Outcome after tendon transfers to restore wrist extension in children with brachial plexus birth injuries.

Children with brachial plexus birth injuries often require tendon transfer to restore active wrist extension and maximize hand function. The purpose of this study is to assess the clinical results in children with brachial plexus birth injuries after tendon transfer to reconstruct active wrist extension. Over a 10-year period, 21 children (11 male, 10 female) underwent tendon transfer to reconstruct active wrist extension by a single surgeon. Eight patients had C5/C6/C7 injury and 13 patients had global palsy (C5-T1). The average age at surgery was 5.5 years (range, 3 to 8 y). Restoration of w

Detecting scaphoid fractures in wrist injury: a clinical decision rule.

INTRODUCTION: The aim of this study was to develop and validate an easy to use clinical decision rule, applicable in the ED that limits the number of unnecessary cast immobilizations and diagnostic follow-up in suspected scaphoid injury, without increasing the risk of missing fractures. METHODS: A prospective multicenter study was conducted that consisted of three components: (1) derivation of a clinical prediction model for detecting scaphoid fractures in adult patients following wrist trauma; (2) internal validation of the model; (3) design of a clinical decision rule. The predictors used w

Infrequent physeal wrist injury of the ulna and radius: a case report.

We describe the case of a 13-year-old patient with a left wrist physeal fracture of the ulna and radius. X-Ray showed a Salter-Harris type I … ction. We believe that this clinical case illustrates the importance of carefully assessing physeal wrist injuries when they occur, especially in those cases involving the ulna and the radius. Diagnostic methods such as CT and MRI scans are indispensable for through evaluation and treatment of the injury.

A validation study for a clinical decision rule for acute wrist injury.

PURPOSE: Acute wrist injury is a common reason for visiting the emergency department. To date, there are no implemented clinical decision rules to predict a fracture in this group of patients. We previously identified six clinical predictors in adult patients with acute wrist trauma. The aim of this study was to validate these predictors as a decision rule in a validation cohort. METHODS: This prospective cohort study was conducted in the emergency department at five hospitals in the Netherlands and included adults with acute wrist injury. All collaborating physicians performed a standardized

Injury risk function for the small female wrist in axial loading.

Previous experiments with human cadavers and side airbags revealed the potential for wrist injuries as a result of the hand becoming entrapped in the handgrip. The purpose of this paper was to develop an injury tolerance for the small female wrist that may be used in the design phase of side airbags in order to reduce the risk of wrist injuries resulting from side air bag deployment. Small female cadaver upper extremities were used to develop the wrist tolerance as a conservative estimate of the most vulnerable section of the driving population. The energy source was a pneumatic impactor that …

Fractures of the scaphoid bone. How to handle this common wrist injury.

A combination of forces makes the scaphoid bone susceptible to fracture. The common mechanism of trauma is a force applied to the palmar aspect of the wrist while it is in extreme dorsiflexion. Because early fracture diagnosis by x-ray study is difficult, all patients with a suspicious wrist injury and tenderness in the scaphoid region should be treated as if they have a fracture until radiographs at two and four weeks are normal. Treatment usually consists of immobilization using a long or short plaster arm cast. Various treatment methods, including electrical stimulation, are used when nonun

The association of hand and wrist injuries with other injuries in multiple trauma patients. A retrospective study in a UK Major Trauma Centre.

BACKGROUND: Approximately 20,000 major trauma cases occur in England every year. However, the association with concomitant upper limb injuries is unknown. This study aims to determine the incidence, injury pattern and association of hand and wrist injuries with other body injuries and the Injury Severity Score (ISS) in multiply injured trauma patients. METHODS: Single centre retrospective study was performed at a level-one UK Major Trauma Centre (MTC). Trauma Audit and Research Network (TARN) eligible multiply injured trauma patients that were admitted to the hospital between January 2014 and…

Novel Presentation of Uncommon Wrist Injury: Simultaneous Lunate and Perilunate Fracture Dislocation (scapho-capitate Syndrome) of Both Wrists.

INTRODUCTION: Perilunate and lunate dislocations are uncommon high energy injuries and have extensive soft tissue, cartilaginous and bony damage. The most common pattern is trans-scaphoid perilunate fracture dislocation which constitutes approximately 50% of these injuries. Unusual injury pattern like Scaphocapitate syndrome can occur with fracture through scaphoid waist and capitate that rotates the proximal capitate 180° so that its proximal articular surface points distally. With this case report, we would like to present a novel presentation of an uncommon wrist injury. CASE REPORT: We pr

Imaging of hand and wrist injuries in alpine skiing: common injury mechanisms, imaging findings, and treatment.

Alpine skiing is associated with characteristic patterns of hand and wrist trauma, which together account for approximately 11% of all injuries in this sport. These injuries typically result from high-velocity falls, pole-related mechanisms, and collisions with infrastructure, including lift pylons and barriers. This review provides an overview of common hand and wrist injuries in alpine skiers, with emphasis on skiing-specific mechanisms, characteristic imaging findings, strengths and limitations of different imaging modalities, as well as clinically relevant implications. Although some of th…

National Estimates of Work-Related Emergency Department-Treated Finger, Hand, and Wrist Injuries, U.S. 2015-2022.

INTRODUCTION: Work-related injuries often occur to fingers, hands, and wrists; however, research on risk factors is limited. The aim of this study was to analyze (in 2024) national estimates and trends of work-related fingers, hands, and wrists emergency department-treated traumatic injuries (2015-2022). METHODS: The National Electronic Injury Surveillance System-Occupational Supplement was used to analyze fingers, hands, and wrists injuries. Estimates and rates per 10,000 full-time equivalent workers were calculated using employment labor force estimates from the Current Population Survey. P

Surgical Treatments for Scapholunate Ligament Injuries and Development of Arthritis in the Wrist: A Systematic Review.

PURPOSE: The scapholunate ligament (SLL) is the most injured carpal ligament; however, surgeons do not agree on the best management. Several studies have reported on the after surgery functional outcomes of different surgical techniques, with varying results. However, there is little literature on the development of arthritis in the context of a SLL injury. The goal of this review was to examine the current literature to investigate the relationship between an SLL injury and the development of arthritis in the wrist and hand. METHODS: We performed a systematic literature review on surgical tr

The influence of psychological factors on outcomes following wrist and hand musculoskeletal injuries: A systematic review.

BACKGROUND: Hand and wrist injuries are prevalent worldwide, impacting athletes and the general population, especially older adults, and may lead to long-term pain and disability. Psychological factors are increasingly recognized as key contributors to symptom persistence in various musculoskeletal conditions. PURPOSE: To determine whether psychological factors influence pain intensity, disability, handgrip strength, range of motion (ROM) limitations, quality of life, return to work (RTW), and/or return to sports in adults with musculoskeletal traumatic injuries of the hand or wrist. STUDY D…

Outcomes of radiocarpal pinning to facilitate nerve repair in wrist-level ulnar nerve injuries with defect.

BACKGROUND: The ideal scenario for ulnar nerve repair is primary end-to-end neurorrhaphy in a tension-free environment. However, this could be complicated by soft tissue loss, scarring, and neuroma formation in a delayed injury, creating a nerve defect. With a wrist-level nerve defect, a flexion position can help shorten the nerve gap; however, maintaining the position can be challenging intraoperatively and postoperatively. METHODS: Previously, we proposed our method of using a 1.6-mm K wire for radius-lunate-capitate pinning of the wrist in flexion to minimize the nerve gap, thereby facilit
