated or incompletely vaccinated infants less than 2 months of age, accounting for the largest burden of hospitalization and death, but adolescents and adults play an important role in pertussis transmission and outbreaks. Prompt recognition and diagnosis remain critical for hospitalists in the management of pertussis disease and prevention.
